Description of the invention
The invention relates to medicine, and in particular to the diagnosis of hereditary pathology. It can be used in medical and genetic counseling of families affected by Gaucher disease. Gaucher disease (GH) is a hereditary disease with an autosomal recessive type of inheritance, which is caused by a decrease in the activity of one of the lysosomal enzymes - glucocerebrosidase. Due to the lack of activity of glucocerebrosidase, the main chain of degradation of pathological metabolites of glucocerebrosides is disrupted, which leads to their accumulation in various tissues and organs. Clinical signs of HG are progressive hepatosplenomegaly, thrombocytopenia, 70 anemia, bone pain, and gradual replacement of bone marrow cells by macrophages that are loaded with glycolipids (Gauche cells). Modern methods of enzyme replacement therapy have been developed for HCG, the success of which depends on early diagnosis and timely initiation of treatment.
There is a known way of diagnosing HG using the molecular genetic method ||. For this, DNA is isolated, gene fragments are amplified using the polymerase chain reaction and the presence of mutations in 19 of these fragments is analyzed. The use of the molecular genetic method is complicated by the probable presence of a new, so far undescribed mutation in the glucocerebrosidase gene in some patients.
There is a known method of diagnosing HCG by determining the activity of glucocerebrosidase in the homogenate of leukocytes |2). To do this, leukocytes are isolated from 3-bml of venous heparinized blood according to the standard method. 3) To 0.2 ml of leukocyte homogenate, add 0.2 ml of a substrate mixture of the following composition: bmM-glucopyranoside in 0.2 M citrate phosphate buffer, pH 5.4, containing 0.195 taurocholate of sodium and 195 tritons
Х-100. X-100. The samples are incubated at 37"C for 1 hour. The reaction is stopped by adding 0.2 M glycine buffer pH 0.6. The amount of released substrate is estimated by determining the optical density of the samples at 515 nm.
Calibration is carried out using standard solutions of 4-methylumbelliferone. Results are expressed in nmol/h/mg protein. The amount of protein in the homogenate of leukocytes is determined by the standard method of Christian et al
Warburg (4). The obtained indicators are compared with the range of normal values of glucocerebrosidase activity and the range of values of glucocerebrosidase activity in HCG.
оThe disadvantage of this method is that when determining the activity of glucocerebrosidase, there is a significant overlap of the activity values of this enzyme in the group of patients with Gaucher disease (0.2 - 6.2 nmol/h/mg of protein) and in the control group (5.1-13, 3nmol/h/mg of protein). at
Fig. 1 presents a diagram of the distribution of glucocerebrosidase activity values in different groups of subjects examined (16 healthy donors and 18 patients with Gaucher disease were examined). The presence of a zone of overlap of areas of glucocerebrosidase activity in these groups significantly complicates the interpretation of indicators falling into overlapping zones. co

Gaucher among similar pathological conditions of other genesis for highly effective medical and genetic counseling of burdened families and timely implementation of enzyme replacement therapy.
The set task is achieved by the fact that patients with a borderline value of glucocerebrosidase activity of 5.1-6.2nmol/h/mg of protein additionally determine the activity of chitotriosidase in the blood plasma and "the obtained indicators are compared with the areas of normal and pathological values, and, if the limits of activity of this Z 50 enzyme in the norm and with CG do not overlap, Gaucher's disease is diagnosed. c The method is carried out as follows.
From" Plasma is obtained from heparinized blood according to the standard method according to the prototype. 22μM 4-MUF-triacetylchitotrioside in citrate-phosphate buffer pH 5.2 is used as a substrate. The reaction mixture is prepared by mixing μl of plasma and 100 μl of substrate. Samples are incubated at 37"C for 1 hour. Figure 2 shows a diagram of the distribution of chitotriosidase activity values in different groups of subjects and individuals (16 healthy donors and 18 patients with Gaucher's disease were examined) obtained by the claimed method. It can be seen from the diagram 20 that the areas of chitotriosidase activity values of the control group (0-150 nmol/h/ml of plasma) and those with CG (3000-30000 nmol/h/ml of plasma) do not overlap. This makes it possible to identify individuals with Gaucher disease with high reliability.
Example.
Patient V., 10 years old, medical history Mo20718 Referred to medical and genetic counseling with 52 previous diagnosis: hepatosplenomegaly of unknown etiology. During the clinical examination, the following was noted: the condition of the child is severe, with pronounced signs of hepatosplenomegaly, thrombocytopenia, anemia. When determining the activity of glucocerebrosidase in the patient, the following results were obtained: Patient B. - b.2nmol/h/mg of protein. When comparing the results with the range of normal values, it can be seen that the glucocerebrosidase activity of patient B. is within normal values, which casts doubt on the presence of Gaucher disease in the patient. To check the presence of Gaucher's disease in this patient, chitotriosidase activity was determined according to our proposed method. When determining the activity of chitotriosidase, the following results were obtained: Patient
В. B. - 27404 nmol/h/ml. Thus, chitotriosidase activity in patient B. is much higher than normal values, which made it possible to confirm the presence of Gaucher disease in patient B.
