Abstract
Description
The invention relates to medicine, namely to dentistry, has a therapeutic and prophylactic orientation: anti-inflammatory, remineralizing, wound healing effect, positive effect with hyperesthesia and hyposalivation.
95-97%.Dental diseases occupy the third place (20-25% in terms of circulation) among the general morbidity of the population of the Russian Federation and are the cause of deterioration of health in general, and also reduce the quality of life of the population. According to the WHO, the prevalence of pathology of hard tissues of teeth and oral mucosa currently reaches 95-97%.
The main and most affordable method of prevention and comprehensive treatment of dental diseases is the use of individual oral hygiene products.
Known toothpaste containing the proteolytic enzyme papain, lactoperoxidase and lactulose for the destruction of pathogenic microflora, hydroxyapatite, aminofluoride, silicone quaternium-16 or guar hydroxypropyltrimmonium chloride, or polyquaternium, cross-linked polymer or glycerin hydroxymethimer or camperpolymerihydroxymerihydromeric, maltodextrin crosspolymer, hydroxypropyl beta-cyclodextrin, lysine ursolate, oxyproline oleanolate, emulsifier, preservative, flavor, sorbitol l 70% and demineralized water (patent RU 2527343; IPC A61K 8/66, A61K 8/21, A61K 8/24, A61Q 11/00; 2014).
However, the disadvantages of the known toothpaste are, firstly, the complex multicomponent composition, and secondly, the instability of the enzyme complex during prolonged storage of finished forms even during the expiration date, and, as a result, a decrease in the therapeutic effect.
Known toothpaste containing hydroxyapatite with a crystal size of 30-40 nm × 100-200 nm in the form of a 10.0-20.0% aqueous paste, L-arginine and cetrimide, monohydric alcohol and distilled water as an antiseptic component (patent RU 2467739; IPC
However, the use of L-arginine in a known composition can lead to digestion, a decrease in blood pressure, nausea, and weakness.
Closest to the proposed solution is SPLAT BIOCALCIUM toothpaste, which contains active ingredients: hydroxyapatite, Calcis mineral complex isolated from eggshell, papain, polydon, omega-3 fatty acids, and sodium bicarbonate (http: // glamlemon .ru / krasota-i-zdorovie / 1917-zubnaya-pasta-splat-sostav.html). As auxiliary components, the paste contains cellulose gum, sorbitol, PEG-8, sodium lauryl sulfate, sodium methyl paraben, sodium saccharin, limonene as a flavoring agent, silicon dioxide as an abrasive, titanium dioxide as a pigment, and water.
The known composition has a remineralizing and whitening effect, reduces hyperesthesia, plaque formation, but does not have anti-inflammatory and wound healing effects.
Thus, the authors were faced with the task of developing the composition of therapeutic and prophylactic toothpaste, which, along with remineralizing effect and increased resistance of tooth enamel, reducing hyperesthesia, also had anti-inflammatory and wound healing effects.
%):The problem is solved in the composition of the toothpaste containing hydroxyapatite, papain, silicon dioxide, titanium dioxide, a thickener, a moisturizer, a surfactant, characterized in that it contains sodium carboxymethyl cellulose as a thickener, and 1,2-propanediol as a moisturizer , as a surfactant - cocamidopropyl betaine, and additionally contains silicon glycerolates of the composition Si (C 3 H 7 O 3 ) 4 ⋅ 6C 3 H 8 O, in the following ratio of components (wt.%):
Currently, from the patent and scientific and technical literature is not known therapeutic and prophylactic toothpaste, characterized by the qualitative and quantitative composition of the components within the claimed limits.
The composition of the proposed paste used biologically active silicon glycerols of Si composition (C 3 H 7 O 3 ) 4 ⋅ 6C 3 H 8 O 3 , non-toxic, exhibiting high penetrating activity in the body tissue and facilitating the penetration of medicinal or biologically active additives into them (patent RU 2255939; IPC C07F 7/04, A61K 47/30; 2005). Silicon glycerolates have anti-inflammatory, wound healing and regenerative effects. In addition, silicon glycerolates provide a positive effect during hyposalivation, as well as the introduction of silicon into the body, which is one of the micronutrients necessary to ensure the normal functioning of the human body.
