Abstract
Description
The invention relates to the field of dentistry, cosmetology and hygiene and can be used for whitening tooth enamel, including at home.
Teeth enamel whitening is currently an extremely common procedure and is usually carried out both in dental clinics and beauty salons. The development of a safe and effective technology for reducing the level of exogenous pigmentation of tooth enamel is an important task in the cosmetology industry.
Enamel is the hardest tissue in the human body, which is explained by the high content of inorganic substances - up to 97%. The thickness of the enamel layer differs in different areas of the crown of the tooth and can reach 2.0 mm, and disappears at the neck of the tooth. Proper care of tooth enamel is one of the key aspects of human personal hygiene.
Despite its hardness, enamel has good permeability to calcium and fluoride ions contained in saliva (or toothpastes and mouthwashes), as well as to pigments contained in food and drinks. Tooth enamel can have different shades - from yellow to various shades of gray and white, which depends on its transparency coefficient. The main factor of endogenous pigmentation is the sorption of organic compounds present in food, drinks, and tobacco smoke in the dental tubules.
The effectiveness of teeth whitening depends on many indicators: the technology used, the quality of the patient’s tooth enamel and his susceptibility to the influencing elements, the current shade of the tooth enamel, and the causes of darkening. In various cases, teeth can be whitened by either 2 or 14 shades.
Most often, the process of teeth enamel whitening is based on the oxidation reaction of chromophore groups of colored substances, as a result of which they lose color. One of the suitable oxidizing agents is hydrogen peroxide, or substances that release it (carbamide peroxide, sodium percarbonate, sodium perborate and others).
Most medical clinics use gels with a high content of hydrogen peroxide (up to 35%). Peroxide molecules freely reach dentin (dense tissue under the enamel), where they break down into water and atomic oxygen - a strong oxidizing agent with high chemical activity. Oxygen destroys dye molecules. Therefore, the tooth becomes “discolored.” Special highly concentrated gels are applied to the teeth and activated with light or laser to initiate the breakdown process and produce active oxygen. However, high peroxide content requires special care and maximum isolation of soft tissue. Side effects such as hypersensitivity often occur after the procedure; if the technology is not followed, severe burns of the oral mucosa are possible. However, working with such aggressive compounds is possible only under the supervision of a highly professional dentist and is not available as a cosmetic procedure.
There is a known method of whitening tooth enamel, which includes pre-treatment of teeth with an activating composition containing catalysts and having a pH of 8-9, followed by application to the surface of the teeth of a whitening composition containing a peroxide bleaching agent in a concentration equivalent to 0.1-6.0% hydrogen peroxide , and irradiation of the whitened teeth surface with blue light with a wavelength of 400-450 nm for 10-30 minutes. In this case, the activating composition contains sodium molybdate and sodium tungstate as catalysts with the following content of components, wt. %: sodium molybdate 0.1-1.0; sodium tungstate 0.1-1.0; Surfactant 1.0-3.0; water - the rest. [Patent No. 2697401, A61K 8/19, A61K 8/22, A61K 8/46, A61Q 11/02, 07/23/2018].
The disadvantage of the above method is the inability to completely isolate the soft tissues of the oral mucosa, since regardless of the method of protection, the interdental space cannot be protected. Another disadvantage is the use of a catalyst containing metals, which can cause negative effects on the human body as a whole.
A known method of teeth whitening includes the step (!) of applying an effective amount of an oral care composition containing a bleaching agent to the teeth and allowing the composition to remain on the teeth for 45 to 90 seconds. Then stage (ii) of brushing teeth with the same composition is carried out. In this case, the bleaching agent is hydrogen peroxide in combination with polyvinylpyrrolidone and the concentration of the said bleaching agent in the composition is equivalent to 0.1-5% hydrogen peroxide. In addition, the composition contains an abrasive substance with calcium. [Patent No. 2608441, A61Q 11/00, A61K 33/40, A61P 1/02, 12/14/2012].
The disadvantages of this method are low efficiency due to the slow formation of active oxygen without initiation, and the presence in the composition of an abrasive substance that injures tooth enamel.
There is also a known method of teeth whitening, which involves applying a tooth whitening agent to the tooth and continuously irradiating the tooth with light. In this case, the teeth whitening agent contains hydrogen peroxide in a concentration ranging from 2 wt. % up to 10 wt. % based on the total weight of the teeth whitening agent. The light that is continuously irradiated to the tooth is generated by the light source during the irradiation time period. Here, the light has a wavelength ranging from 375 nm to 420 nm, and the light source emits light at a fluence ranging from 0.3 mW/cm 2 to 8.1 mW/cm 2 . [Patent No. 2705908, A61Q 11/02, A61C 19/06, 10/27/2016].
The disadvantage of this known method is also the high risk of side complications due to the inability to completely isolate the soft tissues of the oral mucosa from the effects of high concentrations of hydrogen peroxide.
Стоит отметить, что применение перекиси водорода (перекиси карбамида) в высоких концентрациях ограничено для средств домашнего отбеливания во многих странах, более безопасным компонентом таких средств признана фталимидопероксикапроновая кислота (РАР).It is worth noting that the use of hydrogen peroxide (carbamide peroxide) in high concentrations is limited for home whitening products in many countries; phthalimidoperoxycaproic acid (PAP) is recognized as a safer component of such products.
