Способ определения приоритетности реваскуляризации у больных ишемической болезнью сердца с сопутствующей вазоренальной гипертензией по результатам гипотензивной пробы, включающей предварительный контроль артериального давления и состояния коронарного кровообращения путем регистрации электрокардиограммы, введение гипотензивного препарата с последующим контролем артериального давления и состояния коронарного кровообращения в течение каждого часа и анализом динамики изменения сегмента ST и зубца Т, отличающийся тем, что через 30-60 мин после введения гипотензивного препарата дополнительно проводят эхокардиографическое обследование, регистрируя индекс сократимости левого желудочка (WMS ), фракцию выброса (ФВ), время раннего диастолического спада скорости трансмитрального потока (DT), отношение скоростей трансмитрального потока в фазу раннего наполнения левого желудочка и фазу систолы предсердия (Е/А), а электрокардиограмму снимают на протяжении фармакологического действия гипотензивного препарата в режиме холтеровского мониторирования, и в случае,когда отмечают отрицательную динамику систолической и диастолической функции левого желудочка, выполняют реваскуляризацию миокарда, а в случае положительной динамики производят пластику почечных артерий.A method for determining the priority of revascularization in patients with coronary heart disease with concomitant vasorenal hypertension according to the results of a hypotensive test, including preliminary monitoring of blood pressure and coronary circulation by recording an electrocardiogram, administration of a hypotensive drug with subsequent monitoring of blood pressure and coronary circulation during each hour and analysis of the dynamics changes in the ST segment and T wave, characterized in that after 30-60 min after administration of the antihypertensive drug, an echocardiographic examination is additionally performed, recording the left ventricular contractility index (WMS), ejection fraction (EF), the time of early diastolic decline in the velocity of the transmitral flow (DT), the ratio of the velocities of the transmitral flow in the phase of early filling of the left ventricle and atrial systole phase (E / A), and the electrocardiogram is removed during the pharmacological action of the antihypertensive drug in the Holter monitoring mode, and in the case when the negative dynamics of systolic and diastolic functions of the left ventricle, myocardial revascularization is performed, and in the case of positive dynamics, renal arteries are plasticized.
