Description of the invention
A useful model relates to the field of medicine, namely abdominal surgery, and can be used for internal drainage of pancreatic cysts.
Treatment of patients with cystic diseases of the pancreas remains the most problematic section of abdominal surgery.
The range of surgical interventions performed for pancreatic pseudocysts is very wide.
In addition to traditional methods of external drainage of pseudocysts, this operation in some cases can be performed using minimally invasive methods.
Internal drainage of pseudocysts of the pancreas is logical in design, relatively simple and, if performed correctly, an effective method of their surgical treatment.
Indications for the use of various types of internal drainage are: single-chambered, sufficiently mature cysts with a formed capsule; connection of the cyst cavity with the main pancreatic ducts; 72 cystic expansion of the main pancreatic duct. Internal drainage is especially appropriate when pseudocysts are located in the head of the pancreas, as radical operations in these cases are difficult to perform, and external drainage is often complicated by long-standing fistulas.
(11.The closest to the proposed technical solution is the method of treating a pseudocyst of the pancreas, which consists in the introduction of a fibrogastroduodenoscope into the stomach space, dissection with an endoscopic electroknife of the back wall of the stomach and the wall of the pseudocyst soldered to it in the place corresponding to the localization of the liquid component of the pseudocyst, in the introduction through the nose into the lumen of the stomach of the probe, followed by its removal with the help of a fibrogastroduodenoscope into the cavity of the pseudocyst. The location of the liquid component of the pseudocyst is determined under ultrasound control. Then, in the place specified above, under ultrasound control, a puncture of the pseudocyst is performed using a metal needle through the front abdominal wall, the front and back walls of the stomach, after which a flexible conductor is passed through the lumen of the needle into the cavity of the pseudocyst, the needle is removed, and the dissection of the back wall of the stomach with pseudocysts with a wall soldered to it are performed at the entrance of the conductor into the back wall of the stomach (11.
However, the disadvantage of this method is that drainage is performed through the lumen of the stomach under the control of a fibrogastroduodenoscope without manipulators (without the control of a laparoscope), which complicates the operation. o 30 The basis of a useful model is the task of improving the method of treating pancreatic cysts "by miniinvasive internal drainage of it under the control of a laparoscope, which will improve the quality of the operation, reduce the number of complications in the postoperative period, as well as the length of stay of the patient in the hospital. Ha
The task is solved by the fact that, according to a useful model, after determining the location of the 3rd pancreatic cyst, a minimally invasive intervention is performed from standard ports, under the control of a laparoscope, the cyst is punctured with a metal needle, in which a coagulation "path" is performed with the help of an electroknife to stop possible bleeding from walls of the stomach, the front wall of the stomach is dissected with a needle and the edge of the stomach wound is opened with the help of a manipulator, after which the back wall of the stomach and the cyst wall adjacent to it for 3-5 cm are dissected with an electric knife, the metal needle is removed, the contents of the cyst are aspirated, the cavity is treated with 50 with a physiological solution with antibiotics, the wound of the front wall of the stomach is sutured with a single-row wrapping suture, with drainage through the counter-aperture in the right sub-rib, it is brought out to the ventral opening, and a nasogastric tube is inserted into the stomach.
The method is performed as follows.
Under general anesthesia, ports are applied to the patient in standard leads: immediately below the navel 49 for the laparoscope, in both pudribary and epigastric regions for manipulators. Carboperitoneum is created. Under ultrasound control, the cyst is punctured through the front and back walls of the stomach, the contents are aspirated using a syringe. A 3-5 cm long "path" is created on the front wall of the stomach with the help of an electrocoagulator to stop possible bleeding from the vessels of the front wall of the stomach. Along the "path" with an electric knife, the front wall of the stomach is dissected, and the edges of the stomach wound are spread to the sides with the help of manipulators. With a 20-gauge metal needle inserted into the cyst cavity, the back wall of the stomach and the wall of the cyst adjacent to it are dissected with an electric knife. The length of the dissection is 3-4 cm. The metal needle is removed. The cyst cavity is washed with a physiological saline solution with an antibiotic and the contents of the cyst are removed. Then, the front wall of the stomach is sutured with a continuous wrapping suture. The abdominal cavity is cleaned and a drainage tube with a diameter of 0.5-0.7 cm is inserted through the counter-aperture in the right sub-rib. Instruments are removed from the abdominal cavity. Wounds of the abdominal wall are sutured and stickers are placed on them. c In total, patients with a pancreatic cyst with a good postoperative result, the postoperative period without complications, were treated.
