За сучасними уявленнями, остеоартроз (ОА) - хронічне дегенеративно-дистрофічне захворювання суглобів, що характеризується первинною та прогресуючою деструкцією суглобового хряща, проліферативною реакцією хрящової та кісткової тканин із розвитком крайових остеофітів та супроводжується реактивним синовітом. Серед ортопедичної і ревматичної патології ОА займає провідне місце: 10-1295 населення страждають на цю недугу. В 70 Україні, за даними державної звітності, зареєстровано біля 500 тис. хворих. Вважається що ця цифра значно занижена внаслідок пізнього звертання хворих за медичною допомогою. При ОА відзначається протеогліканова недостатність, що призводить до розволокнення й розщеплення матриксу суглобового хряща, надмірної його гідратації з подальшою дегідратацією, змін процесів дифузії метаболітів, дезорганізації і розриву колагенової мережі. Запропонований спосіб здійснюється наступним чином. На 1-ому етапі хворим з рентгенологічне, сонографічно, артроскопічно підтвердженим діагнозом ранньої стадії остеоартрозу колінного суглоба проводять /5 артроскопічний дебридмент. Для проведення артроскопічних втручань використовують артроскопічний комплекс фірми Каїі 500г2 ОтрН «5 Со. КО (Німеччина). Оперативне втручання виконують під загальним знеболюванням, кінцівка знекровлюють турнікетом. Артроскопію розпочинають через передньо-медіальний і передньо-латеральний доступи промиванням суглоба 300-400мл фізіологічного розчину натрію хлориду, під час чого з порожнини суглоба вимивається змінена синовіальна рідина і продукти деградації. Після цього виконують діагностичний етап операції - ревізію суглоба. Артроскоп послідовно переводять з верхнього завороту через латеральний доступ у латеральний відділ, потім у медіальний відділ. Хворий відзначає суттєве клінічне покращання, вагому регресію больового синдрому, відновлення об'єму рухів, відновлення працездатності, покращення 70 функції суглоба, поліпшення якості життя. Як видно, прояви суглобової декомпенсації суттєво зменшились. За період з 2005 по 2007рр. у клініці травматології та ортопедії Національного медичного університету імені О.О Богомольця запропонований спосіб лікування було апробовано у 29 пацієнтів (середній вік 39,6 -6,7 75 років; жінок -18, чоловіків - 11) з ранніми стадіями остеоартрозу колінного суглоба. Результати лікування порівнювали з ретроспективними даними для групи пацієнтів (11 чоловік), яким проводилася терапія за способом-прототипом (послідовне застосування методів хірургічного (артроскопічний дебридмент) і консервативного лікування (тривале застосування в післяопераційному періоді хондропротектора гіалган і судиннотропного препарату пентоксифілін) без використання мікрофрактур склерозованої субхондральної кістки та дії на вільнорадикальну ланку патогенезу остеоартрозу колінного суглоба. Ефективність комплексного лікування оцінювали при виписці зі стаціонару, а також через З, бміс. і 1 рік. При порівнянні результатів лікування відзначено, що в групі, якій проводилося комплексне етапне лікування запропонованим способом, спостерігались більш швидка позитивна динаміка альгофункціональого індексу Лекена, відновлення об'єму рухів, суттєва регресія больових відчуттів, а також покращання метаболізму суглобового хряща (суттєве зниження концентрації глікозаміногліканів в сироватці крові), та нормалізація процесів ланцюгового вільно радикального -в окислення ліпідів (за даними індукованої хемілюмінесценції проб крові). Як видно, запропонований спосіб лікування хворих з ранніми стадіями остеоартрозу, зменшує прояви суглобової декомпенсації, покращує функції суглоба, поліпшує якість життя пацієнтів, забезпечує тривалу ремісію та гальмує прогресування процесів дегенерації суглобового хряща, що можна пояснити врахуванням вільно радикальної ланки патогенезу с остеоартрозу колінного суглоба та створенням умов для заміщенням фіброзною тканиною глибоких дефектів о хряща після виконання мікрофрактур у цих ділянках під час артроскопічної ревізії колінного суглоба.For the period from 2005 to 2007 in the clinic of traumatology and orthopedics of the O.O. When comparing the results of treatment, it was noted that in the group that underwent complex staged treatment by the proposed method, faster positive dynamics of the Leken algofunctional index, restoration of range of motion, significant regression of pain sensations, as well as improvement of articular cartilage metabolism (a significant decrease in the concentration of glycosaminoglycans in blood serum) were observed vi), and normalization of the processes of chain free radical oxidation of lipids (according to the induced chemiluminescence of blood samples). As can be seen, the proposed method of treating patients with early stages of osteoarthritis reduces the manifestations of joint decompensation, improves joint functions, improves the quality of life of patients, provides long-term remission and inhibits the progression of the processes of degeneration of articular cartilage, which can be explained by taking into account the free radical link in the pathogenesis of osteoarthritis of the knee joint and creating conditions for the replacement of deep cartilage defects with fibrous tissue after microfractures in these areas during arthroscopic revision of the knee joint.
