У комплексі передопераційної підготовки, інтраопераційної та післяопераційної терапії корвітин призначають за добу до операції, під час втручання, та в перші доби післяопераційного періоду. Останній застосовують внутрішньовенно, по 100 мл протягом 15-20 хвилин, двічі на добу, через 12 годин. Загальний курс лікування складає 4-5 днів. КорвітинФ розводять безпосередньо перед введенням Розчин готують в два етапи. На першому етапі - для первинного розведення препарату 0,9 956 розчин натрію хлориду в кількості 15 мл вводять за допомогою шприца у флакон з Корвітином?, флакон струшують до повного розчинення ліофілізованого порошку. Запропонованій спосіб ілюструється наступним клінічним прикладом. Хвора 1., 65 років, (медична карта Мо 3702) госпіталізована у хірургічне відділення Сімферопольської ЦКРЛ 15.05.2019 р. в плановому порядку з діагнозом: гігантська післяопераційна рецидивна вентральна грижа живота (МУ/4АНВ1 по ЗМУ В-сіаззіїісайноп, 1999), ІХС, кардіосклероз, гіпертонічна хвороба |! ост. ступінь 2, ризик 2, СН-Ї ст. Враховуючи високий ризик розвитку внутрішньочеревної гіпертензії під час та після запланованої герніопластики з приводу гігантської післяопераційної рецидивної вентральної грижі живота, за добу до операції (15.05.2019 р.) розпочато профілактику внутрішньочеревної гіпертензії за запропонованим способом. Після проведення передопераційної підготовки 16.05.2019 р. виконано оперативне бо втручання - ліквідацію післяопераційної вентральної рецидивної грижі живота з ретромускулярною алогерніопластикою, ліподермектомією, дренуванням надсіткового простору та післяопераційної рани. Інтраопераційно продовжено комплекс профілактичних лікувальних заходів за запропонованим способом. Тривалість операції - 1 година 50 хвилин. Ускладнень не було. В першу добу (17.05.20199 р.) післяопераційного періоду продовжено комплекс профілактичних заходів за запропонованим способом. Також у післяопераційному періоді отримувала знеболюючі, профілактичну антибактеріальну, кардіометаболічну та симптоматичну терапію. Післяопераційний період протікав гладко, без ускладнень. Внутрішньочеревний тиск протягом всього періоду спостереження не перевищував 10 мм рт. ст. Внутрішньошкірний косметичний шов видалено на 8 добу - рана загоюється первинним натягом, та 31.05.2019 р. хвора у задовільному стані виписана на амбулаторне лікування. After preoperative preparation, on May 16, 2019, surgical intervention was performed - elimination of postoperative ventral recurrent abdominal hernia with retromuscular allogeneoplasty, lipodermectomy, drainage of the supraretinal space and postoperative wound. Intraoperatively, the complex of preventive treatment measures was continued according to the proposed method. The duration of the operation is 1 hour 50 minutes. There were no complications. On the first day (May 17, 2019) of the postoperative period, a set of preventive measures was continued according to the proposed method. Also, in the postoperative period, she received painkillers, prophylactic antibacterial, cardiometabolic and symptomatic therapy. The postoperative period was smooth, without complications. Intra-abdominal pressure during the entire period of observation did not exceed 10 mm Hg. Art. The intradermal cosmetic suture was removed on the 8th day - the wound heals with primary tension, and on May 31, 2019, the patient was discharged for outpatient treatment in a satisfactory condition. She was examined three months after the operation, she has no complaints, and there are no signs of disease recurrence.