Phthalimidoperoxycaproic acid is a synthetic organic peroxyacid derived from caproic acid and phthalimide. This compound is an odorless, white crystalline powder at room temperature. It is slightly soluble in water and is a strong oxidizing agent.

The inclusion of a large number of additional components reduces the effectiveness of tooth enamel whitening using a known product, which is due to the slow formation of active oxygen without initiation, as well as the proposed method of applying a known composition to tooth enamel, by adding it to toothpaste, gel, mousse, aerosol, chewing gum , lozenge, etc. In addition, the inclusion of a large number of additional components does not provide a complete understanding of all the effects, including negative ones, exerted by this composition, and the presence of an abrasive substance in the composition clearly leads to injury to tooth enamel.
The invention is aimed at solving the problem of reducing the risk of side effects of tooth enamel whitening and reducing the likelihood of damage or irritation to the gums and mucous membranes of the oral cavity, while simultaneously expanding the functionality of the method of teeth enamel whitening.
0,1%.The essence of the invention lies in the fact that in the method of whitening tooth enamel, including the use of a bleaching agent for tooth enamel and irradiation of the bleached tooth surface with light in the wavelength range of 350-500 nm for 5-15 minutes, it is proposed to use two mixed components as a bleaching agent : the first component in the form of a transparent hydrogel containing carbamide peroxide in a concentration from 1 to 17% wt., and the second component containing phthalimidoperoxycaproic acid in a concentration from 1 to 10% wt., after irradiation, turn off the light source, and the tooth enamel whitening agent leave on the surface of the teeth to be whitened for another 5-10 minutes, while the bleaching agent is applied to the surface of the teeth to be bleached using a tray, for which purpose a layer of the first component is first placed in it and the layer of the first component is covered with a layer of the second component, so that the equivalent the amount of hydrogen peroxide in the contact area of the bleached tooth surface and the oral mucosa with the compositions used did not exceed 0.1%.
A lamp emitting in the wavelength range 350-500 nm can be used as a light source for irradiation.
The use of two mixed components as a bleaching agent: the first component in the form of a transparent hydrogel containing carbamide peroxide in a concentration of 1 to 17% wt., and the second component containing phthalimidoperoxycaproic acid in a concentration of 1 to 10% wt., when applying the bleaching agent on the surface of the teeth to be whitened, using a tray with preliminary placement of a layer of the first component in the tray and covering it with a layer of the second component, helps reduce the risk of side effects of tooth enamel whitening and reduce the likelihood of damage or irritation to the gums and mucous membranes of the oral cavity due to the fact that the direct contact of the bleached tooth surface and the oral mucosa occurs only with the safest layer containing phthalimidoperoxycaproic acid, while the total equivalent amount of hydrogen peroxide in the zone of said contact in the proposed method does not exceed 0.1%, and also due to the fact that in the composition of the mixed The components used in the proposed method do not use abrasive additives that can damage the integrity of tooth enamel. At the same time, the low concentration of hydrogen peroxide creates the prerequisites for using the proposed method in home teeth enamel whitening, which helps expand the functionality of the method of teeth enamel whitening by expanding the scope of its application.
Reducing the risk of side effects of tooth enamel whitening and reducing the likelihood of damage or irritation to the gums and mucous membranes of the oral cavity is also facilitated by reducing the time of irradiation of the oral cavity and exposure to chemically active components due to the fact that irradiation of the bleached surface of the teeth is carried out for 5-15 minutes, while carbamide peroxide from the first component quickly decomposes during irradiation with the formation of highly active free radicals, and after the initial initiation the light source is turned off and the process continues for about 5-10 minutes. Due to this, the induction period of the chain reaction is significantly reduced, since the active particles continue the chain of radical transformations in the layer of phthalimidoperoxycaproic acid; at the same time, a significant amount of oxidizing radicals are released during the breakdown of phthalimidoperoxycaproic acid, while the rate of the oxidation reaction of pigments on the surface of tooth enamel increases significantly.
The proposed method can be illustrated by the following example:
S18.A 51-year-old patient came to the dental clinic with a request for teeth whitening. A visual examination showed that the patient’s tooth enamel has a yellowish tint; in addition, there are stains and plaque on the teeth. The tooth tone on the RS-20 scale was S18.
The patient had his teeth whitened using the claimed method.
A layer of the first component in the form of a transparent hydrogel containing carbamide peroxide at a concentration of 17% by weight was placed in the mouth guard, then it was covered with a layer of the second component containing phthalimidoperoxycaproic acid at a concentration of 10% by weight. The tray was then placed on the teeth to be whitened. After installing the tray on the teeth to be whitened, a light source was directed onto it using an LED lamp emitting in the wavelength range of 350-500 nm, and the photoinitiation process was carried out for 15 minutes. After the initial initiation, the light source was turned off and the tray was left on the teeth to be whitened for another 10 minutes. The tone of the teeth on the RS-20 scale after the procedure was S16. The bleaching efficiency was 4 shades on the RS-20 scale. At the same time, according to the patient: the discomfort of the facial muscles during the procedure was minimal, there was no discomfort in the oral cavity (burning, pain, swelling), and the state of health after the procedure was normal. To achieve greater effect, the described procedure can be repeated 2-3 times.
Thus, the proposed method provides high efficiency of teeth whitening at a low concentration of hydrogen peroxide, which helps reduce the side effects of teeth whitening, and also reduces damage or irritation to the gums and mucous membranes of the oral cavity.
