Description
The invention relates to medicine, namely to the design of an instrument for intracavitary laser coagulation in gastroenterology.
A known design, which is a hollow catheter with an optical fiber, the output end of which is fixed in a cylindrical sleeve with a coaxial hole [1]. The design drawback is the possibility of contamination of the end of the optical fiber through the hole of the sleeve with the biological contents of the gastrointestinal tract, which leads to a loss of power of the coagulating laser radiation.
As a prototype, one should consider the design of a fiber instrument for laser gastroscopy, which is a flexible fluoroplastic tube with an optical fiber mounted on a distal section in a sealed sleeve [2]. Laser coagulating effect is performed when the sleeve is in contact with a portion of biological tissue. The disadvantage of this design is its limited use in operative gastroenterology due to the lack of control over the heating temperature of the sleeve, through which coagulation of the pathological area inside the hollow organ is performed. When the sleeve overheats, there is a risk of organ wall perforation, which is dangerous for the patient with complications.
In the proposed design, this problem is solved due to the fact that in the fiber instrument for laser gastroscopy, consisting of a fluoroplastic tube with an optical fiber fixed in the distal part in the sleeve with a sealed output end, the second optical fiber is additionally contained in the distal portion located in the sleeve of which a temperature sensitive element is placed.
The technical result implemented in a fiber instrument for laser gastroscopy with a thermosensitive element is to increase the safety of operation of the gastroscope and increase the life of the instrument.
The essence of the proposal is illustrated using figure 1, depicting the basic construction of a fiber instrument for laser gastroscopy.
5.The tool has a fluoroplastic tube 1, an optical fiber 2, mounted in the distal part in the sleeve 3, the output end of which is sealed. In addition, the tool contains an additional optical fiber 4 in the distal portion of the sleeve located in the sleeve of which the heat-sensitive element 5 is placed.
Instrument with a fiber instrument for laser gastroscopy works as follows. The body of the gastroscope 6 (Fig. 2), having the instrumental channel 7, is inserted into the organ cavity 8. Observation is performed through the eyepiece 9, the instrument 1-5 is inserted through the instrumental channel 7, while the terminal portion of the instrument is brought to the pathological section 10 of the biological tissue and the end face of the sleeve bring into contact with section 10. Laser radiation 11 coagulates, for example, a neoplasm 10 in contact with the end face of the sleeve 3. The heat-sensitive element 5 (Fig. 1) reacts to the coagulating heating of the sleeve 3 and the neoplasm 10, a signal about agreve transmitted to the optical fiber 4 (Figure 1) on the indicator 12 (Figure 2) to inform about the heating temperature of the biological tissue.
Here is a specific example of a fiber instrument for laser gastroscopy.
In the experiments, the domestic gastroscope EGDB-VO-4 with a hollow instrumental channel was tested. The fluoroplastic tube was made of Tefzel brand material. As the optical fiber 2 (Fig. 1), a fiber with a core of fused silica with a reflective optical sheath made of organosilicon resin was used. As the optical fiber 4 (FIG. 1), a quartz-quartz type fiber with a light guide core diameter of 100 microns was used. The heat-sensitive element 5 is made of monocrystalline silicon. A sample of the instrument provided temperature control in the range of 30-110 ° С with an accuracy of ± 0.2 ° С.
A fiber instrument for laser gastroscopy can find application in operative gastroenterology for coagulation of bleeding and small tumors, as well as for hyperthermic therapy of tumors.
