The author developed a vaginal tip for gynecological pneumovibromassage.

It includes a rod (can be made of fluoroplastic) with a diameter of 8-10 mm and a length of 160-180 mm (1), along the axis of which a through central channel (2) is made. The distal end of the shaft is smoothly rounded and muffled. Along the length of the distal portion of the rod, radial holes (3) are uniformly located, communicating with the central channel. The proximal section of the central channel is connected to the fitting (4). A pneumatic balloon (5) is worn from the distal end of the tip (a condom for a vaginal ultrasound probe can be used). At the border of the distal third of the rod, the pneumatic balloon is hermetically fixed by a cuff. The nozzle of the tip is connected to the pneumatic hose of a pneumatic vibro massager. Gynecological massage has been used to treat various pathologies of the genital organs since 1861. Manual gynecological massage techniques are not standardized, are subjective in nature, do not allow to objectively control the strength of the mechanical effect on the organs and tissues of the genitals. A variety of devices for vibration massage are not dynamic and are intended primarily for sexual satisfaction.

Previously, we developed a cervical tip for vibromassage of the endocervix and vacuum aspiration of the contents of the cervical canal, made in the form of a rod with a central canal and openings communicating with it, it is included in the set of the Cervical Vibro-Aspirator - VAC (Utility Model Certificate No. 21345, registration certificate of the Ministry of Health of the SR of the Russian Federation FS 022F2004 / 1073-05 dated 01/11/2005). Mechanical vibration is transmitted to the cervical tip from the vibrator pistol through an elastic-elastic hose, and the radial holes in it serve to vacuum aspirate the contents of the endocervix. In the middle part of the cervical tip, a circularly arranged conical protrusion limiter is made. Therefore, due to design features, the cervical tip cannot be used for vaginal gynecological pneumovibromassage.

We have developed a vaginal tip for gynecological pneumovibromassage. The proposed device is shown in figure 1, where:

The device is used as follows.

The woman is located in the gynecological chair. A condom is put on the device and it is inserted into the vagina to the cervix (to a depth of 6-8 cm), the fitting is connected to the pneumatic hose, and the proximal tip is fixed with a special holder fixed to the gynecological chair (to maintain a stable position of the tip in the vagina during the procedure) .

Gynecological pneumovibromassage can be implemented, for example, in a pulsed mode using the PVM-R-01 Sanos apparatus (commercially available). Its technical capabilities allow for strictly metered generation of pneumatic pulses that enter the developed tip with a smooth increase and decrease in compression, uniformly and rhythmically acting on the walls of the pneumatic balloon, and cause sinusoidal membrane oscillations with alternating fronts of positive and negative pressure. A programmed uniform intermittent pressure and low-frequency vibration appear on the walls of the vagina. Two components of the effect: massage + vibration, cause a synergistic effect. The duration of the pneumatic pulse and pauses (for example, 2 seconds - pulse, 2 seconds - pause), as well as the pressure of the supplied air, which cannot exceed 40 kPa, is set by the medical personnel individually for each patient using the electronic control unit of the device. During a pause, the air spring completely drops.

Compression in a pneumatic balloon is continuously adjustable within therapeutic doses. The duration of the procedure is 10-15 minutes. The course of treatment is 10-15 sessions, performed daily or every other day.

The mechanism of this method using the developed tip significantly differs from both manual gynecological massage and the action of various vibrators, primarily due to its dynamism, standardization of treatment parameters and the ability to strictly dose the procedure.

Also in the vaginal tip there are no rotating and rubbing surfaces that could have a damaging effect on the mucous membrane of the vagina and cervix.

The rhythmic alternation of the phases of compression and pauses provides both a constant flow of arterial blood to the pelvic organs and an outflow of venous blood and lymph, which has the most favorable effect on the cells and tissues of the pelvic organs.

Cyclically intermittent pressure and vibration factor stimulate mechano- and baroreceptors. Reserve capillaries and arterioles are revealed, lymphatic drainage is enhanced, metabolic trophic processes, tissue respiration are improved. The use of two physical factors simultaneously has a "resonant" therapeutic effect.

Using the proposed device is non-invasive, easy to implement and compatible with other treatment methods, and also reduces staff labor. The use of this tip for gynecological pneumovibromassage in a pulsed mode in 62 patients with the above pathology allowed to reduce the time and increase the effectiveness of complex treatment.

Side effects and complications were not noted.
