Преждевременное удаление временных зубов в детском возрасте является одной из основных причин возникновения зубочелюстных аномалий и деформаций, которые в дальнейшем трудно подвергаются ортодонтической и ортопедической корректировке. Множественные дефекты зубных рядов ведут к структурным и функциональным изменениям в ЗЧС в целом. К сожалению, они развиваются за достаточно короткое время в связи с ростом ребенка (Фанакин В.А., Филимонова О.И., 2011). Сокращение протяженности зубного ряда, потеря его целостности приводит к формированию вторичных деформаций зубных рядов. Морфологические же деформации, в свою очередь, нарушают развитие и нормальную функциональную работу всей ЗЧС.Premature extraction of deciduous teeth in childhood is one of the main causes of dentoalveolar anomalies and deformities, which are then difficult to undergo orthodontic and orthopedic correction. Ряд отечественных и иностранных исследований посвящены обоснованию показаний к профилактическому протезированию у детей с преждевременным удалением временных зубов с целью профилактики формирования вторичных зубочелюстных деформаций и их осложнений. Нами же установлено, что важнейшим фактором в определении показаний к зубному протезированию у детей является восстановление функциональной составляющей ЗЧС.A number of domestic and foreign studies are devoted to substantiating indications for prophylactic prosthetics in children with premature extraction of deciduous teeth in order to prevent the formation of secondary dentoalveolar deformities and their complications. При любых видах зубопротезирования или ортодонтического лечения должно создаваться гармоничное взаимоотношение всех органов и структур полости рта для обеспечения оптимальной эстетики и максимальной эффективности функционирования жевательного аппарата. Окклюзионная гармония должна быть воссоздана как при центральном соотношении челюстей и центральной окклюзии, так и во всех функциональных эксцентрических положениях нижней челюсти. (С.А. За прототип изобретения взят метод морфометрии зубов и зубных рядов, предложенный Постниковым М.А., основанный на определении дефицита места в зубном ряду для постоянных зубов у детей с преждевременным удалением временных зубов (Ортодонтическое лечение пациентов с аномалиями окклюзии в сагиттальном направлении и частичной адентией: монография / М.А. Постников - Самара: ООО «Офорт»; ГБОУ ВПО «СамГМУ», 2015.- 120 с.). На гипсовых моделях челюстей определяли величину промежутка в зубных рядах вследствие раннего удаления временных зубов. Измерения проводили между апроксимальными поверхностями зубов, ограничивающих дефект. Далее определяли величину дефицита места в зубном ряду для постоянных зубов, которые при прорезывании бдут располагаться в зубном ряду в период постоянного прикуса. На основании полученных данных был сделан вывод о необходимости раннего профилактического протезирования детей с преждевременным удалением временных зубов, т.к. чем больше срок, прошедший с момента удаления зубов до начала лечения, тем дефицит места в зубном ряду больше.The prototype of the invention is based on the method of morphometry of teeth and dentition proposed by Postnikov M.A., based on determining the deficit of space in the dentition for permanent teeth in children with premature extraction of temporary teeth (Orthodontic treatment of patients with occlusion anomalies in the sagittal direction and partial adentia: monograph / MA Postnikov - Samara: OOO Etching; SBEE VPO SamGMU, 2015. - 120 p.). On the basis of the data obtained, it was concluded that early prophylactic prosthetics of children with premature extraction of deciduous teeth is necessary. the longer the period elapsed from the moment of tooth extraction to the start of treatment, the greater the deficit of space in the dentition is greater.

Морфологические изменения в свою очередь являются лишь следствием адаптации организма к неправильной функции. В результате анализа данных о комплексной оценке функционального состояния ЗЧС, определили, что активное ортодонтическое лечение в отношении детей с преждевременным удалением временных зубов должно быть, в первую очередь, направлено на нормализацию функции. Методом выбора способа протезирования явилось использование съемных протезов как самых оптимальных не только в плане замещения отсутствующих зубов и сохранения необходимого места в зубной дуге. Как известно, раздражающее действие такого протеза стимулирует рост челюсти и прорезывание постоянных зубов. Кроме того, при сочетании дефектов зубных рядов с зубочелюстно-лицевыми аномалиями базис съемного протеза можно использовать для укрепления различных ортодонтических приспособлений, исправляющих аномалии положения зубов, сужения зубных рядов, смещения нижней челюсти и т.д. Для того чтобы определить особенности движений нижней челюсти у детей до и после протезирования, было отобрано 10 пациентов от 6 до 10 лет с преждевременным удалением временных зубов. Исследование функционального состояния ЗЧС у детей сразу же после протезировании дало следующие результаты: стабилизация диагонали в графике «Miocentrica» (с 1,42±1,06 мм до 1.83±0.47 мм при N=1,81±0,18 мм) за счет полного восстановления окклюзионных контактов и полной догрузки жевательных мышц и снижение биопотенциалов височных мышц; увеличение амплитуды латеральных движений нижней челюсти вправо (с 8,59±2,8 мм до 8.63±2,6 мм при N=9,14±0,4 мм); улучшение движения нижней челюсти вперед (с 5.88±1,83 мм до 6,28±2,29 мм при N=9,77±0,53 мм); увеличение амплитуды открывания рта (с 31,80±14,4 до 32,98±32,2); уменьшение значений дефлексии (с 4,48±15,5 до 2,66±3,7); увеличение скорости поднимания нижней челюсти (с 242,14±114,6 мм/с до 272,14±114,8 мм/с при N=282,5±20,21 мм/с). Снижение скорости опускания нижней челюсти и сохранение нарушенной функции глотания говорит о том, что пациентам еще только предстоит привыкнуть к протезу. Однако функциональные изменения очевидны непосредственно после протезирования. A decrease in the rate of lowering of the lower jaw and the persistence of impaired swallowing function suggests that patients have yet to get used to the prosthesis. However, functional changes are evident immediately after prosthetics. Further, on the basis of the data obtained, the prosthesis was corrected and the study on the kinesiograph was repeated again.
Таким образом, при обследовании пациентов с преждевременным удалением временных зубов необходимо использовать современные методы функциональной диагностики - компьютеризированные кинезиографы, которые наиболее объективно отражают степень нарушения деятельности всей ЗЧС. Далее для уточнения предварительного диагноза, составления рационального плана и прогноза лечения пациентке провели дополнительные методы исследования: рентгенологическое обследования (ОПТГ, ТРГ головы в боковой проекции с расшифровкой в программе Dolphin Imaging) и функциональную диагностику (кинезиографию нижней челюсти).Further, to clarify the preliminary diagnosis, drawing up a rational plan and prognosis of treatment, the patient underwent additional research methods: X-ray examination (OPTG, TRG of the head in lateral projection with decoding in Dolphin Imaging software) and functional diagnostics (kinesiography of the lower jaw).

