Description of the invention
The invention relates to medicine, namely to cardiology, and relates to the treatment of ischemic heart disease 2 complicated by heart rhythm disturbances.
For a long time, adenine compounds in the myocardium were considered exclusively as an intracellular source of metabolic energy, despite the fact that as early as 1929 the active participation of adenosine and ATP in the regulation of cardiac activity was demonstrated. Recent studies have confirmed that adenine nucleotides are physiological regulators of coronary blood circulation, possess pronounced antiarrhythmic activity, antiadrenergic effect, 70 cause well-controlled hypotension with intravenous drip administration, and also exhibit antihypoxic effects. The bright positive properties of adenosine and ATP are combined with the non-toxicity of exogenous drugs and the complete safety of their use in reasonable doses, as they are natural substrates of energy metabolism. The short-term pharmacological effect should be considered the only insurmountable drawback of ATP, as well as ATP-LONG. With intravenous bolus administration of 195 ATP solution to patients with 12 paroxysmal tachycardia, the slowing of atrioventricular conduction continues for 4-7 seconds, which is rationally used to stop the paroxysm. Adenosine's ability to reduce total peripheral vascular resistance by reducing vascular tone has been successfully used for controlled hypotension. But when the drip infusion of the adenosine solution is stopped, blood pressure suddenly rises, which is associated with its very rapid degradation to inosine and hypoxanthine. The short-term pharmacological effects of ATP and adenosine are successfully used for the treatment of paroxysmal tachycardia and controlled hypotension, but it is a negative property of these drugs that limits their use in arrhythmias and coronary heart disease.
ATP-LONG is distinguished by a longer duration of antianginal activity, possibly due to coordination bonds of ATP with histidine, potassium and magnesium. It is known that the original structure of the molecule
оATP-LONG determines its characteristic pharmacological properties, which are not possessed by its separate chemical components. However, 24-hour Holter ECG monitoring proved that the anti-anginal effect of ATP-LONG lasts no more than 2 hours. It is obvious that its use in the form of 3 doses of sublingual tablets cannot ensure a stable concentration of ATP in the body, and therefore a constant therapeutic effect. at
сThe prototype of the proposed solution is unknown. with
The basis of the invention "Method of prolonging the duration of action and increasing the antiarrhythmic, antianginal and antihypertensive effectiveness of ATP-LONG" is the task set through the combined use of Z
ATP-LONG of another pharmacological drug to increase its antiarrhythmic, antianginal and antihypertensive activity, as well as prolong the duration of the drug's positive effects on myocardial cells. 3o This is achieved by the fact that after taking ATP-LONG, patients are injected intravenously with a 295 solution of fish-syn o (inosine) at 10-20 ml in 200 ml of 0.995 sodium chloride solution.
We conducted experimental studies of natural substrates of energy metabolism, in particular inosine, which is a product of adenosine degradation. In the process of further synthesis of adenine nucleotides, inosine and hypoxanthine are again converted into ATP using metabolic energy in the cells. Therefore, inosine and its exogenous analogue riboxin penetrate into myocardial cells, increasing the energy balance of the myocardium, which allows prolonging the effect of ATP on receptors through a modulating effect on purinergic receptors, which was established by us for the first time. In addition, riboxin, degrading to hypoxanthine and entering the cells, leads to a rapid (on average after 4-5 minutes in humans) increase in the concentration of 2,3-diphosphoglyceric acid in erythrocytes, which leads to a certain decrease in the affinity of hemoglobin to oxygen and an improvement in its utilization during oxidation of energy-intensive substrates. Under the same conditions, there is an increase in the arterio-venous difference in oxygen, which reflects the antihypoxic properties of the drug. Penetrating into cells, riboxin

The method is used as follows: in the morning, the patient takes one sublingual tablet of Shk ATP-LONG (2Omg), after which 10-20 ml of a 295 solution of riboxin in 200 ml of a 0.995 solution of sodium chloride ka 20 is injected into his vein. The infusion is carried out by the drip method for one hour. After 30-40 hours from the start of the infusion, the patient takes the ATP-LONG tablet one more time. At noon and in the evening, 1-2 tablets of ATP-LONG are prescribed under the tongue, after which the patient again takes riboxin in 40mg tablets. The course of treatment continues for 10-15 days.
In mild forms of heart disease, riboxin is prescribed in tablets from the very first day.
