Abstract
The invention relates to medicine, namely to surgery, and can be used to treat patients with occlusive arterial disease of the lower limbs. The purpose of the invention is to reduce postoperative complications by increasing the reliability of revascularization. This goal is achieved by revision of the igemized lower limb by placing the omentum subfascially on the thigh, popliteal region and tibia with a single-stage deep artery of the femur by an authovenous patch, in which the omentum artery is anastomotized, and site sewn into the femoral vein. The method was applied in 15 patients, in all cases a good stable effect of revascularization was obtained. The method is recommended for implementation in practical health care.

Изобретение относитс к медицине, а именно к хирургии, и может быть использовано дл лечени больных с окклюзионными заболевани ми артерий нижних конечностей .The invention relates to medicine, namely to surgery, and can be used to treat patients with occlusive diseases of the arteries of the lower extremities.
The purpose of the invention is to reduce postoperative complications by increasing the reliability of revascularization.
The method is carried out as follows. A large omentum is resected, a flap is cut out with preservation of vessels, which is placed subfascially on the inner surface of the thigh with a transition to the popliteal area and the posterior surface of the tibia. Plasticization of the deep artery of the femur is performed with an autovenous patch, into which the omentum artery is anastomized, and the omentum vein is sutured into the femoral area on the autovenous site.
A clinical example. Patient I., was admitted with a diagnosis of obliterating atherosclerosis of the lower limb arteries, occlusion of the femoral-popliteal segment to the left, ischemia 4th stage. According to the angiography data, stenosis of the uterus of the deep femoral artery, occlusion of the femoral artery, critical stenosis of the popliteal artery, occlusion of the arteries of the left tibia.
XISurgery: laparotomy, resected large omentum. A flap is cut with preservation of the vessels connecting the end arteries of the omentum in their middle third. Laparotomy wound. The omentum is laid subfascially on the thigh with the transition to the popliteal region and the back of the leg. Endarterectomy was made from the mouth of the deep femoral artery, its plastic was made with an autovenous patch, in which the artery of the omentum was anastomosed, and XI

плата с венозным анастомозом вшита в бок бедренной вены.plate with venous anastomosis is sewn into the side of the femoral vein.
After the operation, pains at rest disappeared, trophic disorders regressed. According to the Doppler sonography, the patency of the vessels and anastomoses is preserved, the muscular blood flow has increased by a factor of 2-3 according to the data of the radionuclide study. During the observation period of more than 1.5 years, a good effect of revascularization is preserved.
Altogether, 15 patients were operated on using the proposed method, in all cases a stable revascularization of the ischemic segment of the lower limb was obtained.
The method is recommended for implementation in practical health care.
