Description of the invention
The invention relates to medicine, in particular to neonatology. 2 Despite the introduction of new methods of prevention and treatment in recent years, intrauterine hypoxia of the fetus and acute asphyxia of newborns ranks second in the structure of perinatal morbidity and mortality. Moreover, damage to the cardiovascular system is registered in 40-7090 newborns who suffered perinatal hypoxia.
Fetal hypoxia contributes to the development of heart muscle damage of varying depth and severity, with a decrease in its contractility, and a disruption in the functioning of the conduction system and coronary vessels. In the conditions of perinatal hypoxia, the activation of lipid peroxidation occurs, which causes a violation of the permeability of both cell membranes and the membranes of their organelles. After the elimination of oxygen deficiency, these phenomena do not allow to increase the level of redox processes in the myocardium, including due to hypoxic damage to energy enzymes and the inability of cells to assimilate blood oxygen. All this 72 causes a number of functional and morphological disorders in the tissues, with their subsequent realization in more serious damage to the heart muscle as a final result, which can serve as a cause of mortality in newborns, and in the future as a basis for the formation of cardiovascular pathology in older children age and adults.
2,A known method of treating hypoxic damage to the myocardium of newborns is the use of the metabolic drug Cytomak intravenously at a dose of 0.5-1.00 ml/kg of body weight for 7-10 days, which improves energy exchange in the myocardium (Prakhov A.V. "Effectiveness of intravenous administration of Cytomak with the syndrome of transient myocardial ischemia in newborns". Russian Journal of Perinat. and Pediatrics, 1996, Mo2,
С.17-19).P.17-19).
The disadvantage of this method is that cytomak is not used in patients with increased contractile capacity of the myocardium in combination with high (or reduced) indicators of central hemodynamics. At the same time, there is a category of patients who have such indicators.
43-44).A known method of treatment of hypoxic damage to the myocardium of newborns is mildronate internally at a dose of 50-100mg/kg of body weight in With taking it for 10-14 days normalizes indicators of central and intracardiac hemodynamics (Kryvopustov S.P. "Experience of using mildronate in the pharmacocorrection of ee, post-hypoxic cardiac disorders - of the vascular system in newborns". Pediatrics, obstetrics and gynecology, 1996, Mo5-6, P.43-44).
The disadvantage of this method of treatment is that its use is limited by the presence of contraindications in the form of increased indicators of central hemodynamics and contractile function of the myocardium. And it is exactly this Ge») tendency of these indicators that occurs in the majority of patients at the stage of compensation. Use of mildronate
This leads to their growth, which is an undesirable effect (the heart works anyway under the stress of compensatory and adaptive mechanisms) and can lead to a failure of compensation.
«The goal of the proposed method of treatment is to optimize the cardioprotective therapy of severe hypoxic damage to the myocardium in newborns. "
The task is achieved by additionally prescribing a membrane stabilizer, for example, a liposomal drug Z lipin at a dose of 10-15 mg/kg of body weight 2 times a day intravenously for 7-10 days. The method is carried out as follows. z" A newborn who has clinical and instrumental manifestations of a pronounced degree of hypoxic damage to the myocardium is prescribed intravenous lipin at a dose of 10-15 mg/kg of body weight 2 times a day. The course of treatment is 7-10 days.
Відновлення маси тіла відмічено на 6 добу (у групі порівняння наMv, Mea- leads, a shift of VT below the isoline by 1.5 mm in the III standard and M 5, Me in -leads, lengthening of the 29 corrected OT interval to 466b, b/msec, signs of overloading of the right ventricular myocardium. Against the background of decreased indicators of central hemodynamics (ULSh - 5.94 ml, XO - 535 ml/min), an increase in the contractile function of the myocardium (ФВ-8595, ФО-5095) was registered, the diagnostic index of ischemic damage to the myocardium was 2.08. The level of the cardiospecific isozyme MV - creatine phosphokinase increased to 1.5μkat/l, MDA - to 138μmol/ml erythrocytes, lipid hydroperoxides - to 1.08μmol/ml erythrocytes. In addition to 60 traditional drugs, the complex of therapy includes lipin at a dose of 1 mg/kg body weight twice a day intravenously for 7 days. From the second day of treatment, there were positive changes in the patient's condition: acrocyanosis disappeared, pathological reflexes were eliminated, some unstable physiological reflexes began to occur, muscle tone improved, the heart rate stabilized, the sonority of the tone increased, the size of the liver decreased. In a child with stable hemodynamics, on the 5th day, the normalization of ECG indicators, contractile and pumping function of the myocardium, because of the diagnostic index, and laboratory data was noted. Recovery of body weight was noted on the 6th day (in the comparison group on
8.01-41.02 days), discharged from the hospital on the 20th day (comparison group - on 21.725-10.52 days) in satisfactory condition. No pathology was detected during the subsequent examination after 3 months.
Experimental and clinical approval of the treatment method was carried out on the basis of the neonatal resuscitation department of the Institute of Pediatrics, Obstetrics and Gynecology of the Medical Academy of Ukraine in more than 30 cases.
Thus, the declared method of treatment of a pronounced degree of hypoxic damage to the myocardium of newborns allows to achieve faster elimination (compared to the control group that received traditional therapy) of pathological symptoms of the disease, faster normalization of laboratory indicators that characterize the intensity of lipid peroxidation processes, normalization of the level of cardiospecific 7 /0 enzymes, indicators of the contractile and pumping function of the heart, and as a final result - a decrease in mortality and the number of complications in patients with a pronounced degree of hypoxic damage to the myocardium. The results of the observations indicate the expediency of including membrane stabilizers, such as the liposomal drug lipin, in the complex therapy of a pronounced degree of hypoxic myocardial ischemia in newborns who have undergone perinatal hypoxia.
