Изобретение относитс к медицине а именно к лекарственным средствам дл лечени заболеваний центральной нервной системы пирамидного и экстр апирамидно го характера, сопровождающихс повышением тонуса скелетных мышц и спастичностью. Известно средство мелликтин дл лечени пирамидной спастичности 1. Однако источники получени меллик тина ограничены, и он обладает курареподобной активностью. Известен гидрохлорид 1,2-диметил-4-е-этинил-декагидрохинолола-4-а в качестве химического соединени 21. Целью изобретени вл етс расширение ассортимента эффективных средс дл лечени пирамидной спастичности. Эта цель достигаетс применением гидрохлорида 1,2-диметил-4е-этинилдекагидрохинолола-4-а в качестве сред ства дл лечени пирамидной спастичности . Гидрохлорид 1,2-a-димeтил-4e-этйнил- де каги дрохи нолол а-4 а fдимедрохи н) представл ет собой белый кристаллический порошок с т.пл. 264-265 С, хо рошо растворимый в воде. Димедрохин используют в медицинской практике в виде таблеток по О ,02 г белого цвета с желтоватым оттенком без запаха, горького вкуса и в виде стерильного 1%-ного раствора, который представл ет собой бесцветную прозрачную жидкость с рН 3,2-4,5. Димедрохин вл етс антихолинергическим средством с избирательным н-холиноблокирующим действием. Он оказывает выраженное релаксирующее действие при заболевани х головного и спинного мозга, сопровождагацихс патологически повышенным мышечным тонусом пирамидного и зкстрапиргилидного характера. По фармакологическим свойствам диметрохин вл етс холиноблокатором , действие которого избирательно направлено на н-холинорецепторы мозга. Избирательно блокиру н-холинореактивные невроны головного и спинного мозга, димедрохин обладает также выраженным адренопозитивным ( дофаминергическйм) умеренным антисеротониновым и ганглиоблокирующим действием. Нейромедиаторные эффекты димедрохина и создают фармакологическую основу дл успешной терапии спастичности. Димедрохин улучшает фоновую биоэлектрическую активность головного мозга, нормализует ёшьфа-ритм, увеличивает диапазон усзоенил нав занных ритмов, сглаживает межполушарную асимметрию. Под вли ни ем димедрохина уменьшаютс спастичность и гипертонус, увеличиваетс объем активных движений в суставах паретических конечностей, обща двигательна активность. Биологическа активность паретических скелетных мышц коррелирует с клиническим эффек том. Диметрохр№ снижает повышенное арматурное давление крови, у части больных снижает содержание холестери на в крови. Препарат может примен тьс дл уменьшени спастического компонента при хронических прогрессирующих забо левани х нервной системы (торсионна дистони , спастическа параплеги , атакси и др.). Димедрохинон примен ют при пирамидных парезах и парали чах с экстрапирамидным компонентоми сопутствующей артериальной гипертензией . Ввиду малой токсичности рекомендуетс дл применени в гериатрической и периатрической практике. Димедрохин более эффективен при . спастическихсиндромах, обусловленны поражением спинного мозга. Экстрапирамидна ригидность в меньшей степени поддаетс лечению димедрохином. Дл лечени спастических синдромов, сопровождающих различных заболевани центральной нервной системы, димедрохин примен ют внутрь до еды по 0/01-0,04 г 2-4 раза в день. При хорошей переносимости доза можетбыть увеличена до 0,06 г 3 раза в день. Дл больных старших возрастов обычно требуютс дозы в 2-3 раза меньше, чем -ДЛЯ людей молодых и зрелого возраста . Дет м димедрохин назначают внутрь по 0,004, т. ел 1/5 таблетки, на год жи.эни ежедневно. Продолжитель ность лечени спастичности I - U степени - 10-15 дней. Внутримышечно димедрохин ввод т дет м по 0,1 мл 1%-ног6 раствора на год жизни ежедневно до 10-15 дней. При хорошей индивидуальной переносимости доза может быть удвоена. В виде инъекций диметрохин ввод т под кожу или в мышиу по 1-2 мл 1%-но раствора ; 0,01-0,02) 2-3 раза в день Побочные эффекты при применении димедрохина наблюдаютс редко. Они ыстро исчезают после отмены препараа или при уменьшении его дозы. В димедрохине оптимально сочетают высока найротропна активность с изкой токсичностью и высокой безопасостью . Испытание димедрохина в клиниках ервных болезней подтвердило высокую эффективность препарата при лечении постинсультных, посттравматических парезов и параличей, рассе нного склеоза , энцефаломиелита, церебральных параличей у детей и других заболеваний центральной нервной cиcтe ы с гипертонусом и ригидностью мышц. Димедрохин может примен тьс в форме инъекций и таблеток при спастических параличах и парезах независимо от уровн , сроков и генеза поражени структур нервной систег. По эффективности он не уступает препарату мелликтину. Димедрохин менее токсичен, имеет большую широту фармакологического действи , обладает избирательным релаксирующим действием при спастичности пирамидного характера. Б отличие от мелликтина димедрохин не обладает курареподобной активностью.
