«Золотым стандартом» лечения уротелиального рака верхних мочевых путей является выполнение нефроуретерэктомии открытым или лапароскопическим доступом. При наличии единственной или единственно-функционирующей почки данная операция приводит к переводу больного в ренопривное состояние и проведению пожизненного программного гемодиализа.The "gold standard" for the treatment of urothelial cancer of the upper urinary tract is open or laparoscopic nephroureterectomy. Известен способ лечения рака мочевого пузыря, при котором производится удаление опухоли мочевого пузыря единым блоком. Операция проводится в литотомическом положении больного обычно под внутривенным наркозом с применением миорелаксантов короткого действия (при работе на боковых стенках МП). Первоначально с помощью фиброцистоскопа или ригидного эндоскопа тщательно по секторам осматривали полость МП с применением фотодинамической (PDD), узкоспектровой (NBI) или компьютерной виртуальной хромоэндоскопии (SPIES). После обнаружения опухоли, отступив от ее основания на 0,5-1 см, с помощью монополярного (биполярного) крючковидного электрода или лазерного волновода с торцевым прижиганием вокруг опухоли рассекали слизистую оболочку МП, подслизистый и мышечный слои с одновременной коагуляцией кровоточащих сосудов. Опухоль резецировали по кругу вместе с основанием и единым блоком отделяли ее от стенки МП. На начальном этапе водоструйной резекции с помощью точечной монополярной коагуляции специальным гибким полым электродом (гибридным ножом) вокруг основания опухоли намечали границы резекции. Затем через тот же электрод под основание опухоли нагнетали жидкость, приподнимающую ее над слизистой оболочкой МП, и тем же инструментом резецировали ее вместе с основанием. Далее опухоль единым блоком удаляли через тубус инструмента наружу путем активной аспирации или с помощью различных захватов (корзинки, лито экстракторы и др.). Использование предложенного способа в клинике позволяет избежать органоуносящее лечение (нефроуретерэктомию) и выполнить программу гемодиализа, получить гистологический материал лучшего качества, минимизировать объем интраоперационной кровопотери, уменьшить время проведения операции, увеличить абластичность и тем самым уменьшить сроки госпитализации и затраты на лечение.The use of the proposed method in the clinic allows avoiding organ-removing treatment (nephroureterectomy) and performing a hemodialysis program, obtaining better quality histological material, minimizing the volume of intraoperative blood loss, reducing the time of the operation, increasing ablasticity and thereby reducing hospital stay and treatment costs.
