多發性硬化症(MS)是一種大腦和脊髓的慢性神經退化性疾病,涉及環繞和保護神經的髓鞘(myelin sheath)的損失。症狀是非特異性的,但通常包含疲勞和視覺、活動性、感覺和平衡問題。在美國,超過300,000人被診斷出患有MSA。該疾病在女性中比男性更常見,且通常在20-30歲被診斷出來,儘管老年人也可能被診斷出來。

在一實施例中,核小體的表觀遺傳特徵包含一或多種蛋白質-核小體加合物或複合物。另一類的循環核小體子集是核小體蛋白質加合物。多年來已知的是,染色質包含大量的與其組成DNA及/或組織蛋白結合的非組織蛋白蛋白質。這些染色質相關蛋白種類繁多,且具有多種功能,包括轉錄因子、轉錄增強因子、轉錄抑制因子、組織蛋白修飾酵素、DNA損傷修復蛋白以及其他。這些染色質片段包含核小體和其他非組織蛋白染色質蛋白或DNA,並且在本領域中對於其他非組織蛋白染色質蛋白已有所描述。

診斷和監測方法
根據另一態樣,提供一種監測一主體中多發性硬化症的方法,包含:

(ii) 在一或多個時點重複檢測或測量從該主體中獲得的一體液中的無細胞核小體或其組分的含量;

這種方法可以用於監測主體中疾病的進展或發展,以確定是否需要醫療介入。例如,如果確定主體具有輕度階段的疾病或處於緩解期(remission),則本發明可以用於監測疾病進展,以用於未來復發的發展。例如,如果該方法包含來自確定患有輕度階段疾病的主體中的樣本,則可以在另一時間點重複生物標記物含量的測量,以確定生物標記物含量是否改變。
檢測和/或定量可以直接在體液核小體樣本上進行,或在純化的或濃縮的(enriched)核小體樣本上進行,或間接在其萃取物上進行,或在其稀釋液上進行。定量樣本中存在的生物標記物的含量可包含檢測樣本中存在的生物標記物的濃度。根據本文所述之本發明的檢測、監測和診斷的用途以及方法可用於確認疾病的存在、透過評估發病和進展來監測疾病的發展、或評估疾病的改善或消退。檢測、監測和診斷的用途以及方法也可以用於評估臨床篩檢、預後、治療選擇、治療益處評估的方法,即用於藥物篩選和藥物開發。
檢測或測量可以包含免疫測定、免疫化學、質譜、色譜、染色質免疫沉澱或生物感測器方法。特別是,檢測和/或測量可包含用於核小體部分的2位點免疫測定法。這種方法較佳用於核小體或含有表觀遺傳特徵的核小體的原位測量,其是使用兩種抗-核小體結合劑或一種抗-核小體結合劑和抗-組織蛋白修飾或抗-組織蛋白變體或抗-DNA修飾或抗-加合蛋白檢測結合劑。或者,免疫測定法可採用兩種抗-核小體結合劑他們皆直接與核小體的表觀遺傳特徵結合。此外,檢測和/或測量可包含2位點免疫測定,例如採用標記的或固定的組合:抗-核小體、抗-組織蛋白修飾、抗-組織蛋白變體/同功異型體、抗-DNA修飾或抗-加成蛋白結合劑。
在一實施例中,檢測或測量方法包含將體液樣本與一固相接觸,該固相包含用於檢測無細胞核小體或其組分的一結合劑,以及檢測所述結合劑的結合。
在一實施例中,檢測或測量方法包含:(i) 將樣本與結合至一無細胞核小體的一表觀遺傳特徵的一第一結合劑接觸;(ii) 將步驟(i)中由第一結合劑結合的樣本與結合至無細胞核小體的一第二結合劑接觸;以及(iii) 檢測或定量在樣本中第二結合劑的結合。

可以使用一或多種試劑(例如合適的結合劑)來檢測或測量生物標記物的含量。例如,一或多種結合劑可包含對目標生物標記物具有特異性的配體或結合劑,該目標生物標記物諸如核小體或其組成部分、核小體的表觀遺傳特徵、核小體的結構/形狀模擬物或其組成部分,可選地與一或多種介白素組合。

本發明的方法可涉及標誌物含量的標準化。例如可以將含有特定表觀遺傳特徵的無細胞核小體的含量相對於核小體本身(或一些其他類型的核小體或參數)的含量標準化,以將含量表達為包含該特徵的核小體的比例。例如,將瓜胺酸化核小體的含量表示為瓜胺酸化的核小體的比例。
在一實施例中,本文所述的方法在多個時點(occasion)被重複。此實施例提供允許在一段時間監測檢測結果的優點。這樣的安排將提供監測或評估疾病狀態的治療效果的益處。本發明的這些監測方法可用於監測發病、進展、穩定性、改善、復發和/或緩解。因此,在一實施例中,在一或多個時點重複該方法,且使用無細胞核小體或其組分的含量的任何變化來監測主體中多發性硬化症的進展。

