TARIFNAME HASTALIK VE RAHATSIZLIKLARIN TEDAVISININ SINIRLERIN ELEKTRIKSEL UYARIMI ILE GERÇEKLESTIRILDIGI BIR SISTEM VE SISTEMIN ÇALISMA YÖNTEMI Bulus Konusu Bulus, hastaliklarin ve/veya rahatsizliklarin auriküler vagus sinirinin elektriksel uyarimi ile tedavi edildigi bir sistem ve sistemin çalisma yöntemi ile ilgilidir. Teknigin Bilinen Durumu Otonom sinir sistemi vücudun hayati öneme sahip fonksiyonlarinin tamamini kontrol eden bir sistemdir. Diger bir ifade ile organlarimizin tarafimizca kontrol edilemeyen fonksiyonlarinin tamami otonom sinir sistemi tarafindan yönetilmektedir. Otonom sinir sistemi aktivitesi günlük hayatta degiskenlik göstererek, iç ve dis ortamdaki degisikliklere vücudumuzun uyum saglamasinda görev almaktadir. Otonom sinir sisteminin degisken aktivitesindeki bozulmalar, vücudun iç ve dis çevredeki degiskenlere karsi olan uyumunun bozulmasina sebep olmakta ve otonom disfonksiyonu ve bununla iliskili rahatsizliklar ortaya çikmaktadir. Ortaya çikan rahatsizliklarin, hastaliklarin tedavisinde uyarilmaya bagli olarak sinirlerin aktive edilmesinden faydalanilmaktadir. Sinirlerin uyarilmasinda spor, yoga, nefes egzersizleri, masaj gibi aktivitelerden faydalanildigi gibi sinirlerin elektriksel olarak uyarilmasi da siklikla faydalanilan bir yöntemdir. Çesitli sinirlere çesitli bölümlerden elektriksel sinyaller gönderilerek sinirler uya rilabilmektedir. Farkli rahatsizliklarin tedavisi için farkli elektriksel özellikte sinyaller ile farkli sinirlerin farkli bölümlerinden uyarim gerçeklestirilmektedir. Bulus konusu sistem ile auriküler vagus sinirinin invazif olmayan bir sekilde elektriksel olarak uyarilmasi ile hastaliklarin ve rahatsizliklarin tedavisi saglanmaktadir. Bulusun diger bir amaci, vagus sinirinin kulaktan uyarilarak hastaliklarin ve/veya rahatsizliklarin tedavi edilmesinin saglandigi bir sistemin gelistirilmesidir. Bulusun diger bir amaci, vagus sinirinin iki kulaktan da uyarilarak hastaliklarin ve/veya rahatsizliklarin tedavi edilmesinin saglandigi bir sistemin gelistirilmesidir. Daha özel bir ifade ile vagus sinirinin kulak bölümünden, hastanin ihtiyaçlarina göre ayarlanabilen elektrik parametreleri ile uyariminin saglandigi ve böylece hastaliklarin ve/veya rahatsizliklarin tedavi edildigi bir sistem ve sistemin çalisma yöntemi ile ilgilidir. Bulus konusu sistem ile auriküler vagus siniri bilateral simetrik olarak uyarilmaktadir. Auriküler vagus sinir uyariminin bilateral simetrik sekilde uygulanmasi etkinligin artirilmasini saglamaktadir. Stimülasyonun ayni anda iki kulaga uygulanmamasi sayesinde iki kulak arasindaki olasi elektrik kaçagi azaltilmaktadir ve bu sayede akimin vagusa spesifik kalmasi saglanmaktadir. Simetrik ardisik bilateral uyarim, tedavi etkinligini artirmaktadir. Bu yöntem özellikle psikolojik/psikiyatrik rahatsizliklarda tercih edilen uygulamalardan bir tanesidir. Ilk olarak EMDR (Eye Movement Desensitization and Reprocessing), Türkçe açilimiyla Göz Hareketleriyle Duyarsizlastirma ve Yeniden Isleme, seklinde kullanilmaya baslanan