發明背景 一些穿戴於腕部的電子機械裝置,例如:手錶及活動監測器,可作為消費者產品。這些裝置證明消費者對於即時且連續的生物特徵量測及依時性監測之接受,但該等裝置因有限的精準度、正確性、及功能性而僅有適度的價值。

於如圖1及2所示的實施例中,一用於監測及治療一醫療狀況的可穿戴式裝置100被揭露。該裝置包括一外殼110及一固定該裝置至使用者的固定工具150,諸如一錶帶或其他帶件。該裝置外殼110可容納一或多個感測器元件141、142、143、240、241;一使用者介面130、131;軟體;通訊能力;及治療選擇210~212,其等也可被置於該固定裝置150,例如一錶帶。該可穿戴式裝置也可包括一試驗裝置,其具有數據收集及數據分析的能力。在一實施例中,該可穿戴式裝置可為一電子機械裝置,諸如一智慧型手錶。

為了處理、解釋、呈現、並組織由本揭露內容的該等感測器所偵測及產生的原始數據,將認為智慧型手錶,跟智慧型手機及膝上型電腦一樣,將會必然地併入能夠完成所要求任務的軟體應用。該軟體可包括能夠圖像處理、資料庫操控、遠端伺服器通訊、驅動所併入的感測器、將所觀察到的數據與已知的標準比較、評估風險因子、針對相關性比較來自多於一個感測器的數據流等等的應用。
如同本技藝認可的智慧型裝置,本揭露內容的該可穿戴式智慧型裝置包含通訊能力。本文所揭露之實施例的該等通訊元件可包括有線與WiFi網際網路連接、蜂巢式連接、藍牙能力等等。因此,與該揭露的裝置及其使用者、及資料庫伺服器及/或合格的提供者與專家的通訊以及其等之間的通訊係可行的。
期望明白適當的醫學治療決策必然地涉及偵測及分析各種生物特徵量測,其潛在地結合從業人員與專業人員的知識及患者/使用者本身。本揭露內容的該可穿戴式裝置可被組態以藉由僅該裝置、或通過一些內部軟體分析的組合、與提供者及/或資料庫的通訊、及使用者介面輸入來促進及實行治療。該治療可包括任何本技藝認可的自動藥物遞送,其來自如容器210、211、212或併入如錶帶之該使用者固定工具150的泡鼓包裝(blister packs)。參見如美國專利號6,745,071,其內容被併入以作為參考。該藥物遞送在遞送胰島素至糖尿病患者的情境是眾所周知的,且可被應用至容易分配的藥物以治療所揭露的實施例之狀況。或者,藥物遞送可透過通訊元件來執行,該通訊元件可傳送劑量指示至一遠端裝置(如胰島素幫浦或醫療幫浦)。藥物遞送可藉由以下方式完成:表皮遞送、經皮遞送、靜脈內遞送、或任何其他已知合適的遞送方法,其透過可穿戴式裝置提供藥物遞送,諸如經由經皮基質(transdermal matrix)或針。進一步的治療可包括透過本揭露內容的使用者介面與提供者或資料庫通訊而遞送的指示。在一些實施例中治療可以是自動的、或可由使用者或無論是遠離使用者或是在現場之提供者的決策來觸發。
該可穿戴式裝置的實施例可包括使用者介面130、131,其被組態以控制該裝置的功能及顯示數據及與使用者通訊。諸如智慧型手錶之可穿戴式裝置的一般功能可被併入於本揭露內容。該功能包括但不限於圖形化使用者介面。該介面為了變換顯示、輸入使用者數據、操作控制併入之感測器及軟體、操作試驗元件、操作併入之相機、操作通訊原件、起始治療等等而可為觸控敏感的。
此外,應理解該可穿戴式智慧型裝置可被組態以使被併入於該裝置外殼中的各元件、固定工具、及外部試驗元件可經由電子機械工具而可操作地彼此耦合。即是,該感測器、軟體、通訊、治療、及使用者介面元件全部可以一起操作以作為一個結合的裝置來診斷、分析、及治療醫療狀況,該醫療狀況可由本揭露內容的該可穿戴式裝置來評估。 非限制性的例子如下,其中各種所揭露之實施例的組合可被運用以診斷、監測、及治療健康狀況:
神經毒氣的偵測與治療:神經毒氣的作用機制是乙醯膽鹼酯酶的抑制作用。此導致膽鹼活性增加,其中最早的徵象之一是瞳孔的收縮。處於有神經毒氣暴露風險的戰區內的士兵身上隨時有一以阿托品預先填充的注射器。目前用於偵測神經毒氣暴露的程序由辨識手臂揮動的模式及聽聞警告呼喊所組成。若暴露發生時,反應為穿上防護衣及呼吸器並注射阿托品。本揭露內容之用於偵測神經毒氣的另一方法可為使用手錶上的前置相機來監測瞳孔大小。瞳孔縮小(瞳孔收縮)是神經毒氣暴露的最早表現之一。該手錶相機可偵測瞳孔縮小作為神經毒氣暴露的徵象。另一個神經毒氣暴露的徵象是出汗的增加。此可藉由使用嵌入錶帶或錶背的電極來量測皮膚傳導率而監測。當神經毒氣暴露的徵象被偵測到,該裝置可以從如錶帶中含有的泡鼓包裝自動注射阿托品。在意外/過量的阿托品注射情況下,另一個含有毒扁豆素或毛果芸香鹼(阿托品抗拮劑)的泡鼓包裝可被投與。