測試樣本中的生物標記物的含量相對於從相同測試主體中較早取出的先前測試樣本中的含量的變化可以指示有益效果。例如,所述治療對病症或疑似病症的穩定或改善。此外,一旦完成治療,可以定期的重複本發明的方法以監測疾病的重現或復發。
監測治療功效的方法可用於監測在人類主體和非人類動物(例如在動物模型)中現有療法和新療法的治療效果。這些監測方法可以合併至新原料藥和物質組合的篩選中。
在另一實施例中,可以更短的數小時或數天的間隔進行由於快速作用療法而產生的更快速變化的監測。
提供了用於執行本發明方法的診斷或監測試劑套組(kit)(或組(panel))。這類套組合適地包含一或多種用於檢測和/或定量根據本發明的生物標記物的配體,和/或生物感測器、和/或本文所述的陣列,可選地連同試劑套組的使用說明。

用於檢測疾病存在的生物標記物是發現新標靶和藥物分子的重要標靶,其可減緩或停止疾病的進展。由於生物標記物的含量是表示疾病和藥物反應的,因此生物標記物可用於在體外和/或在體內測定中鑑別新的治療化合物。本文所述的生物標記物可用於篩選調節生物標記物活性的化合物的方法中。

可以將檢測和/或定量與臨界量(cut-off level)進行比較。可透過分析來自多個患者和對照的結果並決定用於將主體分為患有或不患有該疾病的合適數值來預先決定臨界值(cut-off value)。例如,對於患有該疾病的患者中生物標記物含量為較高的疾病,如果檢測到的含量高於臨界值,則表示該患者患有該疾病。或者,對於患有該疾病的患者中生物標記物含量為較低的疾病,如果檢測到的含量低於臨界值,則表示該患者患有該疾病。使用簡單的臨界值的優點包括臨床醫生能夠輕鬆理解測試內容,並且不需任何軟體或其他輔助手段即可解釋測試結果。可使用本領域的方法確定臨界量。
也可以將檢測和/或定量與對照進行比較。本領域技術人員將清楚的是,可以在多種基礎上選擇對照主體,例如其可以包含已知為沒有疾病的主體或可以是患有不同疾病的主體(例如,用於鑑別診斷調查)。「對照(control)」可包括健康主體、未患病主體和/或處於緩解期的主體。對照也可以是沒有症狀或症狀輕度的多發性硬化症的主體。輕度症狀可包含不需要醫院干預和/或侵入性醫療的可控制的症狀。
與對照的比較在診斷領域是已知的。在對照組中發現的值的範圍可以用作正常或健康或參考範圍,可將測試主體發現的值與其進行比較。例如,如果參考範圍小於10(<10)單位,則5單位的測試值將被視為正常或不需要治療,但11單位的值將被視為異常且指示為需要治療。
因此,在一實施例中,該方法另外包含將主體的體液樣本中的無細胞核小體或其組分的含量與一或多個對照進行比較。例如該方法可包含將從主體獲得的樣本中的無細胞核小體存在的含量與從正常主體獲得的樣本中的無細胞核小體存在的含量進行比較。對照可以是健康主體。
在一實施例中,與對照相比,無細胞核小體或其組分的含量為升高。
可以理解的是,沒有必要在每次都因為要比較所以測量對照組含量。例如,針對健康/非疾病對照,一旦確定了「正常範圍」,就可以將其用作所有後續測試的基準。可以透過從多個沒有感染的對照主體身上獲取樣品並測試生物標記物含量來確定正常範圍。然後可以檢查懷疑受感染的主體的結果(即生物標記物含量),看他們是否在各自的正常範圍內或之外。使用「正常範圍」是檢測疾病的標準做法。
在一實施例中,該方法還包含確定該患者之至少一臨床參數。此參數可用於結果的解讀。臨床參數可以包含任何相關的臨床訊息,例如但不限於體溫、性別、體重、身體質量指數(BMI)、吸煙狀況和飲食習慣。因此,於一實施例中,臨床參數係選自由:體溫、年齡、性別以及身體質量指數(BMI)所組成的群組。
根據本發明的另一態樣,提供了結合劑在製造用於診斷或監測多發性硬化症的方法中的套組的用途,包含:
(i) 將從主體中獲得的一體液樣本與結合劑接觸,以檢測或測量無細胞核小體或其組分的含量;以及
(ii) 使用檢測到的無細胞核小體的含量來診斷或監測主體。
根據本發明的另一態樣,提供了一種檢測需要對多發性硬化症進行醫療的主體的方法,包含:
(i) 將從主體中獲得的一體液樣本與結合劑接觸,以檢測或測量無細胞核小體或其組分的含量;以及