bu yöntem daha sonra ses, dokunma gibi baska uyaranlar için de gelistirilmistir. Fakat aurikülervagus sinir uyariminda su ana kadar bilinen böyle bir kullanim yoktur. EMDR, güçlü bir psikoterapi yaklasimidir. Bugüne kadar her yastan yaklasik 2 milyon kisinin farkli tiplerde psikolojik rahatsizliklarinin basariyla tedavi edilmesini saglamistir. EMDR'nin gelisimi 1987 senesinde, Dr. Francine Shapiro'nun göz hareketlerinin rahatsiz edici düsüncelerin siddetini azaltabildigini tesadüfen kesfetmesiyle baslamistir. Dr. Shapiro bu etkiyi travmaya maruz kalmis kisiler üzerinde bilimsel olarak incelemis ve tedavide saglanan basariyi gösteren çalismasini yayinlamistir (Journal of Traumatic Stress, 1989). EMDR protokolünde kisi, terapistin iki yöne hareket ettirdigi parmagini gözleriyle takip etmektedir. Kisinin terapi sirasinda kendi zihnindeki düsüncelere ve göz hareketlerine ayni anda dikkatini vermesinin, beynin sag ve sol yarim küresini iliskiye geçirdigi düsünülmektedir. EMDR'de uygulanan çift yönlü göz hareketleri disinda çift yönlü isitsel uyarim, çift yönlü dokunma gibi farkli uyarimlardan da yararlanilmaktadir. Kontrollü arastirmalar sonucunda EMDR'nin hastalarin çogunlugunda travma sonrasi stres semptomlarini etkili bir biçimde azalttigi veya yok ettigi, genellikle psikolojik sorunlari ile baglantili olan semptomlarda (endise gibi) da azalma sagladigi görülmüstür. Vagus sinir uyarimi 1997 yilinda tedaviye dirençli epilepsi ve 2005 yilinda tedaviye dirençli depresyon için FDA (Food and Drug Administration) onayi almistir. Vagus siniri 10. kranyal sinir olup parasempatik sistemin en önemli parçasidir. Vagus sinir uyariminin etki mekanizmasi hala tam olarak bilinmemektedir fakat aferent lifler üzerinden beyin kan akisi ve nöral aktiviteyi etkiledigi düsünülmektedir. Auriküler vagus sinir uyariminda (aVNS) farkli stimülasyon parametreleri farkli sonuçlara yol açabilmektedir. Vücudun karmasik fizyolojisi nedeniyle, sürekli ve aralikli stimülasyonun yani sira güçlü ve orta düzeyde stimülasyon, zit fizyolojik etkilere bile neden olabilir. Sürekli vagus sinir uyarimi (VNS), telafi edici sempatik reflekslere yol açacak sekilde, güçlü bradikardi ve kalbin artmis yükleme kosullari olusturdugu öne sürülmektedir. Bulus konusu sistem en temel halinde, sinirlerin uyarilmasi için gönderilecek elektrik akimini üreten en az bir stimülatör (S) ve stimülatör (S) tarafindan üretilen elektrik akiminin kulaktaki vagus sinirine uygulanmasini saglayan en az bir elektrot (E) içermektedir. Bulusun tercih edilen bir uygulamasinda her iki kulaktaki vagus sinirinin de uyarimi gerçeklestirilmektedir. Sistemin bu uygulamasi stimülatör (5) tarafindan iletilen elektrik sinyallerini her bir kulaga ayri ayri ve/veya ayni anda iletecek en az iki elektrota (E) sahiptir. Diger bir ifade ile kulaktaki vagus sinirinin simetrik ve sirali uyarimini saglayacak en az iki elektrota (E) sahiptir. Bulusun tercih edilen bir diger uygulamasinda, kulagin iki farkli noktasindan vagus sinirine elektrik sinyali gönderilmektedir. Bu