Abstract
Description
The invention relates to medicine, namely to dentistry, and is intended for use in the individualization of structures made on a 3D printer or CAD / CAM system, acrylic prostheses, metal frames of removable and fixed dentures by layer-by-layer application of material in the form of a palette of pasty paints.
Within the framework of this application, the term "paste paint palette" means a set of paste paints of various colors from the claimed polymer composite material for layer-by-layer application on structures made on a 3D printer or CAD / CAM system, acrylic prostheses, metal frames of removable and fixed dentures, in order to achieve natural color reproduction of the set of teeth, detailed texture of the gingival part of the prosthesis with imitation of the capillaries of the gingival tissues, ensuring the achievement of shades that best match the color of the tissues in the oral cavity.
It is known that healthy gums are “coral-pink” in color, however, depending on the age category, gender and ethnicity of a person, the color of the gum tissue differs significantly from the coral-pink color. In addition, the color of the gums depends on the thickness of the epithelium, the degree of keratinization, and the amount of pigmentation. Accurate color matching is of paramount importance when providing aesthetic dental services to patients. While advances in “white aesthetics” for teeth are evident, the development of gingival “pink aesthetics” has lagged behind. Understanding the aesthetics of pink is critical because it allows clinicians to color-match the gingiva of dentures with soft tissues.
Currently, a highly aesthetic result of dental treatment is becoming more and more in demand both in the restoration of lost hard tooth tissues and adjacent soft tissues, and in the manufacture of full or partial dentures, especially in clinical cases associated with the installation of implants. In the modern world, aesthetics is one of the most important success factors in creating an orthopedic structure - a complete or partial removable denture. At the moment, among all the complaints of patients, aesthetics comes to the fore. That is, a prosthesis that is comfortable, but does not have aesthetic properties, is not recognized by patients as successful.
US Patent US4521193 (published 1985-06-04) discloses a method and kit for making dentures. A kit for making a temporary denture contains a molding composition adapted for making a soft molding material for forming an impression of a permanent denture; a shell, generally in the form of a dental prosthesis, adapted to contain the material of the molding composition; fast curing polymerizable acrylic; tooth color powder for adding to a portion of said polymerizable acrylic for coloring said portion of polymerizable acrylic; a pink powder for adding to another part of the polymerizable acrylic for coloring another part of the polymerizable acrylic; and a soft denture liner for filling the interior of an acrylic temporary denture shell made from said various pieces of colored polymerizable acrylic to adapt the temporary denture shell in the patient's mouth to be worn by the wearer. The disadvantage is that the method is used only for acrylic prostheses and the use of the described set cannot approximate the restoration in exact accordance with the surrounding gum tissue and dentition, since only one pink color is used to simulate the gums.
The long and unpleasant process of prosthetics when correcting dentition defects has changed dramatically due to the development of computer technologies associated with the advent of CAD/CAM and 3D printing. Products from thermoplastics made in the process of orthopedic dental treatment are, first of all, aesthetics, strength, lightness and functionality. They are devoid of the shortcomings of materials of previous generations. However, they do not convey the whole individuality, that is, they are not very aesthetic. Therefore, dentistry needs materials that can be used to make dentures that are not only light and durable, but also highly aesthetic. Prostheses with a detailed texture and imitation of the capillaries of the gum tissue, shades that best match the color of the tissues of the oral cavity. That is, the highly aesthetic result of dental treatment is not only the restoration of lost hard tissues of the teeth, but also the imitation of the soft tissues adjacent to them. In a number of clinical cases, especially during treatment with the installation of implants, it is often necessary to imitate healthy tissues of the oral mucosa. But it is not always possible to eliminate gum deficiency with the help of regenerative intervention, especially when defects in the alveolar ridge occur in the area of implants. Therefore, to accurately simulate the appearance of soft tissues, it is promising to use polymer composite masses of various colors and consistency.
There is a patent of the Russian Federation No. 2733161 (Published: 09/29/2020), which proposes a method for manufacturing a removable denture using a 3D printer, for which a polymer is poured into the printer for the manufacture of artificial tooth crowns, corresponding to the color of the teeth agreed with the patient, and printing is started, stop printing forcibly when a part of the prosthesis is made containing teeth and reduced gingival papillae to the cervical line, the printer changes the polymer to pink in color corresponding to the color of the gums, changes the printing settings according to the polymer and starts printing to completion. The removable denture obtained after printing is treated in ethyl alcohol with ultrasonic activation and illuminated in a photopolymerization oven, then the reduced papillae are restored and individualized with light-curing pink plastic, the denture is polished and fixed to the patient. The invention provides for the manufacture of a monolithic multi-colored complete removable denture by 3D printing.

