Abstract
Description
Изобретение относится к области медицины и касается способа лечения пациентов детского возраста с синдромом дефицита внимания и гиперактивности (СДВГ).The invention relates to the field of medicine and relates to a method for treating pediatric patients with attention deficit hyperactivity disorder (ADHD).
Attention Deficit Hyperactivity Disorder (ADHD) is a common disorder, first manifesting in childhood, which is associated with the development of cognitive and functional deficits and comorbid disorders. Manifested by symptoms such as difficulty concentrating, hyperactivity and poorly controlled impulsivity. ADHD is a severe disorder that, if left untreated, can severely impair a person's functional abilities and health, and is often accompanied by comorbidities, mainly in the areas of behavior and mental health. According to some reports, it occurs in about 5 - 10% of school-age children.
Drug therapy is prescribed according to individual indications in cases where violations of behavior and cognitive functions cannot be overcome by non-drug methods. Children under 5 years of age should carefully assess the risk of using drugs to correct ADHD. Actual studies in a group of children over 5 years of age have proven the effectiveness of drug treatment, but there are concerns about the recommendation of drugs to correct ADHD symptoms, in particular uncertainty about the long-term side effects of drugs in growing children. Among the drugs, it is possible to prescribe antidepressants, psychostimulants. However, such an appointment requires special care.
In Russia, nootropic drugs are most often used in the medical treatment of attention deficit disorder in children.
Nootropic drugs are drugs that, as a result of stimulation of metabolic processes and interneuronal transmission in the central nervous system, improve mental activity, attention, memory, the ability to reproduce information, reduce the need for neurons in oxygen during hypoxia, and increase the resistance of the central nervous system to adverse factors.
175-180).Most often, of the nootropics, when prescribed to children, derivatives of gamma-aminobutyric acid are used (Biochemistry, pharmacology and clinical use of pantothenic acid derivatives (collection of scientific articles). - Grodno, 2003, pp. 175-180).
35-40).The nootropic preparation of the peptide structure Cortexin ® , which is a complex of polypeptides isolated from the cerebral cortex of calves and pigs not older than 12 months of age, has also received widespread use for the treatment of children with ADHD by the method of acetic acid extraction. "Cortexin®" showed an effect in the form of intramuscular injections at a dose of 10 mg during a treatment course of 10 injections (CHILD AND ADOLESCENT REHABILITATION No. 1 (22) 2014, pp. 35-40).
Однако убедительных доказательств (уровень доказательности А, В) эффективности такого лечения нет (Практика педиатра, №4, ноябрь-декабрь, 2019).In this area, attempts are being made to use some other drugs of this group, which are quite safe. However, there is no convincing evidence (level of evidence A, B) of the effectiveness of such treatment (Practice pediatrician, No. 4, November-December, 2019).
Thus, the study and search for new methods of its treatment remains relevant.
ADHD is classified as a multifactorial developmental disorder, which means that its treatment requires a multifaceted approach. In terms of such an effect, of interest is the well-known antioxidant drug Mexidol - 2-ethyl-6-methyl-3-hydroxypyridine succinate (ethylmethylhydroxypyridine succinate), synthesized at the Institute of Biochemical Physics of the Russian Academy of Sciences; studied and developed at the Research Institute of Pharmacology of the Russian Academy of Medical Sciences and the All-Russian Scientific Center for the safety of biologically active substances. The drug is an inhibitor of free radical processes, a membrane protector with antihypoxic, stress-protective, nootropic, anticonvulsant and anxiolytic effects. The nootropic properties of Mexidol are expressed in the ability to improve learning and memory, contribute to the preservation of a memorable trace and counteract the process of fading instilled skills and reflexes.
In particular, the use of 2-ethyl-6-methyl-3-hydroxypyridine succinate as a nootropic and tranquilizing agent is described in patent RU 2065299, 20.08.1996. It was shown that Mexidol, when taken orally for 60 days, had a distinct ability to improve the memory of animals, increasing the latent time of the reflex.
Also known from the prior art document EA 20800 B1, publ. 2015.01.30 concerning a pharmaceutical composition for the treatment of conditions associated with impaired cerebral circulation, cerebrovascular and neurodegenerative diseases. It is noted that the composition can be used in the prevention of memory impairment or to improve memory in young people. The composition can be administered orally at a dose of Mexidol 200 mg 2 times a day in the form of tablets or capsules before meals. The duration of therapy is 14 days.
However, these sources do not disclose the possibility of using the composition in children under 12 years of age with ADHD.
Despite the widespread use of Mexidol in clinical practice for various indications, there are very few works related to research in the field of its use in pediatrics. Moreover, the instructions for the registered drug (LSR-002063/07) include “children's age” as a contraindication.
As the closest analogue, the method disclosed in the article by I.S. Potapova et al., The course of school adaptation in children with attention deficit hyperactivity disorder while using Mexidol, Bulletin of Experimental Biology and Medicine, 2006, Appendix 1. This source describes studies of school adaptation in patients who received complex treatment using Mexidol and without. Children 7-9 years old were prescribed Mexidol daily, 1/2 tablet 2 times a day (daily dose - 0.125 g) for 30 days. It is noted that improvement in the condition during the adaptation period was achieved in 74% of children.
The objective of the invention is to develop a more effective regimen for treating pediatric patients with ADHD with Mexidol® .
In the course of the study, we have identified the possibility of increasing the effectiveness of ADHD treatment in patients aged 6 to 12 years inclusive with moderate severity of the disease without increasing the dosage of the drug, respectively, without increasing the risk of side effects, by changing the drug regimen.