(ii) 使用該無細胞核小體的含量作為主體需要對多發性硬化症進行醫療的指示。
其他生物標記物
可以檢測或測量無細胞核小體的含量作為一組測量之一。該組可包含如上文所述的核小體的不同表觀遺傳特徵(例如組織蛋白同功異型體和PTM)。用於檢測多發性硬化症的組合測試中的生物標記物包含但不限於寡株免疫球蛋白G (IgG)或免疫球蛋白M (IgM)蛋白、細胞因子部分(特別是介白素)、C-反應蛋白(C-reactive protein)、D-二聚體(D-Dimer)、因子VII活化蛋白酶(FSAP)、纖維蛋白原和纖維蛋白/纖維蛋白原分解產物。在一實施例中,該組包含一或多種細胞因子,例如一或多種介白素。在較佳實施例中,組測量是在CSF、血液、血漿或血清樣本中進行。
介白素(interleukin,IL)是一組通常由白血球分泌的細胞因子,其是作為訊號分子。它們在刺激免疫反應和發炎中扮演重要角色。它們在1970年代首次被發現,並由於發現了更多類型的介白素而被編號。介白素的例子包含但不限於:IL-1、IL-2、IL-3、IL-4、IL-5、IL-6、IL-7、IL-8、IL-9、IL-10、IL-11、IL-12、IL-13、IL-14和IL-15。
在一實施例中,一或多種介白素選自由:介白素-6(IL-6)和介白素-12(IL-12)所組成的群組。
介白素可為IL-6。介白素-6(IL-6)是一種具有多種生物學功能的細胞因子。它是發燒和急性期反應的有效誘導物。人類IL-6的序列是本領域已知的,並且在UniProt登錄號P05231中記載。在一特定實施例中,介白素可為IL-6。
或者,或另外,介白素可以是IL-12。介白素-12(IL-12)是一種T細胞刺激因子,因為其可以刺激T細胞的生長和功能。其是由IL-12A和IL-12B組成的異二聚體細胞因子。人類IL-12A的序列是本領域已知的並在UniProt登錄號P29459中記載,且人類IL-12B的序列也是已知的並在UniProt登錄號P29460中記載。在一特定實施例中,介白素可為IL-12。
在一實施例中,該套組(panel)包含無細胞核小體或其表觀遺傳特徵和介白素。在另一實施例中,該套組包含無細胞核小體的表觀遺傳特徵和兩種介白素。例如,無細胞核小體測量可以與一種以上的介白素測量相結合,例如IL-6和IL-12。在另一實施例中,無細胞核小體的表觀遺傳特徵選自組織蛋白同功異型體(例如H3.1)、和轉譯後修飾的組織蛋白(例如瓜胺酸化H3和/或甲基化H3)。在又一實施例中,測量套組為H3.1、H3R2,R8,R17Cit和H3K27Me3。
可以使用本發明的生物標記物得到模型。用於得到模型或演算法的方法是本領域已知的且合適的套裝軟體是可買到的。用於此目的的典型軟體工具包含SPSS(社會科學統計軟體)和”R”。這些套裝軟體提供臨床數據的線性和非線性數據建模。
本領域技術人員將清楚的是,本文揭露的生物標記物的任何組合可用於檢測多發性硬化症的套組和演算法中,且可將另外的標記物添加到包含這些標記物的套組中。
根據本發明的一態樣,提供套組測試的用途,以檢測患有多發性硬化症患者,其中套組測試包含用於檢測從患者獲得的樣本中的核小體或其組分和一或多種介白素的測量的試劑。
治療方法
根據另一態樣,提供一種治療患有多發性硬化症的一主體的方法,包含:
(i) 將從該主體中獲得的一體液樣本與一結合劑接觸,以測量無細胞核小體或其組分的含量;
(ii) 使用在步驟(i)中測量的該無細胞核小體或其組分的含量作為該主體中多發性硬化症存在和/或嚴重性的指標;以及
(iii) 對步驟(ii)中確定需要治療的該主體進行治療。
根據另一態樣,提供一種在需要的一主體中治療多發性硬化症的方法,該方法包含將治療劑施用於當與從對照主體獲得的樣本中的無細胞核小體的含量相比,從所述主體獲得的樣本中被識別為 具有不同含量的無細胞核小體的主體的步驟。
