Для визуализации индивидуальных путей лимфооттока используют отечественный радиоколлоидный препарат 99mTc - технефит. Под визуальным контролем в 2 точки, расположенные по краям опухолевого поражения шейки матки, выполняют инъекцию РФП. Суммарная введенная активность составляет 74-200 МБк в объеме 0,2-0,4 мл. Радионуклидную визуализацию сигнальных лимфатических узлов проводят на аппарате ОФЭКТ-КТ «Symbia Т16» (Siemens) с использованием низкоэнергетического коллиматора высокого разрешения (LEHR).To visualize the individual pathways of the lymphatic drainage, the domestic 99mTc radiocolloid preparation, technefite, is used. Исследование осуществляют в положении пациентки на спине. Сбор информации начинают через полтора часа после инъекции 99mTc-коллоида. Верхняя граница поля сканирования располагается на уровне L2-L3 позвонков, нижняя - на уровне тазобедренных суставов. На первом этапе проводится двухдетекторная однофотонно-эмиссионная компьютерная томография (ОФЭКТ) с использованием следующих параметров: матрица 128×128, шаг сканирования - 3 градуса, время экспозиции кадра 19 секунд, угол ротации для каждого детектора 180 градусов. После завершения ОФЭКТ, не изменяя положения пациента на столе, выполняют спиральную компьютерную томографию - напряжение трубки 120Кв, сила тока 80-100 mAs, время оборота трубки 0,5 сек, время сканирования 15 секунд, шаг стола 1 мм, толщина среза 5 мм с последующей реконструкцией по 1,5 мм.The study is carried out in the patient's position on the back. Визуализировано несколько сигнальных лимфоузлов и лимфоузлов второго порядка, которые были локализованы в тазовой и парааортальной областях (фиг. 1А). В соответствии с результатами ОФЭКТ-КТ визуализации лимфооттока от опухоли шейки матки сформированы индивидуальные радиационные поля для облучения регионарных лимфоузлов, в том числе, лимфоузлов парааортальной области (фиг. 2).Example 1. В соответствии с полученными данными об отсутствии лимфооттока от опухоли в парааортальные и пресакральные лимфоузлы сформированы индивидуальные радиационные поля без включения в облучаемый объем пресакральной и парааортальной областей (фиг. 4).Example 2.
