Abstract
The invention relates to medicine, namely to traumatology and orthopedics for eliminating deformity of a finger in the form of a "swan neck". The aim of the invention is to restore the function of the finger by eliminating the extensor contracture in the proximal interphalangeal joint. This goal is achieved by Z-shaped dissection of the middle portion of the tendon-aponeurotic stretch together with the triangular ligaments, flexing the finger in the proximal interphalangeal articulation, fixing it transarticularly.
Description
The invention relates to medicine, namely to traumatology and orthopedics, and is used to eliminate deformity of a finger in the form of a "swan neck."
The purpose of the invention is to restore the function of the finger by eliminating the extensor contracture in the proximal interphalangeal joint.
The method is carried out as follows.
Under local or intravenous anesthesia, a posterior semi-oval incision exposes the region of the proximal interphalangeal joint. At the level of the distal third of the main phalanx, the middle leg of the tendon-aponeurotic stretching is obliquely crossed. Further, from the ends of the oblique incision, triangular ligaments are longitudinally cut on one side in the distal and on the other side in the proximal direction. Thus, in general, a Z-shaped incision is obtained. With this manipulation, lateral portions of the tendon-aponeurotic mobilization are partially mobilized.

Через 3 недели после операции удал ют спицу и назначают физиомеханолечение.stretching at this level and moving these portions to the palmar side, which weakens the flexion contracture of the nail phalanx. Straightening the nail phalanx, bend the middle one to an angle of 120 °, and a position reached in the proximal interphalangeal joint is trans-articulated with a Kirschner needle. Hemostasis. Layered seams on the wound. Aseptic dressing. Gypsum immobilization is not used. Skin sutures are removed for 11-12 days. 3 weeks after surgery, the needle is removed and physiotherapy is prescribed.
Example. Sick A., 40 years. Entered the ward with a diagnosis of deformity in the form of the “swan neck of the fourth finger of the left hand. The operation was performed according to the specified method on the fourth finger of the left hand. Transartikul fixation with the needle of the proximal interphalangeal joint. Gypsum immobilization was not applied. The needle was removed after 3 weeks and started exercise therapy and thermal treatment on the finger. Viewed through 3 months. No deformation, full finger function.