步驟(iii)可包含施用治療劑。治療可集中於治療MS症狀的復發(例如使用類固醇藥物)、治療特定的MS症狀和/或減少復發次數的治療(即疾病改善療法(disease-modifying therapy)。在一實施例中,該治療選自一或多種:類固醇(例如皮質類固醇,例如普賴鬆(prednisone)和甲基普立朗(methylprednisolone)、干擾素β藥物(例如Avonex、Betaferon、Extavia、Plegridy和Rebif)、乙酸格拉替雷(glatiramer acetate)、芬戈莫德(fingolimod)、反丁烯二酸二甲酯(dimethyl fumarate)、反丁烯二酸地羅西美(diroximel fumarate)、特立氟胺(teriflunomide)、西普尼莫德(siponimod)、克拉屈濱(cladribine)、單株抗體治療(例如奧瑞珠單抗(Ocrelizumab)、那他珠單抗(Natalizumab)和阿侖單抗(Alemtuzumab))或造血幹細胞移植(haematopoietic stem cell transplantation, HSCT)。
根據本發明的另一態樣,提供一種治療多發性硬化症的方法,包含使用套組測試識別需要治療多發性硬化症的患者並提供所述治療,其中套組測試包含檢測核小體或其組分的測量的試劑。與對照相比,預期患有多發性硬化症的患者具有更高含量的無細胞核小體。
應當理解的是,本文所述的實施例可以應用於本發明的所有態樣,即針對用途描述的實施例可以同樣地應用於請求保護的方法等。
現在將參考以下非限制性實施例說明本發明。
實施例
1
從24名被診斷患有MS的患者和19名健康主體中收集血漿樣本進行核小體測定。將全血樣本收集到血漿採血管中,並在靜脈穿刺後2小時內離心。將血漿轉移到冷凍管中並冷凍保存直到進行分析。在測定前,將保存的血漿樣本解凍並以14000 g離心2分鐘。
對樣本進行3種不同核小體類型的測量。這些是含有組織蛋白變體H3.1的核小體(H3.1-核小體)、含有組織蛋白PTM H3K27Me3的核小體(組織蛋白H3在離胺酸殘基27處甲基化)和含有組織蛋白PTM H3R2,R8,R17Cit的核小體(組織蛋白H3在精胺酸殘基2、8和17處瓜胺酸化)。使用自動免疫分析儀透過免疫測定進行核小體的測量。簡單來說,將校準物或樣本(50µl)與吖啶酯(acridinium ester)標記的抗-核小體抗體(50µl)和測定緩衝液(100µl)在37℃下培養1800秒。加入塗覆有抗-H3.1、抗-H3K27Me3或抗-H3R2,R8,R17Cit抗體(20µl)的磁珠,並將混合物再培養900秒。接著分離磁珠,洗滌3次,並通過超過7000毫秒的發光輸出來測定磁性結合的吖啶酯。從標準曲線內插獲得結果。
結果顯示,儘管具有不同的模型,MS患者的3種核小體類型的血漿含量升高。在MS中H3.1-核小體含量升高,結果產生的ROC曲線具有曲線下面積(AUC)為87%以及在100%特異性下的靈敏度為54%(圖1)。在MS中H3K27Me3-核小體含量升高,結果產生的ROC曲線具有曲線下面積(AUC)為89%以及在100%特異性下的靈敏度為58%(圖2)。在MS中H3R2,R8,R17Cit-核小體含量升高,結果產生的ROC曲線具有曲線下面積(AUC)為78%以及在100%特異性下的靈敏度為33%(圖3)。
我們推斷,在MS患者體液中的核小體含量升高,且核小體測量值是疾病的存在和嚴重性的有效指標,且可提供復發或發作的可能性和/或可能嚴重性的指標,以及症狀改善或發作的指標。

Claims (26)
- 一種診斷一主體多發性硬化症之方法,包含將從該主體中獲得的一體液樣本與一結合劑(binding agent)接觸,以檢測或測量無細胞核小體或其組分(component)的含量。
- 如請求項1所述之方法,其中,該體液樣本為血液、血清或血漿樣本。
- 如請求項1所述之方法,其中,該體液樣本為腦脊髓液(cerebrospinal fluid)樣本。
- 如請求項1至3中任一項所述之方法,其中,該無細胞核小體之組分包含該無細胞核小體的一表觀遺傳特徵(epigenetic feature)。
- 如請求項4所述之方法,其中,該表觀遺傳特徵為組織蛋白(histone)同功異型體(isoform),例如核心核小體的組織蛋白同功異型體,特別是組織蛋白H3同功異型體。
- 如請求項5所述之方法,其中,該組織蛋白同功異型體為H3.1。
