Description
(54) METHOD OF PRE-OPERATING PREPARATION OF PATIENTS WITH ORTHOPEDIO-TRAUMATOLOGICAL OPERATIONS
1one
Изобретение относитс к медицине, преимущественно к анестезиологии, и может найти применение при анестезиологическом обеспечении ортопедо-травматологических операций .The invention relates to medicine, primarily to anesthesiology, and may find application in the anesthetic management of orthopedic and trauma operations.
1.There is a method of preoperative preparation of patients with orthopedic and traumatologic operations by prior sensory examination and premedication 1.
However, this method does not take into account the possibility of using various methods of anesthesia on the basis of a preliminary study of the thresholds of tactile and pain sensitivity.
The aim of the presentation is an individual approach to the choice of anesthesia.
Способ осуществл етс следующим образом .The goal is achieved by the fact that during the implementation of the method of preoperative preparation of patients during orthopedic and traumatologic operations by preliminary sensometric examination and premedication, thresholds of tactile and pain sensitivity are determined before and after sedation and when thresholds are increased by 9-11 and 19-21% surgery performed under intraosseous anesthesia in combination with neutroleptoanalgesia (NLA), with an increase in thresholds of 19-21 ° / o and 39 41 / o operation is performed under intraosseous anesthesia, and with a decrease and tactile sensitivity by 9-11% and pain by 19-21%, the operation is performed under anesthesia. The method is carried out as follows.
In the preoperative period, tactile and pain thresholds are determined in patients with a special device (sensor), which with the help of a voltage divider provides a 5 V stepped increase in the latter and sends AC pulses with a duration of 1 s at 3 s intervals. If they are respectively equal to 45-50 V and 55-60 V, and 30 minutes after the premedication, consisting of intramuscular administration of droperidol and promedolum in a dose of u, J mg / kg
