Description of the invention
The invention relates to medicine, namely to orthopedics and medical radiology, intended for 2 diagnosing osteoporosis, determining the risk of low-traumatic fractures, as well as evaluating the effectiveness of treatment and preventing the development of pathological changes in the bone system. The invention can be used to detect the pathology of the musculoskeletal system, namely the quantitative assessment of changes in the structural and functional state of bone tissue with age, in primary and secondary osteoporosis, before and after treatment. Osteoporosis is the most common systemic disease of the skeleton, which is characterized by low bone mass and microstructural damage to bone tissue, leading to increased bone fragility and increased risk of low-traumatic fractures.
In medical practice, there are methods of using specialized equipment intended for the diagnosis of pathology of the musculoskeletal system, in particular, ultrasonic densitometers, computer tomographs, two-photon X-ray scanners. One of the well-known methods of diagnosing osteoporosis is the method proposed by V. Alyakna, V. Cheremnykh |Comparative assessment of the state of bone tissue by the method of ultrasound osteometry / Osteoporosis: epidemiology, clinic, diagnosis, prevention and treatment.
Materials of the II Ukrainian Scientific and Practical Conference - Kyiv, 1997. - P.11-16). The diagnosis is made on the basis of the computer determination of the optical transmission of the X-ray image of the PI of the carpal bone in relation to the stepped aluminum standard, but this does not ensure the timely detection of systemic osteoporosis, since the loss of compact bone tissue occurs later than the loss of spongy bone tissue.
сThe absence of a fixed position of the stepped standard on the radiograph leads to the occurrence of significant errors due to the heterogeneity of the X-ray radiation in relation to different sections of the radiograph. It should be noted that for this technique, its use is legitimate if there is a linear relationship between the optical density of bone projection zones on X-ray images and the density of the steps of the wedge-standard. with
The closest prototype of the method of assessing the state of bone tissue is the "Method for determining the content of He) mineral substance in bone tissue" - RF patent Mo2136214, but when using it, due to the lack of automation of the measurement, more significant errors are observed and there is no possibility to compare the pictures taken on films different manufacturers under relatively identical development conditions. In addition, the lack of landmarks for scanning leads to the fact that inappropriate areas of bones will be analyzed on different x-rays and different coefficients of parameters comparable to reference values will be calculated for bone density calculations. The closest analogue of the device for performing the method of assessing the state of bone tissue is the "Device for radiological diagnosis of osteoporosis", Application of Ukraine Mo99074319, о
But its use is limited to the assessment of the structural and functional state of only the spongy bone tissue of the central area of the calcaneus, which reduces the diagnostic value in systemic osteoporotic process 39 or when it is impossible to obtain radiographs of the lower extremities - amputation, fracture, etc. In addition, this C device consists of a continuous frame, and the examined area has a more developed layer of soft tissues with a subcutaneous fat layer, which gives an additional shadow from the absorption of the material and component of soft tissues and, thereby, increases the errors in the determination optical parameters on the radiograph. A larger area of "Ff" of radiography with this method increases the radiation load on the patient. With 50 The basis of the proposed invention is the task of more accurate X-ray osteodensitometric diagnosis of osteoporosis and osteopenia due to the quantitative assessment of changes in the structural and functional state of: » bone tissue.
The method of assessing the condition of bone tissue is performed as follows:
The patient's hand is fixed using a device for its fixation (Fig. 1), after which an X-ray of the bones of the metacarpal part of the hand is performed. The image on the standardized X-ray is converted into digital codes for further mathematical processing and determination using a slide scanner 4! optical density of bone tissue using the computer program "OZTIM". Densitometry of the areas of the radiograph in the zone of the proximal and distal epiphyses of the 1st metacarpal bone and on the o projection of the steps of the wedge-standard is carried out, as well as the cortical indices are determined (this is the ratio of the difference between the outer diameter - 0 and the inner diameter to the outer diameter) I, NI, IM, M - Metacarpal bones. After that, we automatically get relative indicators of mineral saturation of bone tissue: mineral and saturation index, integral coefficient of mineral saturation, cortical indices for the above bones and integral cortical index. Based on the average values and their standard deviations of the most indicative parameter - the integral cortical index and based on the recommendations of the WHO regarding the assessment of the 25 structural and functional state of bone tissue and the diagnosis of osteoporosis, the values are obtained

6.Figure 2 shows the design of the device for performing this method. The device consists of: a frame-case 1, for placing a standard cassette with X-ray film 2. The frame has a cuff with a fastener З for fixing the proximal part of the hand, a movable fixator 4 for fixing the distal part of the hand, a window 5 for excluding an additional shadow on the radiograph and stepped aluminum wedge-standard 6.
The method is carried out as follows:
X-ray of the hand is performed on the patient in the "sitting" position. The patient's hand is placed close to the plastic support of the movable fixator 4 and fixed using a cuff with a fastener 3, as a result, its metacarpal part is clearly above the window 5, parallel to the stepped wedge-standard 6. Positioned radiography is performed together with the mounted stepped aluminum wedge. standard 6. Then, according to the method described above, densitometry of the most informative sections of the radiograph is carried out.
The automatically obtained results are presented in tabular form and entered into the report form - the patient's examination card, after which, depending on the determined indicators of the structural and functional state of the bone tissue, a diagnosis is established.
We determined the structural and functional state of bone tissue in 96 women aged 22-82 years. According to the index of bone tissue strength, three groups were identified among the examined: A - patients with osteoporosis, B - with osteopenia, C - with normal bone tissue. The table shows the obtained relative indicators of X-ray osteodensitometry. iv
Thus, in patients with osteoporosis and osteopenia, the determined parameters are probably reduced, compared to patients with normal indicators of the structural and functional state of bone tissue. Ge
The proposed method of diagnosing osteoporosis based on computerized X-ray densitometry is (5) a cheaper, but sufficiently reliable (regarding diagnostic accuracy) alternative option for determining structural and functional changes in bone tissue, based on the complex application of standardized X-ray photography of the peripheral bones of the skeleton and computer densitometry of X-ray images using the system of algorithmic calculations on the hardware and software complex "O5TIMe". The special significance of the proposed invention lies in the fact that, despite the relevance of the problem of osteoporosis and its formidable complications (low-traumatic fractures of the femur, bones of the distal part of the forearm, spine, etc.), there is still no such diagnosis of this disease today. which, on the basis of (а) modernization of traditional radiographic equipment, would allow to standardize the analysis of radiographs and increase the reliability and sensitivity of diagnostics. Research using the proposed method requires a minimum of time, is publicly available for implementation, and allows for remote diagnostics via communication networks. Timely nosological and nosological diagnosis of osteoporosis makes it possible to carry out effective measures for the prevention and treatment of the disease, but the current state of domestic medical technology requires the implementation of the proposed method of diagnosis, which is optimal from the point of view of the ratio - quality / price of diagnosis.
The invention can be used to assess disorders of the structural and functional state of bone tissue, - in the identification of risk groups for the occurrence of osteoporotic fractures, as well as to assess the effectiveness of the prevention and treatment of osteoporosis.