(Clinical Global Impressions-ADHD-Severity - CGI-ADHD-S)).EFFECT: advantage - taking the drug in the afternoon (twice - at lunchtime and in the evening) has a greater effect in the population of patients with moderate severity of ADHD (4 points on the Scale for assessing the overall clinical impression of the severity of ADHD (Clinical Global Impressions-ADHD- Severity - CGI-ADHD-S)).
Ethylmethylhydroxypyridine succinate can be administered in a dosage form suitable for oral administration to children - powder, tablet, capsule, liquid form.
Thus, the problem is solved by a new method for the treatment of ADHD in patients aged 6 to 12 years inclusive with a moderate severity of the disease, involving oral administration of ethylmethylhydroxypyridine succinate at a dose of 125 mg 2 times a day (at lunchtime and in the evening) for 6-8 weeks .
The possibility of carrying out the invention can be demonstrated by the following examples.
Во всех примерах, описанных ниже, использованы данные субанализа групп и подгрупп, полученные преимущественно в рамках исследования «Многоцентровое двойное слепое, рандомизированное, плацебо-контролируемое в трех параллельных группах клиническое исследование по оценке эффективности и безопасности препарата Мексидол® таблетки, покрытые пленочной оболочкой, 125 мг (ООО «НПК «ФАРМАСОФТ», Россия) в лечении синдрома дефицита внимания с гиперактивностью (СДВГ) у детей 6-12 лет при различных режимах дозирования (МЕГА)».In all the examples described below, subgroup and subgroup sub-analysis data were used, obtained mainly in the framework of the study “Multicenter, double-blind, randomized, placebo-controlled in three parallel groups, a clinical study evaluating the efficacy and safety of the drug Mexidol ® film-coated tablets, 125 mg (NPK PHARMASOFT LLC, Russia) in the treatment of attention deficit hyperactivity disorder (ADHD) in children aged 6-12 years with various dosing regimens (MEGA).
Patients included in further analysis were divided into 3 groups according to the severity of ADHD symptoms:
(Clinical Global Impressions-ADHD-Severity - CGI-ADHD-S).1. Mild severity (Table 1) - 3 points on the Clinical Global Impressions-ADHD-Severity Scale (CGI-ADHD-S).
2. Moderate severity (Table 2) - 4 points on the Clinical Global Impressions-ADHD-Severity Scale (CGI-ADHD-S). Also, in this group of patients, the total score on the subscales "inattention", "hyperactivity / impulsivity" of the SNAP-IV scale ranged from 24 to 36 points.
(Clinical Global Impressions-ADHD-Severity - CGI-ADHD-S).3. Severe severity (Table 3) - 5 points on the Clinical Global Impressions-ADHD-Severity Scale (CGI-ADHD-S).
Дополнительно пациенты, включенные в каждую группу, были разделены на 2 подгруппы по режиму применения препарата (утро/обед и обед/вечер):Additionally, patients included in each group were divided into 2 subgroups according to the drug regimen (morning/lunch and lunch/evening):
Method of application: inside 1 tablet 2 times a day in the morning at 7.00 and at lunchtime at 13.00. Duration of therapy: 6 weeks.
Method of application: inside 1 tablet 2 times a day at lunchtime at 13.00 and in the evening at 19.00. Duration of therapy: 6 weeks.
Evaluation of treatment effectiveness is based on primary and secondary effectiveness criteria:
(Busner J, Targum SD. The Clinical Global Impressions Scale: Applying a Research Tool in Clinical Practice. Psychiatry (Edgmont). 2007; 4(7): 28-37; https://www.ncbi.nlm.nih.gov/pmc/articles/PMC2880930/).7. Clinical Global Impressions-ADHD-Severity (CGI-ADHD-S) score on the Clinical Global Impressions-ADHD-Severity scale after 6 weeks of therapy compared with baseline. The ADHD severity assessment scale (CGI-ADHD-s) provides for the doctor to determine the severity of the disease according to the following gradation, score: 1 - normal, healthy; 2 - minimum degree; 3 - mild (some deterioration in any one environment); 4 - moderate severity of the disease (some deterioration in at least two types of environment, significant difficulties in school and / or control of one's behavior); 5 - pronounced severity of the disease (deterioration in all types of environment, significant stress for others, social ties are broken); 6 severely ill (significantly worsening consistently in all environments, academic/social failure, requires daily supervision for all or most of the day); 7 - the most severe patients (Busner J, Targum SD. The Clinical Global Impressions Scale: Applying a Research Tool in Clinical Practice. Psychiatry (Edgmont). 2007; 4(7): 28-37; https://www.ncbi. nlm.nih.gov/pmc/articles/PMC2880930/).

(Clinical Global Impressions-ADHD-Severity - CGI-ADHD-S)).Thus, there were differences in the effectiveness of therapy with different regimens of taking the drug in a population of patients with moderate severity of ADHD (4 points on the Clinical Global Impressions-ADHD-Severity (CGI-ADHD-S) scale) .
The effectiveness of treatment was 50-100% higher in this population in children in Subgroup 2 compared with Subgroup 1 (comparison of changes in indicators on individual scales / subscales between subgroups). In addition, the improvement was achieved cumulatively in 86% of the children of Subgroup 2, which also demonstrates the advantage of the described method compared to the prototype. There were no side effects associated with taking the drug in all study groups.
Thus, the proposed treatment regimen with Mexidol ® (taking the drug 2 times a day at lunchtime and in the evening) is a new, more effective method of treating children with moderate ADHD, which can be introduced into pediatric practice.
