Ю.М. И.The closest in technical essence to the declared useful model is a needle | see book Yu.M. Pantsyrev, Yu.I.
Gallinger, "Operative Zndoscopy of the Gastrointestinal Tract" - M.: "Medicine, 1984, p. 145-146)|. A needle tip is mounted on the 19th distal end of the hollow flexible rod, which protrudes from the metal spiral probe on the Zmm, which prevents penetration through the entire thickness of the stomach wall. The fluoroplastic catheter hermetically connects the needle to the cannula, which is located at the proximal end of the instrument. The metal spiral probe surrounds the fluoroplastic catheter and provides the necessary rigidity to the needle, which prevents the spontaneous deviation of the needle tip to the sides and its exit from the field of view after passing the endoscope 20 into the stomach through the biopsy channel.
The shortcomings of the previously proposed technical solutions led to the fact that, despite the urgent need, their effective use is not ensured in practice during endoscopic injection therapy and coagulation.
The task of a useful model is the constructive development of an endoscopic coagulating injector, which would allow to increase the efficiency of endoscopic injection therapy and electrocoagulation in non-varicose sources of bleeding, and would also allow to reduce the time of endoscopic manipulations.
оThe task is solved by the fact that in the proposed device, which includes a catheter, at the distal end of which there is a metal head through which passes a needle that is attached to an electrosurgical unit, at the proximal end of the needle in the side wall there is a hole for the passage of the solution, and at 30 at the proximal end of the catheter - a connector for a syringe, according to a useful model, which ensures an increase in the efficiency of the device and a reduction in the time of endoscopic manipulations on non-varicose bleeding sources. at
«Due to the fact that in the presented useful model, the endoscopic coagulating injector consists directly from a catheter, on the distal end of which a metal head is placed, and in the middle of the catheter, a metal conductor is placed, connected to the electrosurgical unit from the proximal end, and on the -- a needle is placed at the distal end, the injection of the solution takes place thanks to the hole at the proximal end of the needle and the connector for the syringe at the proximal end of the injector, which ensures effective endoscopic injection therapy and electrocoagulation on non-varicose bleeding sources. The syringe connector 6 is located at the proximal end of the device. The coagulating endoscopic injector proposed by the authors works as follows (Fig.). After anesthesia of the pharynx and pharynx with a lidocaine solution, an endoscopic examination is performed. When non-varicose sources of bleeding are detected, a coagulating injector is inserted into the biopsy channel of the endoscope, which consists of a needle 1 at the distal end 29, which is connected through a metal conductor 5 to an electrosurgical unit 7. In addition, a metal head 2 is located at the distal end of the coagulating injector, which is rigidly attached to the catheter 4, through which passes the needle 1, which moves with the help of a metal conductor 5. At the proximal end of the needle 1 there is a hole C through which the solution enters. The metal conductor 5 is in the catheter 4, on the distal end of which there is a needle 1 and a metal head 2, and on the proximal end there is a connector for 60 syringe 6 and a metal conductor 5, which comes out of the catheter 4 and is connected to the electrosurgical unit 7.
5.When lancing a non-varicose source of bleeding with a coagulating injector, needle 1, with the help of a metal conductor 5, is brought out beyond the metal head 2 by 2/3 of the length of the needle 1 and needle 1 is inserted into the mucous membrane near the non-varicose source of bleeding. After that, connect the syringe with the solution to the syringe connector b and inject the solution into the mucous membrane. This manipulation is repeated several times as close as possible to the source of bleeding. After circumscribing the non-varicose source of bleeding, without removing the endoscopic coagulation injector from the biopsy channel of the endoscope, the needle 1 is inserted through the metal head 2 into the catheter 4 with the help of the metal conductor 5, but so that the needle 1 is in contact with the metal head 2. Then, touching the metal head 2 to the non-varicose source of bleeding, perform electrocoagulation thanks to the electrosurgical unit 7, which is connected to the metal conductor 5.
With the help of an endoscopic coagulating injector, effective endoscopic injection therapy and electrocoagulation on non-varicose sources of bleeding are ensured. At the same time, one device is not replaced by another, which significantly reduces the time of endoscopic manipulations.
The following examples can be used as proof of the effectiveness of this device, which was tested in the Kyiv City Clinical Hospital of Emergency 70 Medical Care (KMKLSHMADI).
Patient V., 48 years old, medical history Mo5730, was admitted to KMKLSHMD on February 20, 2005. with suspicion of gastrointestinal bleeding. During an emergency endoscopy, a bleeding ulcer of the bulb of the duodenum (DPK) was revealed (according to Roggev 188). An endoscopic coagulating injector was inserted into the biopsy channel. In connection with the massiveness of the bleeding, it was decided to circumcise the bleeding ulcer with a physiological solution, as a result of which the intensity of the bleeding decreased significantly. Without removing the coagulating injector from the biopsy channel of the endoscope, and pulling the needle into the catheter with the help of a metal conductor, electrocoagulation of the bleeding ulcer was performed.
During further endoscopic monitoring, complications and recurrence of bleeding were not detected. The patient was prescribed anti-ulcer treatment according to the standard scheme.
