Abstract

При этом редрессацию выполн ют до достижени  переразгибани  в п стно-фаланговых суставах и полного разгибани  в межфаланговых суставах .The method of treating a hand contracture is related to traumatology and orthopedics and can be used in surgical, trauma and orthopedic departments of hospitals in the treatment of Dupuytren's contracture. The aim of the invention is to restore the function of the brush by reducing the skin defect. For this, excision of the pathologically modified palmar aponeurosis is carried out. In addition, prior to the operation, redressing is performed using a compression-distraction apparatus. In this case, the redressing is performed until re-extension in the fango-phalangeal joints and complete extension of the interphalangeal joints is achieved.

The invention relates to medicine, namely to traumatology and orthopedics, in particular to methods for treating Dupuytren's contracture.

The aim of the invention is to restore the function of the brush by reducing the skin defect.

The method is carried out as follows.

Положение, достигнутое редрессацией, стабилизируют аппаратом в течение 5-7 дн, а затем провод т операцию известным способом .After treatment of the skin of the hand and forearm under local anesthesia, the compression-distraction apparatus is applied to the forearm and hand. Using the apparatus, gradually perform the closed redressing of the fingers and hand until the hyperextension reaches at least 10 in the phalangeal joints and is fully extended in the proximal interphalangeal joints (180. The position reached by the redress is stabilized by the apparatus for 5-7 days and then the operation is carried out in a known manner.

The essence of the operation is as follows. After treatment of the skin of the hand and anesthesia according to a known technique, an angular incision, moving from the ulnar edge of the hand to its distal section in the transverse

После сн ти  кожных щвов начинают физиомеханотерапию.direction, peel off the skin of the palm. Longitudinal thighs of the palmar aponeurosis of 2-3-4-5 fingers are isolated and dissection is performed in an oblique direction from top to bottom, from the ulnar edge of the hand to the radial, starting from the middle of the palm, followed by a wedge-shaped excision of the middle part of the aponeurosis with longitudinal thrusts and vertical partitions with preservation of its transverse fibers at the level of the n-phalangeal joints. In the presence of pronounced contracture of the fingers, longitudinal weights passing over to the fingers are additionally excised. After hemostasis, the wound is inflated according to the generally accepted method. After removal of the skin, the physiomechanical therapy begins.
