The useful model applies to medicine, and in particular to otorhinolaryngology and endocrine surgery, and can be used for early diagnosis of laryngeal paresis due to damage to the laryngeal nerves during thyroid surgery.
One of the complications of surgical intervention on the thyroid gland can be damage to the laryngeal nerves, which is accompanied by paresis of the larynx. Paresis is a functional impairment that is completely eliminated after some time under the influence of treatment. Untimely diagnosis of paresis with damage to the laryngeal nerves leads to 70 irreversible changes - paralysis of the larynx.
И.Б. 3761).Basically, this pathology is diagnosed with the help of mirror laryngoscopy |Methods! studies of the larynx. Handbook of otorhinolaryngology, under the editorship of I.B. Soldatova. -M., "Medicine".-1997.-P.3761).
However, this method is not precise enough and is often subjective.
19).As a prototype, the authors took a method of diagnosing paralysis of the larynx, the essence of which consists in ultrasound 12 scanning and determination of echo signals from the vocal cords. A disease is diagnosed by a change in the level of the location of the echo signal during phonation (AS Mo1734696 USSR, MPK 5 AbЕ188/00. Publ. 05.23.92, Byul.Mo19).
However, this method has disadvantages. He detects pathology in the late period - already in the presence of paralysis of the larynx. With the help of this method, it is not possible to detect paresis of the larynx - a violation of the symmetry and mobility of the vocal folds, the condition of other structures of the larynx, which is due to its anatomical features and the ace for which the ultrasound signal forms an image suitable for analysis.
This useful model is based on the task of improving the process of early diagnosis of laryngeal paresis due to damage to the laryngeal nerves through the additional use of laryngovideostroboscopic examination, which will increase the accuracy and informativeness of diagnosis in the postoperative period.
The set task is achieved by the fact that in the process of early diagnosis of paresis of the larynx due to damage to the laryngeal nerves during surgery on the thyroid gland, which includes the use of ultrasound-PTV scanning of the larynx, according to this useful model, a computer laryngovideostroboscopic examination of the larynx is additionally carried out, the level of the position is determined , symmetry and amplitude of mobility of the vocal folds, completeness of closure and symmetry of the glottis, and when these structures change relative to the norm, paresis of the larynx is diagnosed. high school
Such an additional computerized laryngoscope videostroboscopic study allows you to get timely and accurate information about the state of the structures of the larynx and, if there are changes relative to the norm, to determine the pathology.
--The process is carried out as follows. --
After a detailed examination by a doctor, a patient with complaints of dysphonia, shortness of breath, and coughing is prescribed an ultrasound examination to rule out or confirm the diagnosis of laryngeal paralysis. At
In the absence of signs of paralysis of the larynx, to clarify the diagnosis, a computer - laryngovideostroboscopic study is additionally prescribed.
For this, the patient is subjected to applied anesthesia of the soft palate, the back wall of the oropharynx, and the root of the tongue. In a sitting position, in a state of calm breathing, the endoscope is inserted into the oral cavity with the light guide "down" and carried parallel to the bottom to the back wall of the pharynx. The appearance of a clear laryngoscopic image indicates the correctness of the installation of the endoscope C. After a clear visualization of the middle part of the larynx, the video recording begins. Research is carried out during free breathing and during phonation in the following sequence:
From" - Laryngoscopy with free deep breathing; - Laryngoscopy during phonation; - Laryngostroboscopy (phonation under the control of stroboscopic lighting).
To study the vocal function of the larynx, the patient pronounces vowel sounds ("a", "i"). These sounds are perceived - at the level of the thyroid cartilage by the laryngophone, which fixes the main formant of the speech spectrum and adjusts the frequency of light flashes of the light source during stroboscopic examination.
An analysis and evaluation of the structures of the larynx is carried out, comparing it with the norm. They determine the level of position, - symmetry and amplitude of mobility of the vocal folds, completeness of closure and symmetry of the glottis, and at 20 when these structures change relative to the norm, paresis of the larynx is diagnosed.
Example 1. from Patient S., 55 years old. On the second day after the subtotal resection of the thyroid gland, she turned to the doctor with complaints of hoarseness of voice and coughing. On examination, the mucous membrane of the larynx is unchanged. Ultrasound examination did not reveal paralysis of the larynx and causes of decreased voice function. In addition, 25 computer laryngovideostroboscopic studies were prescribed. The level of the position of the vocal folds - the right is shortened and located lower compared to the left, the oscillation is asynchronous, with a reduced amplitude, the tone is reduced.
Rotation of the glottis to the right is noted. There is damage to the external branch of the upper laryngeal nerve.
The patient was diagnosed with neuropathic right-sided paresis of the larynx.
Example 2. 60 Patient K., 24 years old. On the first day after resection of the thyroid gland, he notes difficulty breathing, hoarseness of voice, weakness. During ultrasound examination of the larynx, the mobility of the vocal folds during breathing and phonation is not determined. To clarify the diagnosis, a computer laryngovideostroboscopic study was performed. An unchanged mucous membrane of the larynx was found. The position of the vocal folds is not symmetrical. They are fixed in the paramedian position, there is no oscillation amplitude. The vocal fold is 2 mm - that is, it is significantly reduced. Movement in the cricoid joints is not disturbed. An injury to the larynx associated with tracheal intubation was excluded. Based on the analysis of these data, a conclusion was made - damage to the right and left inferior laryngeal nerves and as a result - acute stenosis of the larynx, postoperative bilateral neuropathic paresis of the larynx.
Thus, this process allows to increase the effectiveness of diagnostics and is recommended for use in medical practice.
