Description
Затруднений при проведении регионарной гемоперфузии панкреатодуоденальной зоны у больных с острым панкреатитом (в том числе послеоперационным) по предлагаемому способу не было, В таблице представлены сравнительные результаты возникающих осложнений при лечении предложенным и известным способами .This invention relates to the field of medicine and can be used. medical, practice and experimental medicine. There is a method of treating acute pancreatitis, including intraarterial administration of drugs. However, the known method does not provide a persistent clinical effect and various complications are possible. The aim of the invention is to lengthen the terms of remission. This goal is achieved by the fact that in the method of treating acute pancreatitis, including the intraarterial administration of drugs, regional hemoperfusion of the pancreatoduodenal zone with blood enriched with oxygen is carried out. The method is carried out as follows. Surgically, the portal vein is catheterized through the umbilical vein, the blood is passed through a roller pump through an adsorption column filled with activated carbon, and an oxygenator. Then the blood, purified and enriched with oxygen, is sent to the celiac trunk, catheterized previously under x-ray control through the femoral artery according to Seldinger. A system is added to the arterial end of the oxygenator to administer antibiotics, protease inhibitors, painkillers and other medicines used for the conservative treatment of acute pancreatitis. Medicines are injected into the circulatory system during the entire perfusion. Regional perfusion of the pancreatoduodenal zone is carried out according to the venoarterial method (portal vein celiac trunk) at a rate of 6 O-8 Om / min for 1 -2 h. As an apparatus, known constructions of adsorption columns and perfusion devices can be used. If necessary, the treatment is repeated several times. The proposed method for the treatment of acute pancreatitis has been applied in 20 patients with acute pancreatitis. In 12 patients, the use of the proposed method made it possible in all cases to refuse any surgical interventions, except for the necessary surgical catheterization of the portal vein through the umbilical. In 8 patients who were operated on for various diseases of the pancreas (pancreatic cancer, pancreatic necrosis, etc.), the use of the proposed method made it possible to eliminate the effects of acute postoperative pancreatitis, - Two patients died from causes not associated with inflammation of the pancreas (myocardial infarction pulmonary embolism). The method significantly reduced the likelihood of various complications that require surgical correction (cysts, fistulas, necrosis, etc.), contributed to the creation of temporary functional rest of the pancreas, normalization of homeostasis and biochemical processes in the pancreas, removal of toxemia and inflammation of the known drug-induced toxemia and inflammation of the pancreas, relieving toxemia and inflammation of the known medical and pancreatic glands, relieving toxemia and inflammation of the pancreatic gland and biochemical processes in the pancreas, relieving toxemia and inflammation of the pancreas. means. There were no difficulties in conducting regional hemoperfusion of the pancreatoduodenal zone in patients with acute pancreatitis (including postoperative), the table below presents the comparative results of the complications arising from the treatment by the proposed and known methods.
