Description
The invention relates to medicine, namely, cardiology. It can be used in the treatment of patients with myocardial infarction.
There are many ways to treat myocardial infarction, but they are still far from perfect, and in some cases lead to side effects, are accompanied by serious complications, which made it necessary to develop new methods.
The essential characteristics of the analogue and the invention, which coincide, are as follows: the appointment of intravenous administration of a glucose-insulin-potassium mixture to the patient in the acute period of myocardial infarction.
This method is not effective enough because, although its use allows you to significantly reduce the number of arrhythmic complications and mortality in the acute period of myocardial infarction, it does not lead to the elimination of thrombosis of the coronary infarct-dependent artery - the main factor in the development of myocardial infarction.
The closest in technical essence and the result achieved is the method, which consists in prescribing a patient with a myocardial infarction from 10-11 hours after the onset of the disease, a glucose-insulin-potassium mixture, fibrinolytic agents, heparin, aspirin and nitrates. Common essential features of the prototype and the claimed invention are the following: - administration of the glucose-insulin-potassium mixture as an intravenous drip in the acute period of myocardial infarction; - simultaneous appointment of reperfusion therapy in the form of systemic thrombolysis, as well as heparin, aspirin and nitrates.
This method is not effective enough, because the introduction of the mixture begins at 10-11 hours of the disease (late introduction), when there is almost no viable myocardium left in the area of the infarct-dependent artery. Therefore, complications associated with reperfusion of the ischemic myocardium (reperfusion arrhythmias) and rethrombosis are not prevented when treatment is carried out in this way and require a number of additional measures.
The invention is based on the task of improving the method of treating myocardial infarction by changing the timing of administration and the composition of the mixture, which will increase the effectiveness of the treatment and reduce the number of complications.
The task is solved by the fact that in the method, which includes the appointment of the patient with an intravenous drip of a glucose-insulin-potassium mixture, fibrinolytic agents, heparin, aspirin, nitrates, the new thing is that 1 Oml of 2595 magnesium sulfate solution is added to the composition of the mixture in a ratio of 1:50 , and the mixture is administered in the first 60-180 minutes from the moment of myocardial infarction, before the start and simultaneously with systemic thrombolysis for 60 minutes.
The cause-and-effect relationship between the set of declared signs and the technical result is as follows.
The appointment of magnesium in this way will prevent the majority of reperfusion complications associated with energy deficiency and impaired calcium removal from the cardiomyocyte, activation of cellular calcium-dependent proteases and lipases.
Magnesium, which is a natural antagonist of calcium, causes vasodilation, has disaggregant activity, inhibits the automaticity of partially depolarized cardiomyocytes, protects the myocardium from calcium overload in conditions of ischemia and, especially, in the period of reperfusion.
Early (in the first 60-180 minutes after the onset of a heart attack) intravenous administration of a glucose-insulin-potassium-magnesium mixture together with a fibrinolytic drug probably reduces the number of episodes of fluctuating paroxysmal ventricular arrhythmia, the total number of ventricular extrasystoles, the number of high-grade ventricular extrasystoles (according to V. And ohm/p) according to daily ECG monitoring data.
При застосуванні даного способу лікування суттєво зменшується госпітальна летальність і складає 3-595, тоді як при використанні лише фібринолітичних засобів - 5-795, без застосування фібринолітичних засобів і суміші - 14- 1796.When using this method of treatment, hospital mortality is significantly reduced and is 3-595, while when using only fibrinolytic agents - 5-795, without the use of fibrinolytic agents and the mixture - 14-1796.
The number of patients with the formation of a post-infarction aneurysm and acute heart failure is likely to decrease.
Thus, the combination of the above-mentioned positive effects of the proposed method of treatment of patients with myocardial infarction will increase the effectiveness of treatment, speed up the recovery of patients, and reduce the number of complications.
The method is carried out as follows.
In the first 60-180 minutes after the onset of a myocardial infarction, the patient is prescribed a mixture of 500 ml of 5905 glucose solution, 40 ml of 495 potassium chloride solution, tOml of 2595 magnesium sulfate solution, 6 units of insulin simultaneously with systemic thrombolysis with the help of intravenous administration of kabikinase in a dose of 1.5 million MO for 30 minutes. The infusion of the mixture was continued for another 30 minutes. The general term of introduction of the mixture bOkhv. On the first day of the disease, an intravenous infusion of Isoket (20 mg), aspirin 325 mg (chewed) is prescribed once, and then daily: aspirin (125 mg), Cardiket (60-120 mg). 6 hours after the end of thrombolysis, heparin 1000 U/h was prescribed intravenously for the first day, followed by subcutaneous injection 4 times a day. The dose of heparin was changed in accordance with the level of AChT (within 1.5-2 times higher than the derived values).

Hvoriy N., born in 1945. (medical history Me1452), was under inpatient treatment in the infarct department of the city clinical hospital of extreme and emergency medical care from 10.03.2000 to 25.03.2000, where, according to WHO criteria, a diagnosis was made: CHD: large-focal anterior myocardial infarction (10.03. 2000); acute heart failure of the 1st degree (according to T. Kiiiir) (10.03.2000); ventricular extrasystole of the 2nd class (according to V.I ohm/p) (10.03.2000) CH Hist., FC I.. After the examination, he was prescribed treatment according to the proposed method, namely: in the first 60 minutes from the moment of myocardial infarction a mixture of 500 ml of 595 glucose solution, 40 ml of 4905 potassium chloride solution, 1Oml of 2595 magnesium sulfate solution, 6 units of insulin simultaneously with systemic thrombolysis with the help of intravenous kabikinase in a dose of 1.5 million IU for 30 minutes.
The infusion of the mixture was continued for another 30 minutes. On the first day of the disease, an intravenous infusion of Isoket (20 mg), aspirin 325 mg (chewed) was prescribed once, and then daily: aspirin (125 mg), Cardiket (120 mg). 6 hours after the end of thrombolysis, heparin 1000 U/h was prescribed intravenously for the first day, followed by subcutaneous injection 4 times a day. The dose of heparin was changed according to the level
Реперфузія в перші З години супроводжувалася лише шлуночковими екстрасистолами 2-ї градації (заAPTCH (in the range of 1.5-2 times higher than the derived values). The initial level of elevation of the 5T segment in the M2-UZ leads was 0.btu, and 3 hours after the end of the infusion of the thrombolytic agent and the proposed mixture - 0.2tM. Reperfusion in the first 3 hours was accompanied only by ventricular extrasystoles of the 2nd degree (according to
В. V. Gomp). According to daily ECG monitoring, no other rhythm disturbances were registered. The next period passed without complications. According to echodopplercardiographic research, dilation of the left ventricular cavity, aneurysm formation, systolic or diastolic dysfunction of the left ventricle, and violations of the pumping function of the heart have not been established. On the 15th day of the disease, he was transferred to the cardiology department at his place of residence for further treatment and rehabilitation.
