Изобретение относитс к медицине, физиологии труда и эргономике и может быть применено, в частности, в травматологии и ортопедии дл оценки функциональных возможностей пальцев поврежденной кисти, контрол эффективности лечени и степени восстановлени двигательных функций, а также может примен тьс в неврологии и протезировании. Целью изобретени вл етс сокращение времени диагностики. На фиг. 1 показано устройство, общий вид; на фиг. 2 - электрическа схема устройства; на фиг. 3 - устройство в пр моугольной проекции, разрез А-А; на фиг. 4 - полна электрическа схема устройства; на фиг. 5 - контактограммы пальцев нормальной и поврежденной кисти. Устройство дл диагностики функционального состо ни пальцев кисти человека (фиг. 1) представл ет собой цилиндр диаметром 30 мм (60 и 90 мм) и длиной 120 мм, выполненный из диэлектрического материала . Оно состоит из цилиндрического корпуса 1 с нанизанными на него кольцевыми проволочными резисторами 2, которые располагаютс соответственно пальцам кисти. Между кольцевыми резисторами имеютс кольцевые прокладки 3. Все резисторы и прокладки на стержне жестко закреплены (фиг. 3). Над кольцеобразными резисторами закрепл ютс плоские контактные пружин щие элементы 4, выполненные в виде зычков из стальной или латунной фольги или фосфористой бронзы толщиной не более 0,1 мм (дл улучщени контакта их можно посеребрить). Размеры и расположение пластин на устройстве должны соответствовать размеру и расположению пальцев кисти. Устройство с помощью разъема 5 подключаетс к мостовой схеме регистрирующего прибора, в качестве которого могут быть использованы самописец Н-338/6, осциллограф К-121, электрокардиограф ЭЛКАР-6 и др. Устройство имеет направл ющий щтырь 6, который обеспечивает совпадение пальцев кисти с контактными пластинами 4. Устройство работает следующим образом . Кисть испытуемого располагают на предлагаемом устройстве таким образом, чтобы направл ющий щтырь 6 находилс между III и IV пальцами и упиралс в межпальцевую складку. Затем испытуемому дают задание произвести 5-6 сгибательных и разгибательных движений с полным захватом устройства в максимальном темпе. Во врем этих движений на регистраторе производ т графическую запись последовательности контакта ладонной поверхности пальцев с поверхностью устройства через посредство контактных элементов. Таким образом на регистрирующей бумаге получают контактограммы п ти пальцев в виде п ти кривых - графических изображений контакта пальцев с поверхностью устройства (фиг. 5). Описанна операци выполн етс как с поврежденной кистью, так и неповрежденной . Запись контактограмм неповрежденной кисти дает возможность получить более точную индивидуальную картину нормы. Полученные контактограммы каждого пальца поврежденной кисти сопоставл ют с контактограммами соответствующих пальцев неповрежденной кисти, которые принимаютс за индивидуальную норму, т.е. более точную дл данного испытуемого, чем среднестатистическа норма. В случае повреждени второй кисти за норму берут среднестатистические показатели контактограмм людей, соответствующих испытуемому по росту и возрасту. При сопоставлении контактограмм анализируют форму кривых, измер ют их амплитуду , угол наклона восход щей и нисход щей частей кривых, период их колебани , вычисл ют частоту колебаний. Поделив полученные показатели на соответствующие показатели нормы и умножив на 100%, определ ют степень нарущени функции каждого пальца в процентах. Таким образом устанавливают степень нарушени следующих функциональных показателей пальцев: объем схвата - по соотнощению амплитуд кривых; скорость сгибани и разгибани - по соотнощению угла наклона восход щей и нисход щей части кривых; длительность схвата - по длительности периода колебани ; темп схвата - по частоте колебаний кривых. A device for diagnosing the functional state of human fingers (Fig. 1) is a cylinder with a diameter of 30 mm (60 and 90 mm) and a length of 120 mm, made of a dielectric material. It consists of a cylindrical body 1 with ring wire resistors 2 strung on it, which are arranged respectively with the fingers of the hand. Between the ring resistors are ring gaskets 3. All resistors and gaskets on the rod are rigidly fixed (Fig. 3). Flat ring spring elements 4, made in the form of tongues made of steel or brass foil or phosphorous bronze with a thickness of not more than 0.1 mm, are fixed above the ring-shaped resistors (to improve the contact, they can be silver-plated). The size and location of the plates on the device must match the size and location of the fingers. The device is connected via a connector 5 to a bridge circuit of a recording device, in which quality the recorder H-338/6, K-121 oscilloscope, ELKAR-6 electrocardiograph, etc. can be used. The device has a guide pin 6, which ensures that contact plates 4. The device operates as follows. The test subject's hand is placed on the proposed device in such a way that guide pin 6 is located between III and IV fingers and rests against the interdigital fold. Then, the subject is given the task to make 5-6 flexion and extension movements with a full grip of the device at the maximum pace. During these movements, the recorder makes a graphic record of the sequence of contact of the palmar surface of the fingers with the device surface through the contact elements. In this way, contactograms of five fingers are obtained on the recording paper in the form of five curves — graphic images of the contact of the fingers with the surface of the device (Fig. 5). The described operation is performed both with a damaged brush and intact. Recording contactograms of an intact hand allows us to obtain a more accurate individual picture of the norm. The obtained contactograms of each finger of the damaged hand are compared with the contactgrams of the corresponding fingers of the intact brush, which are taken as the individual norm, i.e. more accurate for a given subject than the average statistical norm. In case of damage to the second hand, the average indicators of contactograms of people corresponding to the subject in height and age are taken as the norm. When comparing contactograms, the shape of the curves is analyzed, their amplitude is measured, the angle of inclination of the ascending and descending parts of the curves, the period of their oscillation, the frequency of the oscillations is calculated. Dividing the obtained indicators by the corresponding indicators of the norm and multiplying by 100%, determine the degree of dysfunction of each finger in percent. Thus, the degree of disturbance of the following functional parameters of the fingers is established: gripper volume - according to the ratio of the amplitudes of the curves; the rate of flexion and extension in terms of the angle of inclination of the ascending and descending parts of the curves; the duration of the gripper - by the duration of the oscillation period; gripping tempo - according to the frequency of oscillation of curves. The shape of the contactogram curves and the resulting digital indicators make it possible to quickly diagnose the nature of functional impairment and quantify the degree of their impairment.
