Способ основан на экспериментальных исследованиях пластики дефекта внепеченочных желчных протоков у собак аутовенозным трансплантатом, подготовленным предварительной консервацией на протяжении 7 дней в поддерживающей питательной среде 199 при (+2) - (+4)°С на дренаже-каркасе. Впервые доказана высокая эффективность способа: к полутора месяцам вся имплантированная аутовена прорастает соединительной тканью, и создается соединительно-тканный каркас вновь образующегося участка внепеченочного желчного протока с полной эпителизацией всей внутренней поверхности аутовенозного трансплантата. Способ с успехом применен в клинических условиях. Клинико-экспериментальные данные с изучением морфологической структуры специально подготовленных и имплантированных в различные сроки участков аутовены, представлявших в итоге соединительно-тканный каркас с выстланным по внутренней поверхности желчным эпителием позволили сделать заключение о возможности использовать в качестве пластического материала иссеченный и предварительно разбужированный заращенный фрагмент пупочной аутовены, расположенный ближе к пупочному кольцу. Иссеченный и разбужированный фрагмент пупочной вены представляет собой подготовленный организмом соединительно-тканный каркас, который, как показали успешно выполненные в клинических условиях операции, может быть рекомендован для использования в качестве пластического материала в практическом здравоохранении при восстановлении дефекта гепатикохоледоха у пациентов с рубцовыми стриктурами большой протяженности, у больных при резекции опухоли внепеченочных желчных протоков с применением дренажа-каркаса.The method is based on experimental studies of the plasty of the extrahepatic bile duct defect in dogs by an autovenous graft prepared by preservation for 7 days in supporting nutrient 199 at (+2) - (+4) ° C on the drainage framework. The high efficiency of the method was proved for the first time: by the age of one and a half months, the entire implanted auto-vein germinates with connective tissue, and a connective-tissue framework of the newly formed part of the extrahepatic bile duct is created with complete epithelization of the entire inner surface of the autovenous graft. The method is successfully applied in a clinical setting. Clinical and experimental data with the study of the morphological structure of specially prepared and implanted at different time sections of autoovens, which ultimately represented the connective tissue framework with the biliary epithelium lined on the inner surface, allowed us to conclude that it is possible to use an excised and previously awakened overgrown umbilical fragment autologous located closer to the umbilical ring. The excised and awakened fragment of the umbilical vein is a connective tissue skeleton prepared by the body, which, as successfully performed in clinical settings, can be recommended for use as a plastic material in medical practice for restoring the hepaticoholedochus defect in patients with long-lasting strictures, in patients with tumor resection of the extrahepatic bile ducts using a drainage-skeleton.