uygulama, biri kulagin konka ve digeri kulagin tragus bölümüne temas edecek sekilde konumlandirilmis en az iki elektrot (E) içermektedir. Kulagin iki farkli noktasindan vagus sinirinin uyariminin gerçeklestirildigi ve her iki kulakta yer alan vagus sinirinin de uyarildigi uygulama, en az dört elektrot (E) içermektedir. Bulusun tercih edilen bir uygulamasinda, bulus konusu sistem ile her iki kulaktan akim verilmektedir. Tercihen kulaklara ayni anda akim iletilmemekte, ardisik simetrik uyarim yapilmaktadir. Bulusun tercih edilen bir uygulamasinda; 0 Akim bifazik/bipolardir. 0 Akimin atim genisligi 100 mikro saniye - 500 mikro saniye arasindadir. 0 0 ila 155 boyunca sag veya sol kulaga, sonrasinda 0 ila 155 boyunca diger kulaga akim uygulanmaktadir. 0 0-305 boyunca herhangi bir kulaga akim uygulanmamaktadir. 0 Akim uygulama süreleri her iki kulak için esittir. 0 Akimin uygulama süresi disindaki atim genisligi, dalga özelligi, frekans araligi gibi diger özellikleri iki kulak arasinda farklilik gösterebilmektedir. 0 Akimin frekansi her iki kulak için 1-30 Hz arasinda rastgele veya düzenli olarak degisiklik göstermektedir. o Akim sinüs, kare, dikdörtgen veya üçgen dalga formunda olabilmektedir. Bulusun tercih edilen bir uygulamasi, elektrotun (E) kulak üzerine konumlandirilmasini saglayan en az bir kulak askisi (E2) içermektedir. Kulak askisi (E2) kulak kepçesi arkasindan dolanarak, elektrotun (E) kulak üzerine tutunmasini saglayan bir geometriye sahiptir. Bulusun tercih edilen bir uygulamasi, elektrotun (E) üzerine sabitlendigi ve elektrotun (E) hareket edebilir bir sekilde kulak askisi (E2) üzerine sabitlenmesini saglayan en az bir elektrot tasiyici (El) içermektedir. Elektrot tasiyici (El) kulak askisi (E2) üzerinde kendi ekseni etrafinda serbestçe dönebilmekte ve böylece elektrotlarin (E) hastaya göre istenildigi sekilde ayarlanmasina imkân tanimaktadir. Bulusun tercih edilen bir uygulamasi, stimülatör (5) tarafindan üretilen elektrik akiminin elektrotlara (E) tasinmasini saglayan en az bir baglanti elemani (E3) içermektedir. Bahsedilen baglanti elemani (ES) tercihen bir kablodur. Bulusun tercih edilen bir uygulamasinda stimülatör (S), kullanicinin sistemin durumu hakkinda bilgilendirilmesini saglayan en az bir görsel uyari elemani (Sl) içermektedir. Bulusun tercih edilen bir uygulamasinda bahsedilen görsel uyari elemani (Sl) isik saçan bir elemandir. Bulusun tercih edilen bir uygulamasi sistemin açik ya da kapali oldugunu gösteren en az bir görsel uyari elemani (Sl) ve/veya sistemin kablosuz baglanti durumunu gösteren en az bir görsel uyari elemani (Sl) ve/veya uyarimin durumunu gösteren en az bir görsel uyari elemani (Sl) içermektedir. Bulusun tercih edilen bir uygulamasinda görsel uyari elemanlarinin (Sl) tamami, sistem kapali iken isik saçmamakta, sistem açik konuma geldiginde en az birisi sürekli olarak yanmaktadir. Bulusun tercih edilen bir uygulamasinda görsel uyari elemanlarindan (Sl) birisi kablosuz baglanti henüz kurulmamis, baglanilacak cihaz aranirken yanip