Where oligomeric binder is a homogeneous mass containing (wt.%):
The compound of the formula [RR'SiAlk2OSiO1.5] n is a product of hydrolysis and copolycondensation of γ-methacryloxypropyltrimethoxysilane and phenyltrimethoxysilane, a 1:1 molar ratio refers to polymethacrylphenyloxypropylsesquioxanes with methacrylate groups in the organic radical at the silicon atom. As part of the oligomeric binder in the amount of 5% wt. acts as a highly reactive crosslinking agent, viscosity regulator, dispersant and, at the same time, a nanofiller.
Additional technical result: the combination of the photoinitiating system with polymethacrylphenyloxypropylsesquioxane during the final polymerization of the product prevents the formation of an oxygen-inhibited sticky layer, which leads to the formation of a glass-smooth surface of the polymer composite material applied to the orthodontic product and helps to slow down the growth of pathogenic microflora when wearing an orthodontic product.
The ratio of the mixture of bulk components and the solution of the oligomeric binder in ethanol is 44.5 and 55.5 wt. % respectively. The resulting suspension thermostatically at 70°C with constant stirring at a stirrer speed of 50 rpm for 4 hours.
At the end of the process, the resulting mixture is unloaded in a thin layer of no more than 1 cm into enameled trays and dried in a vacuum oven at a temperature of 60 ° C for 2 hours, periodically stirring the contents of the tray 4-6 times with a spatula to avoid the formation of agglomerates. Then raise the temperature to 120°C and maintain the filler for one hour at a pressure of ~ 0.1 atm. Having turned off the cabinet, the powder is allowed to cool inertially to a temperature of 40 ° C, and sieved through a sieve with a nylon mesh, with a mesh size of 1 mm.
To obtain a homogeneous pasty transparent paint, the oligomeric binder and filler coated with a layer of oligomeric binder are mixed for 2 hours until the bulk components are evenly distributed in the binder, after which they are passed twice through a three-roll mixer.
The task is also solved with a palette of pasty paints based on the above pasty transparent paint, to which pre-mixed required colors and, if necessary, titanium dioxide are added, mixed until a homogeneous mass is formed for one hour, and additionally passed twice through a three-roll mixer.

The surface of the acrylic prosthesis was sandblasted, degreased and dried. A thin layer of primer-adhesive on acrylic was applied to the prepared surface, kept in air for 180 seconds, the excess was removed and photopolymerization was carried out for 30 seconds. A gingival coating of the desired shade was applied using a dental brush, photopolymerization was carried out for 180 seconds. They gave individual characteristics to the gum coating: they reproduced the effect of melanin pigment and the transparency of the surface layer, using red, purple, white and transparent paste-like paints using the layer-by-layer application method. Each layer was polymerized for 180 seconds with a layer thickness of not more than 0.1 mm. To smooth the surface, the prosthesis was coated with transparent paint and polymerized for 30 seconds. We were convinced that the desired result was achieved by comparing with the natural soft tissues of the patient. The final polymerization was carried out for 360 seconds, and the prosthesis was polished in the traditional way. The intended gingiva simulation effect was achieved (FIG. 1).

After setting the dentition, a vestibular silicone key was made with garniture teeth on wax, according to occlusion, without modeling the gingival part, for subsequent positioning of the teeth relative to the model. After evaporation of the wax, internal cavities were created in the teeth to fix the artificial teeth in the photopolymer material. The surface of the teeth in contact with the color-curing modeling polymer mass was treated with an acrylic primer-adhesive, kept in air for 180 seconds, for better bonding of the base material with acrylic teeth. The teeth were installed in a silicone key and positioned on the model. Polymerization was carried out using a photopolymerizer for 30 seconds. The prepared metal frame was treated with a metal primer, exposed to air for 120 seconds, and polymerized for 120 seconds. The treated surface was covered with two layers of opaque polymer mass, the thickness of each layer was not more than 0.1 mm. Each layer was polymerized for 180 seconds.