急性過敏的偵測及治療:急性過敏的特徵在於低血壓、及組織胺與IgE釋放進入循環。腕部血壓監測器是眾所周知的。手錶腕帶可被修改以量測血壓。組織胺及IgE可使用側流免疫測定而在包括唾液、間質液、及血液的各種體液中被量測。該腕部裝置可藉由離子電滲法獲得間質液、執行組織胺或IgE的測流測定,或者個體可自行收集血液或唾液樣本並執行試驗。急性過敏的另一個徵象是支氣管收縮,其導致血氧飽和度降低。該腕帶可包括如紅外線氧飽和度感測器。在偵測到急性過敏的徵象(低血壓、氧飽和度降低、及/或組織胺及/或IgE增加)之後,該裝置可從該帶上預先填充的容器來遞送腎上腺素、抗組織胺、及/或糖皮質素。
鴉片劑過量:鴉片劑過量的特徵在於瞳孔縮小及換氣不足。此可為醫源性的,因為醫療疏失、或因為暴露於噴霧化的鴉片劑(如酚太尼枸椽酸鹽,就如在莫斯科人質救援行動中所使用者:https://en.wikipedia.org/wiki/Moscow_hostage_crisis_chemical_agent)。瞳孔縮小可利用手錶的前置相機來偵測。監測氧飽和度(如上述)及監測呼吸速率(經由任何數量的已知之光學及聲學技術,諸如美國專利申請公開號20140081100,其被併入本文以作為參考)可辨識鴉片劑過量。一個在固定帶或錶帶上的包括諸如那若松(naloxone)之鴉片劑拮抗劑的容器可投與以逆轉鴉片劑過量。
低血糖可藉由感測橈動脈中核心溫度的下降(如以直接定位於橈動脈上方的手錶感測器)及偵測間質葡萄糖位準來偵測。在這樣的情形下,該固定工具可包括用於低血糖的標準治療,諸如升糖素。
高血壓可如上述被監測。顯著的血壓上升可用芬妥胺(phentolamine)或任何其他快速作用降血壓劑來治療,其被置於該可穿戴式裝置上(如腕帶上)的容器中。再者,血壓趨勢及諸如血氧飽和度的指數可隨著時間被監測。因此睡眠呼吸中止、嗜鉻細胞瘤、及代謝症候群可被診斷。依據此診斷,諸如可尼丁(clonidine)或組合之經皮的抗高血壓藥物可從該可穿戴式裝置投與。
低血壓也可以被監測。在因糖尿病或帕金森氏症而有起立性低血壓(orthostatic hypotension)的患者中,姿勢性的血壓下降是一主要的臨床問題。該手錶可偵測與姿勢改變相關的血壓降低(藉由手錶加速儀或陀螺儀來偵測)並從在該錶帶上的容器投與快速作用升血壓劑(如多巴胺、或力復非他(levophed))。在有腎上腺機能不足或艾迪森氏病的患者中,低血壓可以迪皮質醇(dexamethasone)治療。
心律不整可以藉由使用在該裝置上的電極來監測心率及心律而被偵測。此可藉由投與諸如胺碘酮(amiodarone)、腺核苷、或利多卡因之快速作用抗心律不整劑而被治療。此可替換植入式自動除顫器。
氣喘:重積性氣喘(status asthmaticus)是危及生命的緊急情況。此可藉由增加的心率、呼吸速率、及降低的氧飽和度而被偵測。藉由本揭露內容的該可穿戴式裝置之治療可由投與腎上腺素、迪皮質醇、及/或其他抗支氣管痙攣劑組成。
細菌性敗血症可藉由連續測量降鈣素原(procalcitonin)(來自間質液或微血管血液)及從橈動脈監測紅外線核心溫度而被監測。其他本技藝認可的敗血症指標,諸如c-反應蛋白(CRP)、白血球計數、可溶性膜攻擊複合物、及介白素,可被相似地監測。藉由本揭露內容的該可穿戴式裝置的治療可由投與諸如頭孢曲松(ceftriaxone)之廣效抗生素組成。
肺栓塞可藉由監測氧飽和度、心率、呼吸速率、及d-二聚物濃度而被偵測。治療可由從置於該裝置上的預先填充之容器來分配肝素組成。
礦坑瓦斯或礦場中空氣汙染的偵測:當地下地層被暴露,累積在礦場中的有害氣體包括但不限於甲烷、二氧化碳及相關氣體、硫化氫、及一氧化碳。危險氣體可藉由包括用於各種氣體的感測器於該可穿戴式裝置中來偵測,諸如在錶面上、在錶帶上、或在錶背上。危險氣體的偵測可被連結至啟動通風系統、警報器等等。

Claims (22)
- 一種用於監測及治療一醫療狀況的可穿戴式裝置,其包含: 一裝置外殼及用於固定該外殼至一使用者的工具; 一感測器元件; 一使用者介面,其置於該裝置外殼的主面上; 一軟體元件,其可操作地耦合至該感測器元件且組態以在該使用者介面上顯示數據; 一通訊元件,其可操作地耦合至該軟體元件;及 一治療元件,其置於該裝置外殼或固定工具中且可操作地耦合至該軟體元件,組態以遞送藥物治療至該使用者。
- 如請求項1之可穿戴式裝置,其中該裝置係一數位電子腕錶。
- 如請求項2之可穿戴式電子機械裝置,其中該腕錶係一智慧型手錶。
- 如請求項1之可穿戴式裝置,其中該感測器元件係組態以偵測及監測至少下列之一者:瞳孔收縮、皮膚傳導率、血壓、血氧位準、呼吸速率、橈動脈溫度、心率、心律、間質液、微血管血液、及大氣氣體。
