TECHNICAL FIELD
The present invention relates to liquid compositions of dantrolene or a pharmaceutically acceptable salt thereof and methods for their use in the treatment of diseases.
BACKGROUND OF THE ART
(1):Dantrolene (1-{[5-(4-nitrophenyl)-2-furyl]methylidenamino}imidazolidine-2,4-dione) has the structure of formula (1):
(SIDS).Dantrolene is the emergency drug of choice for the treatment of malignant hyperthermia (“MH”) and is widely available in most sites where anesthetics are delivered. First synthesized in 1967, dantrolene was initially used to treat muscle spasms in 1975 and later received FDA approval in 1979 to treat MH. Dantrolene is recognized as a powerful muscle relaxant and against spasticity associated with nerve damage. Since its initial discovery, dantrolene has been used to prevent and treat other life-threatening conditions such as overdose of psychoactive substances such as “ecstasy” (N-methyl-3,4-methylene-dioxyphenylisopropylamine), heat syncope, neuroleptic malignant syndrome and ischemic damage to the peripheral nervous system, and may be important in preventing sudden infant death syndrome (SIDS).
Dantrolene is very poorly soluble in water. The poor solubility of dantrolene greatly complicates its use. For example, DANTRIUM™ is dantrolene sodium supplied in 20 mg vials that must be diluted with 60 ml of sterile water before intravenous administration. The recommended dose of dantrolene for the treatment of MH is 1 mg/kg to about 10 mg/kg. Thus, to treat MH, an 80 kg subject would require a rapid infusion of up to 2400 mL of the drug.
In addition to poor solubility, dantrolene solutions have a high pH level. The pH of DANTRIUM™ is approximately 9.5. RYANODEX®, an improved formulation of dantrolene sodium that can be reconstituted to 50 mg/ml, significantly increases the rate of administration of dantrolene sodium. However, reconstituted RYANODEX® also has a high pH level of around 10.3. Currently, due to high pH levels, dantrolene preparations cannot be administered subcutaneously or intramuscularly, only intravenously. Of course, care must be taken to prevent extravasation of the drug into surrounding tissues to prevent tissue necrosis.
There is a need for new dantrolene compositions with suitable concentration and pH levels that make them suitable for intravenous administration.

Супернатант дантроленовой кислоты и раствор дантролена натрия разбавляли 100- и 200-кратно 25%-ным ацетонитрилом для анализа методом СВЭЖХ, описанным ниже. pH levels in PEG400 of various dantrolenic acid:dantrolene sodium samples at a concentration of 10 mg/ml. 13.0 mg of dantrolenic acid was dissolved in 1.3 mL of PEG400 to obtain a concentration of 10 mg/mL of dantrolenic acid. 12.7 mg of dantrolene sodium was dissolved in 1.27 mL of PEG400 to obtain a concentration of 10 mg/mL of dantrolene sodium. The liquid in the dantrolenic acid sample, after vortexing, was cloudy and some solid remained at the bottom of the Eppendorf tube, indicating a saturated solution. The dantrolene sodium sample was not turbid, indicating complete dissolution of dantrolene sodium. Both tubes were centrifuged for 5 minutes at 3900 rpm at room temperature to separate the sample into a supernatant (dantrolenic acid or dantrolene sodium in solution) and a pellet (insoluble material). The dantrolenic acid sample had a precipitate form, while the dantrolene sodium sample did not have this form. The supernatant of the dantrolenic acid sample was transferred to a clean tube. The dantrolenic acid supernatant and dantrolene sodium solution were diluted 100- and 200-fold with 25% acetonitrile for UHPLC analysis described below.

The column was equilibrated with 75% mobile phase A. The percentage of mobile phase A was reduced to 57% A within the first 4 minutes. The column was then washed with 30% A for 2 minutes, returned to 75% for 0.1 minute, and re-equilibrated with 75% A for 3.9 minutes for a total run time of 10 minutes. A 10 μL sample volume was injected and the analytes were detected by UV light at 385 nm. Dantrolene eluted after approximately 2.3 minutes. Peak areas were determined by manual integration.
Значения концентрации на кривой доводили до 5,84, 11,68, 23,36, 46,72 и 93,45 мкг/мл для учета разницы в молекулярной массе между дантроленовой кислотой (314 г/моль) и дантроленом натрия (336 г/моль).The standard curve was prepared by dissolving 1.0 mg of dantrolene sodium in 1 ml of 25% acetonitrile. This stock solution was further diluted with 25% acetonitrile to prepare standards at concentrations of 100, 50, 25, 12.5, and 6.25 μg/mL. A linear standard curve of peak area versus concentration was used to determine the concentration of unknown samples. The concentration values on the curve were adjusted to 5.84, 11.68, 23.36, 46.72 and 93.45 μg/ml to account for the difference in molecular weight between dantrolenic acid (314 g/mol) and dantrolene sodium (336 g/mol). mole).
Определение эффективного уровня pHExample 11 Determination of effective pH level
Прибор был откалиброван в течение одного дня измерения эффективного уровня pH с использованием фосфатных буферов при pH 4, 7 и 10 (набор Hydrion Tri-Chek Buffer Capsule).Effective pH measurements were taken at room temperature using a Thermo Scientific Orion Star A111 benchtop pH meter. The instrument was calibrated over one day of effective pH measurements using phosphate buffers at pH 4, 7 and 10 (Hydrion Tri-Chek Buffer Capsule kit).
Samples were prepared by diluting the non-aqueous/anhydrous sample 10-fold with water. The diluted sample was mixed with vortex for approximately 10-15 seconds. Effective pH was measured using a pre-calibrated benchtop pH meter immediately after vortex mixing.
After each effective pH measurement, the pH vessel was rinsed with a suitable wash solution to remove all analytical sample from the flask.
