Abstract
A method of treating mental disorders by prescribing hyperthermia. Inhalation of inert xenon gas with subsequent moderate cerebral hypo-, hyperthermia is performed.
Description
The useful model belongs to medicine, namely to medical rehabilitation and psychiatry, and can be applied to the treatment and rehabilitation of patients with psychosomatic, neurotic and organic mental disorders.
The problem of mental disorders is extremely urgent. More than 956 inhabitants of the planet suffer from mental disorders. There is no doubt that mental disorders significantly worsen the quality of life of patients. Depending on the severity and severity of symptoms of a mental disorder, patients experience significant psychological discomfort, and sometimes even physical discomfort. They have significant difficulties in social and professional adaptation and often require disability. Constant worry about one's health or fear of leaving the house, or significant memory impairments are significant obstacles in performing 10 basic life functions, such as self-care, walking, finding a life partner. Social phobias can prevent the patient from working in certain jobs, studying in public educational institutions, playing some sports, taking care of family members, etc. A significant decrease in the cognitive level in dementia often represents a special psychological problem for the patient's family members due to constant nervousness about the patient's compromising behavior, the inability to leave him unattended, and often pronounced aggression directed at caregivers. An even bigger problem is caused by the presence of hallucinatory and delusional experiences in patients with dementia. In addition, the treatment and rehabilitation of mental disorders is quite expensive, it interferes with the patient's socialization and arrangement of personal life.
23.02.1982).There are many ways to treat mental disorders. The closest to the declared useful model is the method of treating psychoses by using hypothermia (AS USSR, Mo 906577, IPC: Ab61B 18/02, AbIM 21/00, Method of treating psychoses, publ. 23.02.1982).
The disadvantage of the known method is its low therapeutic efficiency and a number of complications from the use for the purpose of premedication of such drugs that carry out neurovegetative blockade, such as: sodium oxybutyrate 20 95 to 50.0 v/v in combination with 1 95 sodium thiopental - 1.0 v/v in, suprastin 3.0 in/in, seduxen 2.0 in/in.
The basis of a useful model is the task of improving the method of treatment of mental disorders, using the effect of preconditioning: cerebral hypo- and hyperthermia and xenon inhalation, which will allow to achieve stabilization of the patient's condition and the transition to long-term stable remission.
The task is solved by the fact that in the method of treating mental disorders by using hypothermia, according to a useful model, inhalation of inert xenon gas is carried out followed by moderate cerebral hypo- and hyperthermia.
The essence of the method is that cerebral hypo- and hyperthermia, in addition to the main mild stress effect, which is a preconditioning factor, has a special spectrum of psychotropic activity, namely in the form of changes in the functional state of the mediator systems of the body by stimulating and inhibiting synaptic processes, increases the permeability to pharmacological preparations of the histohematological barriers of the brain, normalizes the tone of brain vessels and restores cerebral hemodynamics, affects neuronal plasticity, reduces intracranial pressure; in turn, xenon as an antagonist of MMOA receptors has analgesic, neuroprotective, antidepressant, anxiolytic effects, and increases the adaptive capabilities of the body due to the creation of the so-called "heterostasis" (H. Selye, 1950; L.IM. Harkavy et al., 1979). A course of cerebral hypo- and hyperthermia is prescribed with a frequency of 5 sessions on Sunday, a course of 10-15 sessions with a decrease in the temperature in the ear canal to 34-36 "C and after 5 minutes an increase in temperature to 37-38 "C. Change of hypothermic and hyperthermic states - 10-15 times per procedure. Exposure of the entire procedure up to 1.5-2 hours, depending on the pathology. Each procedure is carried out after oxygen-xenon inhalation (70/30 95, respectively) with repetition of the course in the spring-autumn period and in the period of psycho-emotional stress.
The method is performed as follows.
Before the treatment, permission was obtained for the patient's consent regarding the proposed therapy. Treatment was carried out on an outpatient basis. Patients with psychosomatic, neurotic, organic mental disorders were prescribed a course of cerebral hypothermia according to the standard method ("Application of craniocerebral hypothermia in the treatment of depressive disorders in patients with suicidal behavior". Maruta N.A. et al., Kharkiv, 2007), but not with using a hypothermic helmet with a freon cooler, and electric, semiconductor plates made on the Peltier effect, with a frequency of 5 sessions per week, a course of 10-15 sessions depending on the pathology, the procedure was carried out after oxygen-xenon inhalation (at a concentration of 70 95 by 30 95 respectively), within three minutes. Patients received standard pharmacological therapy, depending on clinical symptoms.
32 patients (the main group) were treated with the proposed method using cerebral hypo- and hyperthermia against the background of inhalation of a xenon-oxygen mixture along with standard pharmacological treatment, with a stable remission within 12 months. During this period, the course was repeated in the spring and autumn periods. The control group consisted of 15 patients with analytical clinical symptoms (neurotic anxiety-depressive disorders, E. 40) and therapy, which includes only traditionally used pharmacological drugs, without xenon-oxygen mixture and hypo- and hyperthermia.
Each of the groups of patients (control and main) underwent a thorough clinical psychopathological and experimental psychological examination before and after the treatment. The obtained results indicate a statistically significant (Р«0.001) greater effectiveness of therapy with the use of preconditioning agents in the form of oxygen-xenon inhalations and sessions of cerebral hypo- and hyperthermia.
The method is explained by the following example:
Example. Patient A., 23 years old, turned to the clinic for pronounced attacks of anxiety and panic, which periodically recede, he notes that they are not limited to any special situation or complex of circumstances, therefore they are unpredictable. At this time, he feels a strong heartbeat, pains behind the sternum, a feeling of suffocation, nausea, and a feeling of unreality. During this period, the fear of death appears. Standard drug therapy was prescribed along with craniocerebral hypo- and hyperthermia. 10-15 minutes before the hypo- and hyperthermia session. inhalation was carried out with an oxygen-xenon mixture (70/30 95, respectively) for 3 minutes.
Anxiolytic and plant-stabilizing effects appeared already on the 5th day. On the 3rd day, the patient's irritability disappeared, and on the 5th, the level of anxiety decreased significantly, the desire for an active life appeared, and sleep improved. At the end of the week of treatment, panic attacks disappeared, somatic sensations were not noted, but the "premonition" of a possible panic attack was disturbing, the patient could not fall asleep for a long time, as "...anxious thoughts do not leave...", but did not wake up in the middle of the night as before, and slept until morning. At the end of the second week, the disturbing "premonition" no longer bothered me, sleep improved.
In comparison with the known methods of treatment, the proposed useful model will allow to achieve stabilization of the patient's condition and the transition to long-term stable remission.
