Известен способ хирургического устранения тяжелых контрактур лучезапястного сустава с помощью артродеза лучезапястного сустава (Van Heest А.Е., Strothman D. Wrist arthrodesis in cerebral palsy. J Hand Surg Am 2009 Sep; 34(7): 1216-24), при которых после удаления хряща с поверхностей формирующих сустав костей он фиксируется с помощью различных металлических конструкций, установленных на тыльной или ладонной поверхностях предплечья и кисти.A known method of surgical elimination of severe contractures of the wrist joint using arthrodesis of the wrist joint (Van Heest A.E., Strothman D. Способ коррекции сгибательной контрактуры лучезапястного сустава у пациентов с детским церебральным параличом, включает в себя хирургический доступ по локтевой поверхности руки к пятой пястной кости 1 (Поз. 1), к костям запястья (Поз. 2) и локтевой кости (Поз. 3) и фиксацию лучезапястного сустава с помощью металлоконструкции в виде пластины с угловой стабильностью (Поз. 5) с шестью винтами (Поз. 6), установленных в локтевую (Поз. 3), лучевую кость (Поз. 4), кости запястья (Поз. 2) и метадиафизарную область V, IV, III и II пястных костей (Поз. 1) для фиксации лучезапястного сустава в положении разгибания кисти 100°.A method for correcting flexion contracture of the wrist joint in patients with cerebral palsy includes surgical access along the ulnar surface of the hand to the fifth metacarpal bone 1 (Pos. 1), to the bones of the wrist (Pos. 2) and ulna (Pos. 3) and fixation of the wrist joint using a metal structure in the form of a plate with angular stability (Pos. 5) with six screws (Pos. 6) installed in the ulna (Pos. 3), radius (Pos. 4), carpal bones (Pos. 2) and the metadiaphyseal region of the V, IV, III and II metacarpal bones (Pos. 1) for fixing the wrist joint in the 100° wrist extension position.
Способ коррекции сгибательной контрактуры лучезапястного сустава у пациентов с детским церебральным параличом осуществляется следующим образом: производят разрез по локтевой поверхности предплечья и кисти от середины V-ой пястной кости до границы нижней и средней третей предплечья. Накостно устанавливают пластину с угловой стабильностью, которую фиксируют винтами, проводившимися через локтевую, лучевую кость, кости запястья и метадиафизарную область V, IV, III и II пястных костей. Кисть стабилизирут в среднем положении или в положении физиологического разгибания в лучезапястном суставе (100°). Предплечье фиксируют при среднем ротационном положении. Конечность иммобилизируют короткой гипсовой лонгетой или тутором на 2-3 дня до исчезновения болевого синдрома. The limb is immobilized with a short plaster splint or splint for 2-3 days until the pain syndrome disappears. In the future, orthotics are not applied.
Положительный эффект заключается в повышение функциональности верхней конечности путем временной фиксации лучезапястного сустава накостной пластиной. Такой вид хирургического лечения предусматривает отсутствие травматизации сухожильного аппарата и это позволяет проводить двигательную реабилитацию непосредственно сразу после оперативного лечения, а пациент не нуждается в послеоперационном ортезировании. Временное артродезирование лучезапястного сустава как способ коррекции контрактуры включает в себя следующие плюсы: стабильность артродеза при установленной металлоконструкции и возможности активных движений в лучезапястном суставе для получения максимальной функции кисти, после удаления конструкции. Кроме того, если после удаления металлоконструкции функциональные перспективы верхней конечности будут оцениваться более позитивно, такой подход сохраняет возможность выполнения мягкотканых операций вторым этапом.The positive effect is to increase the functionality of the upper limb by temporarily fixing the wrist joint with a bone plate.
