Description of the invention
A useful model relates to medicine, namely to surgery and intensive care and can be used 2 in the treatment of arterial insufficiency of the lower extremities.
24311.A well-known method of treating arterial insufficiency of the lower extremities consists in catheterization of the femoral artery according to the Seldinger method followed by alternating forced administration of infusate and slow suction of an equal volume of blood followed by reverse rapid administration | patent of Ukraine Mob24311.
Проте, у відомому способі не враховують співвідношення рівня стояння катетера /його внутрішнього робочого 70 кінця і рівня відходження артерії, в басейні якої планується корекція кровотоку/.However, the known method does not take into account the ratio of the level of standing of the catheter / its inner working 70 end and the level of the departure of the artery, in the basin of which blood flow correction is planned/.
The useful model is based on the task of improving the effectiveness of treatment of occlusive diseases of the arteries of the lower extremities by placing the inner working end of the catheter at the level of the departure of the deep femoral artery.
The task is carried out by the "Method of treatment of arterial insufficiency of the lower extremities", which 12 involves catheterization of the femoral artery and intra-arterial reverse injection of infusate, in which, according to a useful model, the inner end of the catheter is placed at the level of the departure of the deep femoral artery.
The method is carried out as follows.
Before the intervention, the patient undergoes ultrasound examination of the femoral artery in the upper part of the thigh /at the place where the deep femoral artery departs/. At the same time, the place of departure of the deep femoral artery and the condition of the vessels are determined. Catheterization is carried out in an open or closed way. With the closed method, the patient is in a horizontal position on his back. The operative field is treated according to generally accepted rules. At an acute angle, the femoral artery is punctured and it is catheterized according to Seldinger. The inner end is placed at the level of the departure of the deep femoral artery / determined according to ultrasound data /. With the open method, control is carried out by palpation and visually. The artery is isolated in the area 8-12 cm below the inguinal ligament and punctured below the departure of the deep femoral artery. The catheter is advanced to a height equal to the distance between the puncture point and the level of departure of the deep femoral artery. 40-80 ml of infusate is injected into the artery at a rate of 2-4 ml/sec. Next, with a frequency of 2-4 times per minute, alternating administration of 10-20 ml of the same infusate at a rate of 2 ml/sec, followed by slow suction of an equal volume of blood and subsequent reverse rapid administration. At the same time, periodically /once every 1-2 minutes/ a new portion of the infusate is introduced. co

ЗPatient I. due to chronic ischemia of the lower extremity. the femoral artery was catheterized with the placement of the inner end of the catheter at the level of the departure of the deep femoral artery. Next, 3 mL of 0.2595 novocaine solution and 0.995 sodium chloride solution were injected into the artery at a rate of 2 ml/sec. Next, with a frequency of 3 times per two minutes, the introduction of 2 Oml of the same infusate at a speed of 2 ml/sec was alternated with the subsequent slow 3 o suction of an equal volume of blood and the subsequent reverse rapid introduction. At the same time, 1 time per minute - a new portion of the infusate was introduced. The total consumption of the infusate was 1b0ml. After 8 minutes, the patient noticed paresthesia, warming of the foot and lower leg. Moderate paresthesia and slight limitation of independent movements were observed. The patient was in this condition for 1.5-2 hours. I slept well at night. The feeling of warming the foot "lasted until the morning. WITH
Bench tests were conducted to justify the method. A section of the main artery with a lateral branch branching off from it was modeled on the test bench from PVC pipes. The diameter of the main line was
From" Bmm, branching - 1.3 mm, branching angle - 302. Water was passed through the stand with a flow rate in the side branching of 22 ml/min. A catheter was inserted into the trunk at different levels (in relation to the branch). Water was passed through it at different speeds. The obtained results are shown on the graph /Fig.1/. The graph shows -from 75 changes in the fluid flow rate in the branch depending on the location of the inner end of the catheter at a fluid injection rate of 2ml/sec. As can be seen from the graph, the maximum hydrodynamic effect was observed when (Ce) the catheter was standing at the level of the branch.
сWhen the liquid is introduced at a considerable distance from the branch due to the inertia of the flow, there will be a local expansion of the vessel, and then the outflow of the liquid to the branch. In the branching area, the stiffness of the vascular wall (Ce) is 250 greater than in other areas. Therefore, when additional liquid is introduced into the branching area, its flow into the corresponding branches occurs more intensively. The method is simple, safe, and minimally traumatic. There are no complications associated with a long stay of the catheter in the artery. Complications and side effects due to the effect of the drugs and the volume of the infusate, the puncture of the artery, were not noted. The method is economical. with
