Abstract
Дл повьппени точности и ускорени способа определени полноты ваготомии во врем операции по поводу звенной болезни двенадцатиперстной кишки помещает платиновые электроды субсерозно в област х тела, дна и кардиального отдела желудка. Спуст 3-5 мин измер ют величину окислительно-восстановительного по- тенщ1ала (ОВП) в этих област х. После стволовой или селективной проксимальной ваготомии вновь измер ют величину ОВП. У больных во врем операции по поводу звенной болезни двенадцатиперстной кишки под интубационным наркозом (с исключением, введени атропина и метацина) платиновые электроды помещают субсерозно в област х тела, дна и кардиального от- дела желудка. Хлоркалиевые агар- агаровые мостики, идущие ctr электродов сравнени , укрепл ют на предплечье . Спуст 3-5 мин после субсерозного введени платинового электрода измер ют величину окислителЬно-вос- становительного потенциала (ОБП) в трех област х желудка. После основного этапа операции - стволовой или селективной проксимальной ваготомии вновь измер ют величину ОВП в тех же точках в област х тела., дна и кардиального отдела желудка. При снижении ОВП в указанных област х не менее чем на 70 мВ считают ваготомию полной . После проведенной ваготомии величины ОВП в тех же точках соответствующих областей желудка сос- ставл ют 280-190 и 190 мВ. После ваготомии наблюдают снижение величины ОВП в теле на 210 мВ, в дне - на 150 мВ, и в кардиальном отделе желудка - на 110 мВ. После ваготомии на фоне стимулированной секреции 0,1%- ным сол нокислым гистамином 0,01 мкГкг величины рН составл ют соответственно по отделам 5,3; 6,1; 5,8, что свидетельствует о действительном пода)злении кислотопродуциру- ющей функции. Послеоперационный период протекает без осложнений. Правильность вывода подтверждают измерени ми рН, значени которого после ваготомии увеличиваютс с 5,0; 5,7 и 4,8 до 8,6; 8,7 и 8,9 соответственно .
