Известен способ общей анестезии путем проведени перидуральной анес зии при травматичных оперативных вмешательствах, заключающеес во вв дении в перидуральное пространство местного анестетика (лидокаин, тримекаин ). Сознание больного при этом варианте перидуральной анестезии выключаетс закисью азота, а расслаблени мышц достигают антидепол ризующими релаксантами Cl Д. Одндко дл этого способа характерн недостаточна анальгези ,высокий рис опасных гипотонии, применение антидепол ризующих релаксантов в обынны дозах, относительно замедленный выход больного из наркоза. Целью изобретени вл етс ускор ние выхода больного из наркоза и уменьшение дов миорелаксантов. Цель достигаетс тем, что соглас но способу общей анестезии путем проведени перидуральной анестезии одновременно провод т нейролептанал гезию второго типа. Способ общей анестезии состоит в проведении перидуральной анестезии с одновременным проведением нейролептанальгезии второго типа. При этом в перидуральное пространство ввод т 2,5%-ный раствор местного анестетика, тримекаина, внутривенно ввод т наркотический L анальгетик короткого действи фента нил и большой транквилизатор дроперидол , провод т искусственную венти л цию легких. Сознание выключаетс эндотрахеальным наркозом закисью азота. Пример. Больной С,, 42 лет Рост больного 168 см, а масса 62 кг Проведена перидуральна анестези , дополненна .нейролептанальгезией второго типа при субтотальной резек ции желудка по поводу рака. Катете ризаци перидурального пространств на уровне накануне операции. Премедикаци и введение в наркоз осуществлены обычным способами. Ввод т 5 мл 2,5%-ного раствора три мекаина в перидуральное пространство . Одновременно внутривенно вво д т 0,5 мг фентанила. Ноддерживаю щиедозы фентанила по 0,1 мг внутрпвенно ввод т четыре раза за 3 ч 20 мин оперативного вмешательства. 2,5%-ный раствор тримекаина за врем общей анестезии в перидураль ное пространство ввод т всего четы раза по 5 мл. Доза антидепол ризую щего релаксанта, тубарина за врем общей анезтевии составила 5 мг, чт в 5-10 раз меньше используемых доз при нейролептанальгезии второго типа или перидуральной анестезии, когда они провод тс отдельно. Доза депол ризующего релаксанта, дитилина, за врем анестезии 360 мг. Течение анестезии гладкое. Клинические признаки боли почти не выражены (частота сердечных сокращений не выше 70. в мин, теплые и сухие кожные покровы). Экструбаци за 5 мин до конца операции на фоне хорошего ббёзболивани , сного сознаний и эффективного самосто тельного дыхани . Больной сам перебралс на каталку, в ночь после операции отошли газы и стала выслушиватьс перистальтика. За все врем пребывани в послеоперационном отделении (три дн ) температура была нормальной. В первые сутки после операции стал ходить. Перидуральна анестези и нейролептанальгези второго типа проводились 15 больным при таких оперативных вмешательствах как резекци желудка, гемиколонэктоми , холецист-д эктоми . Об усилении анальгезии относительно обезболивани при перидуральной анестезии и нейролептанальгезии второго типа, проводимых отдельно, свидетельствует отсутствие клинических признаков боли во врем операции у больных. Частота сердечных сокращений у них во врем анестезии всегда была ниже частоты сердечных сокращений накануне операции, а артериальное давление испытывало тенденцию к умеренному снижению. Ксзжные покровы былы розовыми, тенлыми-и сухими. Больные сами говорили нам об отсутствии боли, когда их Экстубировали за 5-10 мин до конца операции. Свидетельством хорошего блока болевой импульсации было и быстрое восстановление почти не затронутого болевым ноцицептивным потоком сознани сразу после прекращени ингал ции закиси азота. Следствие хорошего обезболивани отсутствие функциональной после.операционной непроходимости кишечника у всех 15 больных. Все это выгодно отличает проводимую нами общую анестезию от одной перидуральной анестезии или от одной нейролептанальгезии второго типа, когда больные долго выход т из наркоза, по ходу анестезии часто возникают клинические признаки боли и нередким осложнением вл етс послеоперационна функциональна непроходимость кишечника.
