Abstract
Description
The present invention relates to surgery, oncology and is intended for conduction analgesia/anesthesia during operations on the anterior surface of the neck. Inadequate pain management has a significant impact on the survival of patients in the postoperative period (Rodgers A., Walker N., Schug S. Reduction of postoperative mortality and morbidity with epidural or spinal anesthesia: results from overview of randomized trials. BMJ, 2000; 321: 1-12). High-quality pain relief helps to minimize cardiovascular and thrombotic complications, faster activation and rehabilitation of the patient, minimizes the chronicity of pain, reducing the likelihood of neuropathic pain.
One of the most important tasks of analgesia in the postoperative period is to interrupt the afferent flow of nociceptive stimuli from peripheral pain receptors in organs and tissues to the segmental structures of the central nervous system. This task can be successfully solved using various methods of conduction analgesia/anesthesia. During operations on the anterior surface of the neck, they can also be used as wound pain relief in the intra- and postoperative period.
There is a known method of blocking the cervical plexus according to Winnie (V.M. Timerbulatov, F.S. Taneyev, M.M. Mehdiev, M.A. Sadridinov. Regional anesthesia and novocaine blockades in surgery. Ufa. 1993 - p. 18- 19), in which 10-15 ml of a 1.5-2% lidocaine solution is injected onto the lateral surface of the transverse process of the IV cervical vertebra (150-300 mg lidocaine) on one side. The disadvantage of this method is the large dose of the administered anesthetic, as well as the impossibility of using it on both sides, the lack of targeted administration of the anesthetic, a large volume of anesthetics used is required, which leads to the inclusion in the blockade of anatomical formations unnecessary for its implementation, such as the vagus nerve, stellate ganglion, carotid vascular -nerve bundle, inferior laryngeal nerve, recurrent laryngeal nerve, which can lead to unintended reactions - from toxic to autonomic disorders.
There is a known method of combined conduction anesthesia during operations on the thyroid gland, which includes determining the anatomical landmarks of the cervical plexus using an electromyostimulator during blockade of the cervical plexus, while the patient is in a supine position with his head turned in the opposite direction, injecting a 2% solution of lidocaine, 5 ml each sides to the lateral surfaces of the transverse processes of the IV cervical vertebra (total dose of lidocaine 100 mg). An electrically insulated needle is inserted at the intersection of lines running along the posterior edge of the sternocleidomastoid muscle and another parallel to the cervical fold, drawn through the middle of the cricoid cartilage, where the transverse process of the IV cervical vertebra is palpated. The correct location of the needle cut is determined by the contraction of the trapezius muscle when pulses are supplied to the needle from an electrical generator (single-phase rectangular pulses with a frequency of 60 per minute and a current strength of 6 mA), the passive electrode being on the opposite arm. After administration of 5 ml of a 2% lidocaine solution, the contractions fade and stop. Subsequently, conduction anesthesia of the cervical plexus is performed on the other side. A total of 10 ml of a 2% lidocaine solution (200 mg) is injected on both sides (patent RU 2177740, 01/10/2002). However, anesthesia of the deep branches of the cervical plexus, even in the presence of myostimulation, in the absence of visual control, may carry the risks of intravascular injection of anesthetic, for example, into the vertebral artery and into the epidural and subarachnoid space, and may also lead to the inclusion of unnecessary substances in the blockade. its implementation of anatomical formations, such as the vagus nerve, stellate ganglion,
Ф. А. В. - 2008. - №2. - С. 39-44. - EDN JKGBIF). There is a known method of interscalene blockade of the brachial plexus, in which two points are marked: the first - in the middle of the sternocleidomastoid muscle along its posterior edge (projection of the exit of the branches of the superficial cervical plexus from the superficial muscle of the neck), the second point - 1 cm medial to the middle of the clavicle ( the most likely place of projection of the brachial plexus onto the clavicle). The line connecting these points corresponds to the location of the trunks of the brachial plexus. The first point is the location of the needle, and the line connecting both points is the direction for its advancement. Infiltration anesthesia of the skin and subcutaneous tissue is carried out with 5-7 ml of anesthetic solution. In this case, there is a blockade of the branches of the superficial cervical plexus, which innervates the skin in the neck from the lower jaw to the subclavian and deltoid region. Then the needle is advanced in the direction of the second marked point at an angle of 30-40° to the skin surface to a depth of 2-3 cm until signs of puncture of the fascial sheath of the brachial plexus appear (Safronov, N.F. New method of interscalene blockade of the brachial plexus / N.F. Safronov , S.A. Kravtsov, S.V. Vlasov // Polytrauma. - 2008. - No. 2. - P. 39-44. - EDN JKGBIF). The disadvantages of this method are that the terminal branches are blocked through infiltration of the skin and subcutaneous tissue, which provides limited anesthesia - only in the area of injection of the anesthetic, if it is necessary to anesthetize a large surface, it is necessary to inject a large volume of solution, which can lead to a toxic dose, especially if do this on both sides.
