Abstract
Description
The invention relates to medicine, namely to cardiac surgery, and can be used to provide access to different surfaces of the heart during surgeries on its surface, such as coronary artery bypass surgery. The method allows not to use for this purpose an additional assistant and not to use special expensive devices, such as vacuum stabilizers of the myocardium of various designs. At the same time, the exposure of the most "uncomfortable" areas of the heart, such as the posterior interventricular artery or the circumflex artery basin, remains quite satisfactory and allows for the implementation of a vascular anastomosis of any complexity.
There are a number of vacuum hardware systems to stabilize the myocardium.
17/02, 30.04.2002.Vacuum apparatus complexes for immobilization of the heart described in the applications for the invention: PCT WO 01/17437 A2; АВВ 17/02, 03/15/2001; PCT WO 00/62680 A1 A61B 17/02, 10.26.2000; PCT WO 00/15119 Al A61B 17/02, 03/23/2000; RU 2216284 C1, А61В 17/02, 30.04.2002.
The closest to the proposed method is the method proposed by P. Sergeant et al. [12]. The method consists in suturing the posterior surface of the pericardium at the base of the heart between the lower pulmonary veins and then fixing the gauze band with a long turnstile. The tape fixed in this way is used to fix the heart in different positions.
These solutions have their drawbacks.
The use of vacuum systems requires, in the first place, additional expenditure on their purchase and maintenance. And the method of fixation of the myocardium with vacuum can injure the area of fixation of the myocardium.
Application of the method described by P. Sergeant et al. It also has its drawbacks. The tape fixation point is the suture on the posterior surface of the pericardium in the zone where it borders the esophagus. With a deeper imposition of this suture, it is possible to flush the wall of the esophagus and, further, since this suture is the fulcrum when turning the heart, it is possible that the integrity of the esophagus is disturbed. Also for the imposition of this suture is required to turn the heart, that with unstable hemodynamics in a coronary patient is not always feasible.
Objectives: to ensure optimal exposure of the operated areas of the heart without the involvement of an additional assistant or the use of additional devices.
For better understanding, the method is described in Figures 1-4, where FIG. 1 - venue of the tape (1) for fixing the heart between the inferior vena cava (2) and the right lower pulmonary artery (3) in the oblique sinus region (4); FIG. 2 - tape position (1) when accessing the back surface of the heart (on the heart model); FIG. 3 - tape position (1) with access to the lateral surface of the heart (on the heart model); FIG. 4 - intraoperative photo-position of the tape (1) with access to the lateral surface of the heart.
The essence of the invention is the use of gauze tape not less than 1 cm wide, held in the hole, formed manually or instrumentally, between the inferior vena cava and the right inferior pulmonary vein, fixed on one side, while the other part of it removes and fixes the desired heart region. In this case, the point of support for the gauze loop is the inferior vena cava with the venous cannula installed in it. The venous cannula is installed in the inferior vena cava to connect the heart-lung machine. Thus, the foothold for the gauze loop is reinforced by the venous cannula, which minimizes the risk of damage to both the vena cava and adjacent structures.
The technical result consists in accurate and easy fixation of the heart in the required position and providing the exposure of the region of interest to the heart area that is necessary for performing the vascular anastomoses. The use of this method allows to reduce the number of surgeons involved in the operation, as well as to avoid the use of expensive myocardial stabilizers.
The method was tested on 500 patients who underwent coronary artery bypass surgery under cardiopulmonary bypass.