Also, crystalline hydroxyapatite with a particle size of 20-80 nm is used as the active component of the composition. Hydroxyapatite - a calcium-phosphorus-containing compound of composition Ca 10 (PO 4 ) 6 (OH) 2 is the closest in chemical composition to bone tissue and tooth enamel, has a pronounced osteoplastic effect, activating the processes of reparative osteogenesis. As shown by experimental studies conducted by the authors, the content of hydroxyapatite is less than 1.00 mass. % - does not provide a pronounced remineralizing effect, more than 2.00 mass. % - impractical, as it does not lead to a further increase in remineralizing properties. It should be noted that the use of active components in the paste in the nanoscale range is preferable, since this form is characterized by improved biocompatibility with tooth tissue.
Amorphous finely divided silica, which is also part of the paste, is an abrasive material that polishes teeth without excessive abrasion of enamel. Silicon dioxide also acts as a filler and structure-forming agent, gives a uniform, thixotropic consistency to the paste. When using silicon dioxide less than 5.00 mass. % insufficient enamel cleaning is observed, and rheological characteristics are also deteriorating. The use of more than 10.00 mass. % silicon dioxide significantly thickens the paste, which can damage the tooth enamel.
1,2-propanediol is used as a moisturizer - it prevents the paste from drying out and gives a feeling of moisture in the oral cavity. In addition, 1,2-propanediol gives the paste plasticity and stability. 1,2-propanediol is used in the paste in the range from 3.00 to 5.50 mass. %, which provides the necessary hydration in the oral cavity.
Sodium carboxymethyl cellulose is a thickener that gives the necessary consistency to toothpaste. The content of sodium carboxymethyl cellulose in the range of 0.20-0.80 mass. % provides the necessary rheological properties (consistency), shelf life and stability of the paste.
Papain - a proteolytic enzyme of plant origin, has a cleansing, detoxifying and whitening effect. Papain breaks down the protein base of plaque and exfoliates it, which allows you to remove it without the intervention of coarse abrasives, thus providing a soft and effective cleansing of teeth and gums from food debris and plaque. Papain also has an immunostimulating and antibacterial effect. In addition, papain effectively restores the tone and elasticity of the oral mucosa, moisturizes it, and promotes rapid regeneration. The papain content is less than 0.01 mass. % does not have a sufficient positive effect, more than 1.00 mass. % may cause unwanted side effects, such as allergic reactions.
Cocamidopropyl betaine is an amphoteric surfactant that has a multifunctional effect: solubilizing, dispersing, wetting, foaming. It determines the stability of the paste, gives it the best cleansing properties. The use of cocamidopropyl betaine is less than 0.10 mass. % insufficiently effective, more than 0.20 mass. % - worsens the consistency of the paste.
Titanium dioxide is a white pigment that improves the appearance of toothpaste. Titanium dioxide is used in a concentration range from 0.10 to 0.20 mass. % Using less titanium dioxide does not give the toothpaste the necessary white color; the use of a larger amount is impractical because it does not improve the appearance of the paste, but worsens the rheological characteristics.
It should be noted that the proposed therapeutic and prophylactic paste does not contain fluoride and a preservative.
Thus, the qualitative and quantitative composition of the components of therapeutic and prophylactic toothpaste, which ensures pharmaceutical and pharmacological compatibility and, as a result, a high therapeutic and prophylactic effect, is the result of studies conducted by the authors to achieve synergistic action of the combination of components.
Исследование реминерализующих свойств по уменьшению шероховатости зубной эмали (in vitro)Study of remineralizing properties to reduce the roughness of tooth enamel (in vitro)
Roughness measurement of tooth enamel was carried out on the example of enamel sections of permanent teeth with dimensions of 4 mm × 5 mm and a thickness of 2 mm, removed according to orthodontic indications in patients aged 18-30 years. The obtained samples after appropriate processing were aged for 1 month in the inventive therapeutic and prophylactic toothpaste (composition according to example 5). The studies were carried out in comparison with the prototype paste (remineralizing paste SPLAT BIOCALCIUM) under the same conditions.
