The useful model refers to medicine, namely to anesthesiology. It can be used to block 2 chronic pain syndromes in cancer patients, mainly MI of the clinical group.
There is a known method of blocking chronic pain syndrome in an oncological patient, which involves catheterization of the epidural space at the level of the affected organ and exclusion of sympathetic, sensory innervation of the corresponding segments of the spinal cord by chemical denervation with injections of absolute ethyl alcohol |J.
Edward Morgan Jr., et al. Clinical Anesthesiology, St. Petersburg, 1998, p. 3881. 70 The disadvantage of this method is the insufficient level of anesthesia. In MI patients of the clinical group with widespread or multilocal pain, the treatment result, of course, does not live up to expectations.
The task of a useful model is to develop such a method of blocking chronic pain syndrome in an oncological patient, which would provide a higher pain-relieving effect by performing additional different-level catheterizations of the peridural space and chemical denervation of the corresponding segments of the spinal cord.
The task is solved by the fact that in the method of blocking the chronic pain syndrome in an oncological patient, which involves catheterization of the epidural space at the level of the affected organ and exclusion of sympathetic, sensory innervation of the corresponding segments of the spinal cord by chemical denervation with injections of 962 ethyl alcohol, according to a useful model, additional catheterization is performed epidural space and perform denervation at the levels of organs and areas affected by metastases, respectively.
Performing additional catheterizations of the epidural space with subsequent chemical denervation allows to obtain a higher analgesic effect due to the exclusion of sympathetic and sensory innervation of spinal cord segments at the levels of organs and areas affected by metastases, respectively.
The method is performed as follows. After treatment of the skin, catheterization of the 29th peridural space is performed at the level of the corresponding affected organ (T MI! - T IH - pancreas, liver, etc.). On the basis of ultrasound, CT, MRI data, as well as data from intraoperative revision of the chest and abdominal cavities, the localization, topography of distant metastases and segments receiving pain impulses are determined.
-Based on these data, puncture levels are determined and catheterization is performed with a second, and, if necessary, a third catheter of the epidural space. Catheters are passed cranial to a distance of 7-8 cm. The following drugs are used: lidocaine 295, ethyl alcohol 962. Lidocaine 5-7 ml is administered. This is necessary to determine the clear zones of the segmental block, to control the correctness of the catheter, and to prevent painful shock. It is considered that the spread of a neurolytic substance beyond the pathologically changed area can lead to irreversible consequences. After 30 min. evaluate the effectiveness of analgesia, if necessary, perform traction /Iche catheters for 3-4 cm and repeat the introduction of lidocaine. When the effectiveness of analgesia is confirmed After 30-60 minutes, ethyl alcohol 962 cooled to -10-159225 is injected in the amount of 4-bml, the injection rate is 0.Tml/min. -
-вDisappearance or significant reduction of pain is noted for 2-4 days. If necessary, repeat the introduction of alcohol 2-3 times, in the same doses. In the intervals between the introduction of alcohol, lidocaine is used. The transfer of incurable patients, accustomed to large doses of narcotic drugs, to analgesia by this method is carried out "20 gradually, in order to exclude the phenomena of abstinence. -in