- 2019. - Т. 16, No 1. - С. 35-41. DOI: 10.21292/2078-5658-2019-16-1-35-41).The closest analogue of the proposed invention is a method for the same purpose, in which, in the position of extension of the cervical spine with the head turned to the contralateral side from the place of manipulation under ultrasound control, focusing on the bifurcation of the carotid artery Hm.sternocleidomastoideus, an anesthetic is injected paravasally around the native fascia of the neck and as close as possible to the carotid artery, providing anesthesia to the nerves passing through the carotid fascia. In this case, a large volume of anesthetic is used so that it spreads along the entire neurovascular carotid bundle - at least 15-20 ml of anesthetic solution - 1-2% solution. For anesthesia of the sensitive branches of the cervical plexus, additional infiltration anesthesia of the sensitive branches of the cervical plexus is required - subplatysmal and subcutaneous injection of anesthetic, which requires large volumes and can also lead to a toxic reaction, intravascular entry. In addition, subplatysmal and subcutaneous infiltration eliminates the possibility of administering anesthetics bilaterally. This method does not provide analgesia over the entire anterior surface of the neck, but only together with the introduction of an anesthetic (Kraynik V.M., Novikov D.I., Zaitsev A.Yu., Kozlov S.P., Deshko Yu.V., Gavrilenko A. O.V., Kuklin A.V. Experience in the clinical use of ultrasound navigation to perform cervical plexus blockade in reconstructive surgery of the carotid arteries // Bulletin of Anesthesiology and Reanimatology. - 2019. - T. 16, No. 1. - P. 35-41. Disclosure of the Invention
The objective of the invention is to improve the approach to analgesic therapy during the intra- and postoperative period during operations on the anterior surface of the neck.
The technical result of the proposed method is to increase the effectiveness of analgesia over the entire anterior surface of the neck, regardless of the volume of surgery, with a decrease in the toxic effect of anesthetics and the risk of intravascular administration, blockade of the sympathetic trunk and vagus nerve.
The technical result is achieved due to anesthesia/analgesia of the entire surface of the neck, reducing the volume of the analgesic mixture and blocking the sensitive branches without additional infiltration of the subcutaneous tissue and skin.
The use of certain ultrasound control techniques allows for the most complete visualization of the structures of the anterior surface of the neck to select an adequate site for administering anesthetics. Performing sequential anesthesia on each side of the neck provides anesthesia/analgesia for the entire anterior surface of the neck, since part of the nerve branches innervating the skin exit from the right, and part of the branches from the left cervical plexus. It is not possible to achieve complete anesthesia/analgesia of the entire anterior surface of the neck with unilateral anesthesia.
Unlike its closest analogue, in which the bifurcation of the common carotid artery is just a landmark for the exit of the branches of the cervical plexus, which at approximately this place emerge from the deep layers (from the spine) and are divided into sensitive branches that go to the surface of the skin on each side, in In the proposed method, the introduction of an analgesic solution 1 cm below the bifurcation of the common carotid artery ensures a more uniform distribution of the anesthetic under the entire surface of the m.sternocleidomastoideus, which ensures the involvement of the maximum number of sensitive branches of the cervical plexus emerging from under this muscle and a wider area of skin anesthesia on each side.
The introduction of a small volume but high concentration of an analgesic mixture - Ropivacaine 0.75% or 1% in a volume of 5 ml, Lidocaine 10% in a volume of 1.0 ml and Adrenaline 1:1000 - allows you to obtain a full analgesic/anesthetic effect with a smaller volume of the administered mixture .
Unlike the closest analogue, which is applicable only for operations in the area of the carotid neurovascular bundle, the proposed method, in addition to anesthesia of the carotid bundle area, can be used for operations on the thyroid gland, during cervical lymph node dissection in combination with other types of plexus anesthesia. The method is carried out as follows.
1:1000.Anesthesia is performed under ultrasound control in the position of cervical extension with the head turned to the contralateral side from the place of manipulation. An ultrasonic linear sensor is placed in a horizontal plane, the location of the carotid artery, its bifurcation, m.sternocleidomastoideus, and the deep plate of the fascia propria of the neck is determined. Sequentially, on each side of the neck, 1 cm below the level of the bifurcation of the carotid artery at the outer edge of the m.sternocleidomastoideus, a needle is inserted and placed in the linear plane of the sensor. The end of the needle is visualized in the interfascial space under the m.sternocleidomastoideus in the middle of the distance between the carotid artery and the lateral edge of the m.sternocleidomastoideus. A mixture of 5.0 ml of 0.75% or 1% Ropivacaine and 1 ml of 10% Lidocaine with the addition of Adrenaline 1:1000 is injected .

A total of 10 operations were performed using bilateral blockade of the superficial branches of the cervical plexus in patients of various age categories. The results are characterized by a decrease in total doses of fentanyl, no reaction to pain intraoperatively at the time of incision, a decrease in inhalation consumption of Sevoran, stable smooth hemodynamics, quick awakening and a calm postoperative period, no complaints of pain in the postoperative period (VAS -0-1), rapid activation and rehabilitation of the patient, absence of complications from the postoperative wound.
Thus, the proposed method makes it possible to achieve anesthesia/analgesia over the entire anterior surface of the neck, regardless of the volume of surgical intervention, including in the postoperative period, to reduce the risk of the toxic effect of anesthetics and intravascular administration, blockade of the sympathetic trunk and vagus nerve.
