(54) ИМПЛАНТАТ ДЛЯ ЗАМЕЩЕНИЯ ТКАНЕЙ БРЮШНОЙ СТЕНКИ Изобретение относитс к медицине, а именно хирургии, и может быть использовано при операци х на брюшной стенке по поводу злокачественных и доброкачественных новообразований ее больших (особенно рецидивных и после операционных) грыж при травматическо утрате части брюшной стенки. Известен имплантат дл замещени тканей брюшной стенки t содержащий фиксирующий и разделительный элементы из синтетических капроновых волокон . В этом двухслойном имплантате капронова сетка примен етс в сочетании с капроновой тканью, непосредственно прилегающей к органам брюшно стенки 1. В живом организме капроновый имплантат быстро тер ет прочность, что св зано с высокой гидрофильностью капрона. Тканева жидкость, проника в капроновое волокно, раздвигает элементы его структу, что приводит к образованию продольных « поперечных микротрещин с последующим отщеплением частиц капронового волокна, из которого состоит имплантат. Это может привести в течение 1-го, 2-го полугоди после операции к повторному возникновеншр дефекта вследствие того, что имплантат не сможет прртнвосто ть внутрибрюшному давлению. Серьезным недостатком двухслойного имплантата вл етс применение в качестве элемента, подкладываемого под капроновую ,сетку, текстильной ткани из капрона. Текстильный материал , особенно из ткани густого переплетени , плохо прорастает соединительной тканью. Соединительна ткань, образующа с ме щу нит ми нмплантата, неполноценна, преимущественно гиалинизирована . Процесс образовани соединительнотканого-рубца протекает длительно, вновь образованна соединительна ткань подвергаетс процессам фибриноидного набухани и фибриноид«ого покрова, после чего вновь возникают регенеративные процессы. Целью изобретени вл етс снижение послеоперационных осложнений. Поставленна аель достигаетс тем, что в имплахтате дл замещени тканей 61ИОШЙОЙ стенки,содержащем фиксирующий и разделительный элемент из синтетических волокон, фиксирующий элемент состоит из сетчатого полотна, а раделительный элемент - из нетканого материала, причем оба элемента представл ют собой единое целое к(54) IMPLANT FOR THE SUBSTITUTION OF ABDOMINAL WALL TISSUES . В верхней половине живота по средней линии имеетс грыжевое вып чивание , легко вправимое в брюшную полость. Размеры грыжевых ворот 15x5 см. Операци . Под перидуральньм обезболиванием проведена пластика передней брюшной стенки по Напалкову, в нижней части раны дефект закрыт синтетическим дмплантатом из полипропилена . Послойные швы на рану. Послеоперационное течение осложнилось образованием инфильтрата в области послеоперационной раны вне зоны имплантата. Выздоровление. Описанный имплантат обеспечивает замещение дефектов брюшной стенки, при этом достигаетс профилактика послеоперационных осложнений путем исключени образовани нагноени послеоперационной раны, образовани сером, по влени гнойных свищей, некроза кожи над имплантатом. Применение двухслойного имплантата позволит спасти жизнь и вернуть трудоспособность больным с травматической уа затой части брюшной стенки , предотвратить опасное, а иногда и смертельное осложнение, каковым вл етс спаечна кишрчна непрохоимость .