Для цифровой обработки и анализа данных использовали программу SPIP (SPIP, Дания).The change in the surface relief of the tooth enamel before and after exposure in the tested pastes was determined by the method of atomic force microscopy (AFM) using an Asylum MFP3D scanning probe microscope. Visualization of the surface of the samples using AFM was performed with a resolution of 256 * 256 and 512 * 512 pixels. Igor Pro (Asylum Research, USA) was used to obtain images of the surface topography of the samples and to preserve data export. For digital data processing and analysis, the SPIP program (SPIP, Denmark) was used.
The topography and three-dimensional image of the surface relief of tooth enamel before and after immersion in the inventive paste and the prototype paste are shown in FIG. 1 and 2, respectively.
Для количественной оценки были вычислены среднеарифметическое и среднеквадратичное отклонение от среднего уровня шероховатости эмали в пяти точках каждого исследуемого образца (таблица 1).For a quantitative assessment, the arithmetic mean and standard deviation from the average level of roughness of the enamel at five points of each test sample were calculated (table 1).
S a - arithmetic mean deviation from the average level
S q - standard deviation from the average level
Как следует из таблицы, изменение шероховатости поверхности исследуемых средств (уменьшение на 14-17%) значимо не отличается; при этом следует отметить, что полученные значения не отражают скорости процесса реминерализации, а являются результатом 1-месячной выдержки в исследуемых пастах, что, вероятно, отвечает уровню равновесия (насыщения).As follows from the table, the change in the surface roughness of the investigated funds (a decrease of 14-17%) does not significantly differ; it should be noted that the obtained values do not reflect the rate of the remineralization process, but are the result of a 1-month exposure in the tested pastes, which probably corresponds to the level of equilibrium (saturation).
2.The same samples of tooth enamel before and after exposure (1 month) in the inventive paste and in the prototype paste were examined by scanning electron microscopy using a Quanta 200 FEI SEM scanning electron microscope: a quantitative change in the chemical composition (Si, Ca, P) of the enamel surface was performed using the Pegasus elemental analysis system at 300 kV and a resolution of more than 5 nm. The results are shown in table 2.
*р ≤0,05 - результаты статистически значимы по сравнению с прототипом и контролем* p ≤0.05 - the results are statistically significant compared with the prototype and control
The table shows that there is a statistically significant increase in the silicon content in the inventive paste compared with the paste-prototype and control, which confirms the advantages of the claimed composition in terms of enamel hardening.
The microhardness of tooth enamel was measured using the Vickers method using the PMT-3 device. This method is based on measuring the length of the diagonal of the imprint obtained by indenting the diamond pyramid (angle at the apex of 136 °) into the sample. The linear dimensions of the prints were measured by microphotographs.
Microhardness was calculated by the formula:
НВиккерс=9,8×1854×Р/С2 (МПа),N Vickers = 9.8 × 1854 × P / C 2 (MPa),
где Р - нагрузка (г), С - диагональ отпечатка (мм)where P is the load (g), C is the diagonal of the print (mm)
3.The microhardness values of tooth enamel after processing the inventive paste and paste prototype are shown in table 3.
The microhardness index of tooth enamel samples after processing the inventive paste increased ~ 1.4 times compared with the prototype paste. The study involved a group of 32 somatically preserved volunteers aged 18 to 20 years. All examined were divided into three groups in accordance with the classification of levels of resistance to caries VB Not enough. A group of people with an average level of resistance were 12 people. A group of people with low resistance were 11 people. A group of people with a very low level of resistance were 9 people.
7-14 days before the start of the study, participants underwent professional oral hygiene and training in the rules of individual hygiene. To clean the surface of the teeth, the Leonard method and a toothbrush with a Medium bristle hardness of one manufacturer were recommended. No other forms of hygienic oral care were allowed during the study period. Also, in accordance with the manufacturer's recommendations, it was necessary to brush the teeth with therapeutic and prophylactic paste on their own at home at least twice a day for 2-3 minutes.
All patients underwent a comprehensive dental examination, which included collecting an anamnesis (determination of complaints, an anamnesis of life, an anamnesis of a detected disease), external examination, examination of the oral cavity, identification of pathology of hard tissues of teeth, malocclusion.
