Abstract
Method for facsiotomy of the anterior tibia case in compartment syndrome in which a longitudinal incision of the skin and subordinate tissues is performed at the site of the affected area. The incision is made up to 2 cm in length. Under the front fascia, the fasciitis is inserted and multiple incisions are made in an amount directly proportional to the volume of muscle tissue. The incisions diverge upwards.
Description
The useful model belongs to the field of medicine, in particular to surgery and traumatology, and can be used in the treatment of patients with lower limb injuries complicated by compartment syndrome.
(11.A well-known analogue is the method of fasciotomy of the anterior sheath of the lower leg in case of compartment syndrome, which includes an incision of the skin and underlying tissues of the damaged area, a linear incision of the anterior fascia of the affected compartment, which is performed subcutaneously with scissors, thus reducing the intramuscular pressure in the injured limb (Fig. 11.
The drawback of the analogue is insufficient clinical effectiveness, which consists in the fact that a single linear incision of the wall of the front sheath of the lower leg of the affected limb cannot provide adequate decompression of soft tissues, and the wide opening of tissues during the above-mentioned manipulation can be the cause of infection of damaged soft tissues and additional traumatization.
The useful model is based on the task of improving the well-known method in which, by changing the fasciotomy technology aimed at reducing the length of the incision of the skin and underlying tissues and performing multiple incisions of the affected compartment with the help of a fasciotome, optimal pressure reduction in the injured tissues surrounded by the anterior fascia is ensured. The number of linear incisions of the fascia is directly proportional to the volume of tissue in the fascial bag, the incisions diverge in a fan-like manner upwards.
The task is solved by the fact that, under conductor anesthesia, a longitudinal incision of the skin and underlying tissues in the place of the affected area, up to 2 cm in length, is performed, a fasciotomy is inserted under the front fascia and multiple incisions are made in a quantity directly proportional to the volume of muscle tissue, the incisions diverge in a fan-like fashion up. After the fasciotomy, an aseptic bandage is applied to the entrance wound.
A useful model is explained with drawings.
6.In Fig. 1: a cross-sectional view of the right lower leg: tibia 1, fibula 2, anterior fascial sheath 3, lateral fascial sheath 4, deep posterior fascial sheath 5, superficial posterior fascial sheath 6.
In Fig. 2: view of the direction of the fascia incisions made with a fasciotome inserted through the incision of the skin and underlying tissues.
From Fig. 3: general view of the lower leg with the insertion site of the fasciotome for dissection of the fascia.
So, a useful model allows you to optimally reduce the pressure in the tissues of the fascial case due to multiple incisions, the number of which is directly proportional to the volume of tissues in the case, the incisions diverge in a fan-like manner upwards, and reducing the length of the tissue incision for the introduction of a fasciotome reduces traumatization and the likelihood of soft tissue infection , which shortens the period of inpatient treatment of trauma patients.
