Известен способ диагностики когнитивной дисфункции у больных с сахарным диабетом (СД) 1-го типа, который включает в себя определение нейроспецифических белков в крови (глиальный фибриллярный кислый белок (GFAP), основной белок миелина (MBP) и S100) и проведение магнитно-резонансной томографии (МРТ) головного мозга в стандартных проекциях (аксиальной, сагиттальной и корональной) с использованием Т1 и Т2 взвешенных изображений. При этом в крови у пациентов обнаруживается повышенный уровень указанных белков: GFAP - 0,12±0,04 нг/мл, MBP - 0,13±0,05 нг/мл, S100 - 125,65±66,97, а по результатам МРТ ряд изменений: косвенные признаки атрофии серого вещества головного мозга (расширение ликворокистозных, конвекситальных и пространств Вирхова-Робина). Поставленная задача решена следующим образом: пациенту с СД 1-го и СД 2-го типа проводят бесконтрастную магнитно-резонансную перфузию с оценкой церебрального кровотока. Предлагаемый способ был апробирован у 116 пациентов с СД 1-го типа и 204 пациентов с СД 2-го типа, которые находятся на диспансерном наблюдении по СД. Всем пациентам была проведена бесконтрастная магнитно-резонансная перфузия и определен общий церебральный кровоток, который был значимо снижен у пациентов с СД 1-го и 2-го типа при верификации когнитивных нарушений (χ2 =6,7, p=0,009; χ2 =4,7, p=0,029). С помощью ROC анализа в исследовании провели оценку значимости церебрального кровотока в качестве диагностического метода когнитивных нарушений и выявили, что при СД 1-го типа чувствительность его определения составила 94,6%, специфичность - 72,3% (AUC 0,618±0,054). При определении ценности в диагностическом плане церебрального кровотока при СД 2-го типа выявили, что для диагностики когнитивных нарушений чувствительность была 96,2 %, специфичность - 75,8 % (AUC 0,680±0,073). При анализе показано, что прогноз верификации когнитивных нарушений у пациентов с СД 1-го и 2-го типа при церебральном кровотоке 56 мл/100 г/мин и менее при СД 1-го типа и 37,7 мл/100 г/мин и менее при СД 2-го типа.The proposed method was tested in 116 patients with type 1 diabetes and 204 patients with type 2 diabetes, who are undergoing dispensary observation for diabetes.