Анализ телерентгенограммы головы в боковой проекции проводился в программном продукте Dolphin Imaging, США (модуль Ceph Tracing) по Панкратовой Н.В. по 37 приведенным параметрам. С использованием аппарата «Кинезиограф» (компания «Биокет», Италия), оснащенного компьютеризированной программой KEY-WIN, проведена кинезиография нижней челюсти с использованием различных проб в трех плоскостях - сагиттальной, фронтальной, горизонтальной. В результате выявлено неоднородное движение нижней челюсти при опускании и поднимании, дефлексия нижней челюсти вправо на 10,12 мм, снижение показателей скорости опускания и поднимания нижней челюсти, нарушение отношения графиков «Глотания» и «Миоцентрика», изменение функции глотания, смещение графика «Миоцентрика» влево, уменьшение его амплитуды. Изменение окклюзионных взаимоотношений верхней и нижней челюстей, уменьшение амплитуд графиков «Протрузия» и «Латеральные движения нижней челюсти» Графики голубого цвета показывают нормальные показатели движений нижней челюсти (фиг. 5, 6).Using the apparatus "Kinesiograph" (company "Bioket", Italy) equipped with a computerized program KEY-WIN, kinesiography of the lower jaw was carried out using various samples in three planes - sagittal, frontal, horizontal. Дефлексия нижней челюсти вправо. Изменение окклюзионных взаимоотношений верхней и нижней челюстей. На основании полученных данных пациентке П. было предложено и проведено ортодонтическое лечение, которое включало изготовление аппарата съемного механического действия на верхнюю челюсть и частичного съемного протеза на нижнюю челюсть (фиг. 7).Based on the data obtained, the patient P. was offered and carried out orthodontic treatment, which included the manufacture of a removable mechanical device for the upper jaw and a partial removable denture for the lower jaw (Fig. 7).

Нарушение соотношения графиков «Глотания» и «Миоцентрика», изменение функции глотания сохранено из-за отсутствия еще адаптации к протезу. Смещение графика «Миоцентрика» влево уменьшилось, увеличение его амплитуды. Увеличение амплитуд графиков «Протрузия» и «Латеральные движения нижней челюсти». Графики голубого цвета показывают нормальные показатели движений нижней челюсти (фиг. 8).After that, a re-recording of the lower jaw kinesiography was carried out. Через 1 месяц Пациентке П. была проведена повторная кинезиография нижней челюсти. В результате выявлено более однородное движение нижней челюсти при опускании и поднимании. Дефлексия нижней челюсти незначительна, амплитуда при открывания рта увеличилась и соответствует норме. Смещение графика «Мисцентрика» влево незначительное. Изменение окклюзионньтх взаимоотношений верхней и нижней челюстей, графики «Протрузия» и «Латеральные движения нижней челюсти» стали более однородные и близкие к норме. Графики зеленого цвета показывают нормальные показатели движений нижней челюсти (фиг. 9). Нарушение соотношения графиков «Глотания» и «Миоцентрика» сохранилось, дополнительно был назначен комплекс миогимнастических упражнений (дыхательные упражнения, упражнения для языка и губ) в течение 1 месяца для нормализации Функции глотания. Спустя месяц проведено повторное функциональное исследование, которое показало положительную динамику. Для закрепления результата рекомендовано продолжить выполнение комплекса миогимнастических упражнений в течение полугода.After 1 month, Patient P. underwent repeated kinesiography of the lower jaw. A month later, a repeated functional study was carried out, which showed positive dynamics. To consolidate the result, it is recommended to continue performing the complex of myogymnastic exercises for six months.
