В данное проспективное поисковое исследование были включены 90 пациентов (71 мужчина и 19 женщин, медиана возраста 62 (47-79) года), наблюдавшихся в отделении общей ангионеврологии Научного центра неврологии (Москва) за период с апреля 2015 г. по март 2016 г. У всех больных с помощью ультразвукового исследования сонных артерий был установлен атеросклеротический стеноз КС, измеренный по алгоритму исследования ECST [European Carotid Surgery Trialist's Collaborative Group. Стандартизация протокола измерений позволяет минимизировать возможную погрешность измерений. Результаты ультразвукового исследования структуры АСБ сопоставлялись с результатами морфологического исследования АСБ, удаленных при КЭЭ. Для оценки морфологической структуры АСБ в целом и отдельных ее участков, тех, для которых интенсивность сигнала была установлена до оперативного вмешательства, каждая бляшка разрезалась на блоки толщиной 0,3-0,5 см в плоскости, перпендикулярной продольной оси артерии. С каждого блока, залитого в парафин, получали серийные срезы толщиной 5-6 мкм, которые окрашивали гематоксилином и эозином по методу ван Гизона. Для выявления эластических волокон, соединений железа, извести и фибрина в бляшках срезы дополнительно окрашивались по методам Вейгерта, Перльса, Коссы и Шуенинова соответственно. При микроскопическом исследовании структуры каждой АСБ оценивалась по 24 компонентам и процессам: очагам атероматоза, фиброза, кальциноза, отека и некроза волокнистых структур, наличию липофагов, вновь образованных сосудов, очагов кровоизлияний, истончению и изъязвлению покрышки бляшки, тромбам на ее поверхности и др. Определялись соотношение в срезах АСБ площади очагов атероматоза, фиброза и кальциноза, а также количество (степень выраженности) других компонентов (малое, умеренное, большое). Статистическая оценка однородности сравниваемых групп проводилась с применением критерия Краскела-Уоллиса (с последующим post-hoc-анализом). To assess the morphological structure of the CRS as a whole and its individual sections, those for which the signal intensity was established prior to surgery, each plaque was cut into blocks 0.3-0.5 cm thick in a plane perpendicular to the longitudinal axis of the artery. From each block embedded in paraffin, serial sections of 5–6 µm thick were obtained, which were stained with hematoxylin and eosin by the method of van Gieson. To identify elastic fibers, compounds of iron, lime and fibrin in plaques, sections were additionally stained using Weigert, Perls, Kossa and Shueninov respectively. Microscopic examination of the structure of each ASB was assessed by 24 components and processes: foci of atheromatosis, fibrosis, calcification, edema and necrosis of fibrous structures, the presence of lipophages, newly formed blood vessels, foci of hemorrhage, thinning and ulceration of the plaque tire, thrombus on its surface and others. the ratio in ASB sections to the area of atheromatosis, fibrosis and calcification foci, as well as the amount (severity) of other components (small, moderate, large). Statistical evaluation of the homogeneity of the compared groups was carried out using the Kruskal-Wallis criterion (with subsequent post-hoc analysis). For calculations, the statistical package SAS 9.4 was used.
Атеросклеротический стеноз КС у одной группы исследованных пациентов протекал симптомно (симптомные пациенты), у другой - асимптомно (асимптомные пациенты). К асимптомным пациентам мы относили тех пациентов, у которых не было анамнестических данных за наличие нарушений мозгового кровообращения в системе стенозированной ВСА в течение последних 6 месяцев, а также на ипсилатеральной (по отношению к стенозу) стороне головного мозга отсутствовали клинические симптомы церебральной или ретинальной ишемии. Пациенты, перенесшие за последние полгода те или иные формы ишемического НМК (в рассматриваемом сосудистом бассейне), расценивались как симптомные. Пациентов с асимптомным течением (среди исследованных 90 человек) было более чем в 2 раза больше, в сравнении с пациентами с симптомным течением: 62 (69%) к 28 (21%). Это соотношение среди исследованных пациентов в известной степени оценивает соответствующее соотношение и в популяции людей с атеросклеротическим стенозом КС в целом.Atherosclerotic stenosis of the COP in one group of patients studied was symptomatic (symptomatic patients), in the other - asymptomatic (asymptomatic patients). Asymptomatic patients, we attributed those patients who did not have anamnestic data for the presence of cerebral circulatory disorders in the stenotic ICA system for the last 6 months, as well as for the ipsilateral (in relation to stenosis) side of the brain, there were no clinical symptoms of cerebral or retinal ischemia. Patients who have undergone some form of ischemic NMC (in the vascular basin under consideration) over the past six months were regarded as symptomatic. There were more than 2 times more patients with asymptomatic course (among 90 people studied), compared with patients with symptomatic course: 62 (69%) to 28 (21%). This ratio among the studied patients to a certain extent assesses the corresponding ratio in the population of people with atherosclerotic stenosis of the CS as a whole.

С целью определения количественных ультразвуковых значений интенсивности от определенных компонентов АСБ в 37 из них микроскопически исследовано от 1 до 4 участков (всего 67 участков размером до 10 мм).In order to determine the quantitative ultrasonic intensity values from certain components of the ABS in 37 of them microscopically investigated from 1 to 4 sections (a total of 67 sections of size up to 10 mm).
Анализ микроскопического исследования 67 участков размером до 10 мм АСБ, выбранных для количественной ультразвуковой оценки, позволил установить, что в 29 из них были выявлены очаги атероматоза, в том числе в 17 - очаги с частичным кальцинозом жиро-белкового детрита, в 12 - очаги с многочисленными кристаллами холестерина среди атероматозных масс. В 34 участках были выявлены очаги фиброзной ткани, при этом в 6 участках содержались очаги с диффузно расположенными липофагами и в 5 участках с диффузно расположенными сосудами, в 13 - большие их скопления, а в 10 участках очаги фиброза наряду с отдельными липофагами и/или сосудами определялись небольшие кальцификаты. Остальные 4 участка представляли собой крупные очаги кальциноза.