sönmekte, baglanti kuruldugunda sürekli olarak yanmaktadir. Bulusun tercih edilen bir uygulamasinda, görsel uyari elemanlarindan (Sl) birisi tedavi baslamis ve uyarimin gerçeklestigi periyotta iken sürekli yanmakta, uyarimin gerçeklesmedigi periyotta iken veya tedavi kapali iken isik saçmamaktadir. Bulusun tercih edilen bir uygulamasinda sistemin açik ya da kapali oldugunu gösteren görsel uyari elemani (Sl) ve/veya baglanti durumunu gösteren görsel uyari elemani (Sl) ve/veya uyarim durumunu gösteren görsel uyari elemani (Sl) içermektedir. Bulusun tercih edilen bir uygulamasinda stimülatör (S) üzerinde en az bir ayar bileseni (SZ) yer almaktadir. Söz konusu ayar bileseni (52) stimülatörün (5) en az bir degiskenini ayarlanmaktadir. Bulusun tercih edilen bir uygulamasinda ayar bileseni (SZ) ile stimülatörün (S) açilip kapanmasi kontrol edilmektedir. Bulusun tercih edilen bir uygulamasinda stimülatör (5) en az bir akilli cihaz ile baglanti kurabilmektedir. Bulusun tercih edilen bir uygulamasinda söz konusu baglanti kablosuz olarak saglanmaktadir. Söz konusu akilli cihaz üzerinden, stimülatörün (S) ürettigi elektrik akiminin parametreleri ayarlanabilmektedir. Bulusun tercih edilen bir uygulamasinda, sinire uygulanacak elektrik akiminin darbe genisligi ve/veya frekansi ve/veya tedavi süresince sinirin uyarilacagi ve/veya uyarim sonrasinda bir sonraki uyarim gerçeklesene kadar beklenecek süre ve/veya uyarimin baslatilmasindan sonra tedavinin toplam ne kadar süre boyunca devam edecegi ve/veya elektrik akiminin yogunlugu gibi degiskenler stimülatörün (S) kendisi üzerinden ya da bagli oldugu akilli cihaz üzerinden kontrol edilebilmektedir. Darbe genisligi mikrosaniye, frekans herz, yogunluk miliamper, süre ise saniye cinsinden degiskenleri ifade etmektedir. Bulusun tercih edilen bir uygulamasinda tedavinin baslatilmasi ve sonlandirilmasi da cihaz üzerinden ya da akilli cihaz üzerinden kontrol edilmektedir. Bulusun tercih edilen bir uygulamasinda tedavinin bitmesine kalan süre de akilli cihaz üzerinden takip edilebilmektedir. Bulusun tercih edilen bir uygulamasinda akilli cihaz bir cep telefonudur ya da kablosuz haberlesme imkanina sahip herhangi bir elektronik cihazdir. Bulus konusu sistemin çalisma yöntem en temel halinde asagidaki adimlari içermektedir; stimülatörün (5) açik konuma getirilmesi, stimülatörün kablosuz bir cihaz üzerinden kontrol edilecegi uygulamada stimülatörün (S) kablosuz baglantisinin aktif hale getirilmesi ve en az bir akili cihaz ile baglantinin gerçeklestirilmesi, uyarim parametrelerinin akilli cihaz ve/veya cihazin kendisi üzerinden belirlenmesi, uyarimin ve dolayisiyla tedavinin baslatilmasi. Bulusun tercih edilen bir uygulamasinda uyarim öncesi ve uyarim sonrasi, hastaya ait birtakim veriler kayit altina alinmakta ve bir veri tabaninda depolanmaktadir. Söz konusu veriler, uyarim öncesi ve sonrasi veriler olarak siniflandirilmaktadir. Bulusun tercih edilen bir uygulamasinda söz konusu veriler anketler ve/veya biyolojik veriler ve/veya hastaya ait ses kayitlaridir.
