Abstract

The invention relates to medical technology and can be used for the surgical treatment of fistulous passages, in particular, for paraproctitis and in military field surgery. The purpose of the invention is to reduce the morbidity at excision of pathological soft tissues in the fistulous passages located in the wall of the anal sphincter. The device contains a handle 1, a flexible rod 2, a cutting element in the form of a cup 3, a guide sleeve 4. The invention relates to medical technology and can be used for the surgical treatment of fistulous passages, in particular, for paraprostates, as well as for treatment of suppurative wound canals, including those containing wounds (bullets, fragments) i. under conditions of military surgery.

The purpose of the invention is to reduce trauma when excision of pathological soft tissues in the fistulous passages located in the wall of the anal sphincter.

Depending on the nominal size of the truncated cone of the glass 3, the diameter of its leading edge is greater than the diameter of the base by 1-3 mm, which ensures the stabilization of the cutting tool relative to the axis of the fistulous passage during its passage and a complete surgical treatment of the sinuous fistulous passages is achieved.

The device works as follows.

After treatment of the operative field and staining of the fistulous passages with methylene blue of the perianal area, the fistulous course is excised to the muscle plate of the anal pulp. A rectal speculum is inserted into the lumen of the straight collar, and depending on the thickness of the fistulous passage, the diameter of the cutting element 3 is selected.

From the wound side, the free end of the flexible shaft 2 is inserted into the fistula up to its exit through the internal opening of the rectum fistula to the outside.

The rod 2 is pulled through the hole of the guide sleeve 4 and connected, for example, by means of a collet clamp to the handle 1. The surgeon with one hand presses the guide sleeve 4 to the internal opening of the fistula, and the other hand pulls the rod 2 with the cutting element 3 through fistulous passage in the direction of the rectal lumen. The guide bushing 4 prevents damage to the wall of the rectum by the flexible probe when it is pulled.
