Abstract
Description
A method for treating burn patients with deep and / or extensive burns, in which both dressings soaked in water-soluble ointments and / or wound dressings and elastic electric heaters are simultaneously used.
A known method of treating burns using ointments with a wide range of therapeutic effects (RF Patent No. RU 2003134532, 05.20.05, A61K 35/64, A61K 47/44, A61K 9/06). In this method, the ointment is applied with a thin layer of 0.5 ÷ 3 mm on a dense cotton cloth, followed by application of the dressing on the wound surface and dressing (dressing replacement) after 1-3 days for 3-14 days.
The choice of the thickness of the ointment layer applied to the tissue (when using a bandage, the effectiveness of the ointment decreases sharply), due to its greatest effectiveness: when the thickness of the ointment is less than 0.5 mm and the thickness of the ointment is more than 3 mm, the effectiveness of the ointment decreases.
The temperature of the "core" of the human body is 1 ÷ 1.5 ° C higher than the temperature on the surface of the intact skin. By the temperature of the "core" of the body we mean the relatively constant temperature of internal organs and tissues: liver, brain, stomach, lungs, proximal rectum.
In patients with deep and extensive thermal lesions, due to a violation of the integrity of the skin, the “core” of the body comes into contact almost directly with the external environment. This leads to increased heat loss. As a result, unintentional hypothermia can result: - arrhythmia, decreased immunity, hypokalemia, impaired acid-base state, coagulopathy (platelet dysfunction), postoperative stress reaction and protein catabolism, a change in mental status, impaired renal function, a change in drug metabolism , the development of chills and muscle tremors, especially in the postoperative period.
Since the application of dressings soaked in water-soluble ointments and / or wound dressings to burn and donor wounds increases heat loss, as a result, the use of water-soluble ointments applied to tissue dressings for topical treatment of burn wounds not only affects the wound process, but also and to the level of thermal balance, increases heat loss, promotes the development of hypothermia.
In order to eliminate the manifestation of hypothermia in the treatment of burn patients, methods are used to compensate for heat loss using fluidizing beds and heat guns.
It is well known that fluidizing beds are used not only to reduce pressure on burned areas of the body and to ensure the drying of the scab, but also to combat hypothermia. However, these devices cannot be used in vehicles, in particular during the evacuation of burn patients, in addition, the warm air currents generated by these devices contribute to the evaporation of moisture from ointments, which reduces the effectiveness of this treatment method.
The aim of the invention is to develop a method that allows you to achieve the state of normothermia in the contact mode of heating patients with deep and / or extensive burns, when using medical dressings, including soaked in water-soluble ointments, and / or wound dressings and at the same time extend the therapeutic effect of the dressings and / or wound dressings.
The technical solution to the problem is a method of compensating for heat loss in burn patients, including applying dressings to burn wounds, including soaked in water-soluble ointments, and / or wound dressings. In this method, at least one flexible electric heater is placed on the surface of the body both on the unaffected areas and / or on the surfaces of the burned areas of the body over the dressings soaked in water-soluble ointments and / or wound dressings, and the person’s body temperature is simultaneously heated, controlled and automatic and / or manual control of the heating temperature of each elastic electric heater in a predetermined temperature range. In this case, a multi-aspect control of the human body temperature is carried out, measured in various ways using sensors: either rectally, and / or orally, and / or in the bladder, and / or in the pulmonary artery, etc., on the surface of the body and on the surface of an elastic electric heater , - as well as using the mechanism of both manual and automatic temperature control, taking into account the influence of evaporation of moisture contained in water-soluble ointments and / or wound coverings.
As a flexible electric heater can be used as a woven and / or knitted and / or film electric heater. The measurement of human body temperature is carried out in various ways: rectally and / or orally, and / or using a temperature sensor inserted through a catheter into the bladder, and / or through a catheter inserted through a catheter into the pulmonary artery.
It should be emphasized that flexible electric heaters provide heating by contact, rather than by air flow, as is the case with fluidizing beds or heat guns, which significantly reduces the degree of drying of water-soluble ointments and preserves their healing properties for longer.
Flexible electric heaters can provide local heating, which allows you to manipulate their location when compensating for heat loss in a patient with thermal lesions. Such heating, if possible, is even preferable to carry out on the surface of the patient’s body, which is outside of thermal lesions, but in close proximity to them. Further heat transfer is provided by the circulatory system. With this heating, the thermal effect on the medical dressings is also reduced, which increases the time for their effective use, since the evaporation of moisture in water-soluble ointments decreases.
The use of two or more flexible electric heaters for heating allows, if necessary, to provide different modes of operation of these electric heaters in order to better adapt the generated heat flux in different parts of the body to compensate for heat loss, which allows flexible thermal effects on the burn patient.
The prerequisites for the technical solution of the problem - compensation of conditions conducive to the occurrence of trembling and hypothermia, is the use of flexible electric heaters made on the basis of modern technologies, primarily woven ones, which allow contact heating and reduce unproductive energy costs for its implementation. Such heaters also provide a high degree of electrical safety, as they operate from ultra-low voltage ~ 6 ÷ 24 V.
1, 2:The effectiveness of the proposed method, the reduction of the manifestations of hypothermia in the treatment of patients with extensive burns, is confirmed by the results obtained during the autologous graft transplant operations shown in FIG. 12:

2 - изменение температуры во времени у ожогового больного при включенном тканом электронагревателе.- FIG. 2 - temperature change over time in a burn patient with a woven electric heater turned on.
