Description
The invention relates to methods for plastic restoration of the tear duct in the absence of a lacrimal sac and the preservation of the lacrimal canaliculi.
For example, the canaliculorhinostomy is known, performed according to the Arroug method and consisting in the fact that the scar tissue that developed at the site of the removed lacrimal sac is excised and the lower lacrimal canal is pulled to a previously made hole in the bone, where the end of the tubule is fixed to the periosteum and nasal mucosa. Thus, the lower lacrimal canal is connected to the nasal cavity.
The dryness of the invention is that in order to extend the lower lacrimal duct from the surrounding tissues, the upper lacrimal canal is separated and the upper lacrimal point is fixed to the periosteum and mucosa at the edge of the previously prepared opening leading to the nasal cavity.
To perform the operation, a skin incision is made, as in the case of a conventional dacryocystorhinostomy, under local infiltration anesthesia with 2% novacain. A tampon moistened with a solution of cocaine with adrenaline is inserted into the nose, and the skin wound is spread with two hooks. The probe of Baumen No. 4 is inserted into the upper and lower lacrimal canaliculi and the soft tissues are stratified in a blunt way. The periosteum is separated with a dissector, the scar tissue is excised from the former hole in the bone made in previous rhinotomy, and the nasal mucosa is carefully prepared. The upper tubule is released first from the nose, and then from the outer side, for which a circular incision is made near the upper lacrimal opening. The incision is extended along the superior lacrimal canaliculi (in which there is a Baumen probe) until it is completely separated. Then, one suture is applied through the tissues surrounding the upper lacrimal point, a Baoumen probe is taken out and the upper lacrimal canal is pulled into the surgical wound. The upper lacrimal point is fixed to the periosteum and the nasal mucosa at the edge of the bone aperture with a pre-cut suture. Through the lower lacrimal canalicus, the upper lacrimal canalicus, and the nasal cavity, a nylon filament is conducted, which in the postoperative period should be pulled daily in one direction or the other.
