Abstract
The invention relates to medicine, in particular to surgery. The goal is to prevent ileostomy edema in patients operated on for inflammatory diseases of the colon. For this, endolymphatic administration of aminoglycoside antibiotics is carried out in a single dose of 4-5 ml of solution before and after the operation. This allows 93% of patients to have normal ileostomy, while traditional methods of managing patients in 37% of cases develop stoma edema with impaired function. The method reduces the time needed to restore intestinal function in the postoperative period.
Description
The invention relates to medicine, in particular to surgery.
The purpose of the invention is to prevent edema of the ileostomy.
Example 1. Patient M., 56 years old, was admitted with a diagnosis of: Nonspecific ulcerative colitis, severe form, total lesion of a chronically continuous course, cachexia. Due to the ineffectiveness of long-term conservative therapy, the patient was offered surgical treatment. Before the operation, genotymicin was administered endolymphatically at a dose of 80 mg and a volume of solvent of 4 ml. Subtotal resection of the colon with ileo-and sigmostomy was performed. In the postoperative period, the endolymphatic administration of gentamicin solution was continued daily at a similar dose and volume for three days. The postoperative period was uneventful. Ileostomy began to function on the second day; there was no stoma swelling. On the 17th day after the operation, he was discharged home in a satisfactory condition.
Example 2. Patient A, 24 years old, was admitted with a diagnosis of Crohn's disease of the colon, total lesion, heavy form, infiltrate of the abdominal cavity, perianal complications. Due to the ineffectiveness of conservative therapy, surgical treatment has been undertaken. In the preoperative period, 60 mg of brulamycin in a 5 ml solution was endolymphatically administered. Produced colproctomy with the imposition of ileostomy The postoperative period was uneventful. In the next postoperative

The proposed method of treatment can reduce the time required for recovery of intestinal function in patients with ileostomy in the postoperative period on the first or second day, while in the treatment by a known method, a long-term recovery of intestinal function is observed (4-5 days).
