Незважаючи на наявність широкого спектру препаратів на фармацевтичному ринку, терапія когнітивних розладів залишається досить складною проблемою. Одним з детально вивчених та відомих ноотропних препаратів є "Олатропіл" - комбінований препарат, який складається з пірацетаму та аміналону. Пірацетам володіє мемраностабілізуючою дією, антигіпоксичним, антиоксидантним, нейромедіаторним та вазотропним ефектом, залишається "еталоном" серед препаратів - ноотропів, але іноді виникають побічні дії у вигляді збудження, порушення сну, дратівливості. Тому, з метою нейтралізації даних ефектів, до складу олатропілу був введений аміналон. Аміналон -гамма-аміномасляна кислота бере участь в процесах центрального гальмування, благоприємно впливає на енергетику нейрону, мозковий кровообіг, когнітивні та неврологічні функції, має анксиолітичний та протисудомний ефект. Поєднання пірацетаму та аміналону дозволяє в 2 рази зменшити терапевтичні дози кожного з них, що в свою чергу призводить до зниження частоти та вираженості побічних реакцій, забезпеченню більш високої безпеки та ефективності (18, 19, 20, 211.Despite the availability of a wide range of drugs on the pharmaceutical market, the therapy of cognitive disorders remains a rather complex problem. One of the well-known and well-studied nootropic drugs is "Olatropil" - a combined drug consisting of piracetam and aminalon. Piracetam has a membrane-stabilizing effect, antihypoxic, antioxidant, neurotransmitter and vasotropic effect, remains a "standard" among nootropic drugs, but sometimes side effects occur in the form of excitement, sleep disturbance, irritability. Therefore, in order to neutralize these effects, aminalon was introduced into the composition of olatropil. Aminalon -gamma-aminobutyric acid participates in the processes of central inhibition, has a beneficial effect on neuron energy, cerebral blood circulation, cognitive and neurological functions, has an anxiolytic and anticonvulsant effect. The combination of piracetam and aminalon allows to reduce the therapeutic doses of each of them by 2 times, which in turn leads to a decrease in the frequency and severity of adverse reactions, ensuring higher safety and effectiveness (18, 19, 20, 211.

Скліфосовського було проведено комплексне неврологічне, нейропсихологічне дослідження та лікування 60 пацієнтів із розсіяним склерозом у стадії ремісії.Sklifosovsky, a complex neurological, neuropsychological study and treatment of 60 patients with multiple sclerosis in remission was carried out.

Основна група включала 30 пацієнтів, які в комплексній терапії отримували олатропіл по 1 капсулі три рази на день, контрольну групу склали 30 пацієнтів, які замість олатропілу приймали пірацетам по 400 мг З рази на день. Середній вік складав - 44,2:21,56 років. Було проведено комплексне обстеження пацієнтів, яке включало загальноклінічні методи, лабораторні методи дослідження (загальний аналіз крові, сечі, біохімічний аналіз крові), інструментальні методи (електрокардіографія, транскраніальна ультразвукова доплерографія).A comprehensive examination of patients was carried out, which included general clinical methods, laboratory research methods (general blood analysis, urine analysis, biochemical blood analysis), instrumental methods (electrocardiography, transcranial ultrasound dopplerography).

Оцінку неврологічного дефіциту проводили з використанням розширеної шкали інвалідизації - ЕО55 (Ехрапавєа Різарійу зіаше5 ЗКаїе) (221. Для об'єктивізації стану когнітивних функцій у всіх пацієнтів використовували шкалу ММЗЕ та Монреальську шкалу, яка оцінює різні когнітивні сфери: увагу, пам'ять, мову, зорові та конструктивні навички, абстрактне мислення, рахування та орієнтацію. Властивості уваги та динаміку працездатності визначали за допомогою таблиць Шульте (23). Оцінку якості життя - за російським варіантом опитувальника МОБ 5Е-36, який складається із восьми шкал з яких формують два параметри: психологічний і фізичний компоненти здоров'я. Для статистичної обробки отриманих даних використовували пакет прикладних програм на персональному комп'ютері. Вірогідність розбіжностей між порівнювальними групами оцінювалася за критеріями Стьюдента. Результати оцінки показників показали, що на фоні тривалої терапії в обох групах достовірно покращилися показники соціального і емоційного функціонування, а в основній групі відмічали достовірне покращення психологічного здоров'я і життєздатності пацієнтів (Табл.2).The results of the evaluation of indicators showed that against the background of long-term therapy in both groups, indicators of social and emotional functioning improved significantly, and in the main group, a significant improvement in the psychological health and vitality of patients was noted (Table 2).

Отримані дані аналізу шкали депресивності Бека свідчать про зниження кількості балів згідно шкали-опитувальника в усіх групах пацієнтів після проведеного лікування. Достовірне зниження депресії спостерігали в основній групі пацієнтів (Табл.4).The obtained data of the analysis of the Beck depression scale indicate a decrease in the number of points according to the scale-questionnaire in all groups of patients after the treatment. A significant decrease in depression was observed in the main group of patients (Table 4).

Реактивна тривожність практично не відрізнялась серед груп пацієнтів до початку лікування і знижувалась у основній групі після лікування. Подібна картина спостерігалась і для особистої тривожності в даній групі. Тенденція до підвищення показників в контрольній групі, можливо, пов'язана зі збуджуючою дією ноотропів. Достовірне зниження рівня реактивної та особистої тривожності після лікування в групі хворих, що приймала олатропіл свідчить про зменшення психоемоційної напруги, тривожно-депресивних, тривожно-фобічних і неврастенічних розладів (Табл.5).Reactive anxiety practically did not differ between groups of patients before the start of treatment and decreased in the main group after treatment. A similar picture was observed for personal anxiety in this group. The tendency to increase indicators in the control group may be related to the stimulating effect of nootropics. A significant decrease in the level of reactive and personal anxiety after treatment in the group of patients taking olatropil indicates a decrease in psycho-emotional tension, anxiety-depressive, anxiety-phobic and neurasthenic disorders (Table 5).

Таким чином, використання запропонованого способу дозволяє покращити когнітивні функцій у пацієнтів із рецидивно-ремітуючим РС, що дає можливість використовувати його у комплексній терапії для поліпшення нейропсихологічного статусу.Thus, the use of the proposed method allows improving cognitive functions in patients with relapsing-remitting MS, which makes it possible to use it in complex therapy to improve neuropsychological status.
