Известно компрессионно-дистракционное устройство, содержащее резьбовые стержни, переход щие в крючки с заостренными концами , и муфту, соедин ющую ст ержни 1. Однако с помоц1ью такого устройства нельз достичь дозированной дистракции костных отломков. Целью изобретени вл етс создание дозированной дистракции. Поставленна цель достигаетс тем, что в компрессионно-дистракционном устройстве, содержащем резьбовые стержни , соединенные муфтой, и спицы, резьбовые стержни выполнены в виде болтов с полусферическими головками с отверсти ми дл спиц и снабжены элементами креплени спиц. На чертеже изображено компрессионнодистракционное устройство, общий вид. Устройство содержит резьбовые стержни, выполненные в виде болтов 1 с полусферическими головками 2 с отверсти ми 3 дл спиц 4, болты 1 соединены с помощью муфты 5 и снабжены также элементами 6 креплени спиц 4. Устройство работает следующим образом . Под анестезией производ т два разреза кожи длиной 5 мм: один - в области суставной головки, другой - в области угла нижней челюсти. Спицу ввод т через разрез кожи в суставную головку, другую спицу 4 - через разрез кожи в тело нижней челюсти позади последнего больщого коренного зуба. Свободные концы спиц 4 вставл ют в отверсти 3 в головках 2 болтов 1 и зажимают их элементами креплени в виде фиксирующей щп|1льки. Болты 1 соединены между собой муфтой 5 с внутренней резьбой и фиксируютс в определенном положении с помощью контргаек 7. Если необходимо сблизить отлОмки суставного отростка или наоборот раздвинуть, или повернуть по Оси их, то отворачивают контргайки 7 и вращают муфту 5, после чего новое положение фиксируетс снова контргайками 7. В тех случа х, когда во врем перелома происходит вывих суставной головки, можно спицы 4 вводить под разными углами , дл чего в головке болта I имеютс отверсти 3, просверленные под углами от 30 до 90°. Раны на коже ущиваютс только жилкой. После наложени аппарата больной может питатьс -обычной пищей, назубные щины не накладываютс . В клинике оперировано 10 больных с повреждени ми суставного отростка нижней челюсти, у которых отломки закрепл лись с помощью компрессионно-дистракционного устройства. В отличие от других способов, когда больным не разрещают принимать обычную п-ищу, так как зубы у них фиксированы в прикусе назубными щинами с помощью резиновых колец или после остеосинтеза костным щвом, больные могут сразу есть благодар жесткой фиксации отломком с помощью предлагаемого устройства . Движени в височнонижнечелюстно.м сочленении не ограничены компрессионнодистракционным устройством, а поэтому не возникают контрактуры в суставе. Kpoivie того, сокращаетс количество койко-дней до 5-7. Предлагаемое компрессионно-дистракционное устройство лишено этих недостатков. Применение его несложно и не требует специальной подготовки. The device contains threaded rods made in the form of bolts 1 with hemispherical heads 2 with holes 3 for the spokes 4, the bolts 1 are connected by means of the coupling 5 and are also equipped with the elements 6 for fastening the spokes 4. The device operates as follows. Under anesthesia, two skin incisions with a length of 5 mm are made: one in the region of the articular head, the other in the region of the angle of the mandible. The needle is inserted through the skin incision into the articular head, the other needle 4 through the skin incision into the body of the mandible behind the last molar tooth. The free ends of the spokes 4 are inserted into the holes 3 in the heads 2 of the bolts 1 and clamped with fastening elements in the form of a fixing pin. In the known devices, fractures are applied to the tooth membranes, which cause the occurrence of traumatic gingivitis, lead to the mobility of the teeth, impair the function of respiration, speech and nutrition. The proposed compression-distraction device does not have these disadvantages. Its application is simple and does not require special training. The use of such a compression-and-distraction device for treating, for example, fractures of the articular process provides the possibility of correcting the relationship between the fragments of the articular process, the metered load, the better fixation of the fragments, does not require complex surgical interventions.
